This claim arises from a motor vehicle accident which occurred on January 30, 2001, at
3:00 PM, at 11th Avenue, between Cranford Place and 18th Street, in the City of Greeley, State
of Colorado. Unknown employee of Weld County Public Works, operating a county truck, had a
tool box slide off the truck and hit the insured's parked vehicle, causing damage to the vehicle of
the insured, George McCarthy. The collision caused extensive damage to the State Farm Mutual
Automobile Insurance Company insured vehicle.
The insured vehicle was a 1997 Nissan Maxima GLE.. The property damages were

$3779.10 plus the insured's deductible of$500.00, plus rental of$400.00 and out of pocket
expenses by insured for$765.00, for a total of$5444.10.
