RE: APPROVE AMENDMENT TO AGREEMENT TO ADMINISTER GRANT FUNDS AND
AUTHORIZE CHAIR TO SIGN - UNITED WAY OF WELD COUNTY
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to the Agreement to
Administer Grant Funds between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Public Health
and Environment, and United Way of Weld County, commencing September 15, 2001, and
ending November 15, 2001, with further terms and conditions being as stated in said
agreement, and
WHEREAS, after review, the Board deems it advisable to approve said agreement, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Amendment to the Agreement to Administer Grant Funds
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Public Health and
Environment, and United Way of Weld County be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 17th day of December, A.D., 2001, nunc pro tunc September 15,
2001.

SUBJECT: Amendment to Agreement with United Way
of Weld County
Enclosed for the Board review and approval is an amendment to the agreement between United
Way of Weld County and the Board of Weld County Commissioners on behalf of the Weld
County Department of Public Health and Environment(WCDPHE).
In the provisions of the original agreement, WCDPHE agreed to administer a program to provide
outreach to Weld County residents to inform and assist them in applying for the Child Health
Plan Plus (CHP+) insurance program. The time period of the agreement was originally from
September 15, 2000 through September 15, 2001. This amendment will extend the ending date
of the agreement to November 15, 2001.
I recommend your approval of this amendment.

AMENDMENT TO AGREEMENT TO ADMINISTER GRANT FUNDS
THIS AMENDMENT is to the Agreement to Administer Grant Funds between United
Way of Weld County and the Board of County Commissioners for Weld County, Colorado, on
behalf of the Weld County Department of Public Health and Environment, executed January 29,
2001. Pursuant to the provision in paragraph 16 of the said Agreement which provides for •
modification or amendment to the Agreement,paragraph 3 of that Agreement is hereby amended
to read:
3. Term. The term of this Agreement is from September 15, 2000 through
November 15, 2001.
All other terms and provisions of the said Agreement to Administer Grant Funds shall
remain in full force and effect for the term of that Agreement.
IN WITNESS WHEREOF,the parties hereto have set their hands and seals this rrik day
of December, 2001, nunc pro tunc November 15, 2001.
