RE: APPROVE CHANGE ORDER LETTER#1 FOR CHLAMYDIA PROJECT TASK
ORDER AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Change Order Letter#1 for the
Chlamydia Project Task Order between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Weld County Department
of Public Health and Environment, and the Colorado Department of Public Health and
Environment, commencing June 1, 2001, and ending December 31, 2001, with further terms
and conditions being as stated in said Change Order Letter#1, and
WHEREAS, after review, the Board deems it advisable to approve said Change Order
Letter#1, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Change Order Letter#1 for Chlamydia Project Task Order
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby
is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said Change Order Letter#1.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 26th day of March, A.D., 2001.

State Fiscal Year 2001 -2002,Task Order Change Order Letter Number 1 Contract Routing Number 01-00180
Pursuant to Part F.3.of the Master Contract with contract routing number 00 FAA 00008 and paragraph C.3 of the
Task Order with contract routing number 01-00090 and contract encumbrance number EPI0100090, hereinafter
referred to as the "Original Task Order"(a copy of which is attached hereto and by this reference incorporated herein
and made a part hereof)between the State of Colorado,Department of Public Health and Environment and
WELD COUNTY DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT, for the term from June 1,
2001,through December 31, 2001,the parties agree that the maximum amount payable by the State for the eligible
services referenced in paragraph C_1 of the Original Task Order is decreased by FIVE HUNDRED FORTY TWO
DOLLARS, ($542.00)for a new total financial obligation of the State of SEVEN THOUSAND TWO HUNDRED
FIFTY FOUR DOLLARS, ($7,254.00). The revised budget below is incorporated herein by this reference and
made a part hereof The first sentence in paragraph C.1 of the Original Task Order is hereby modified accordingly.
All other terms and conditions of the Original Task Order are hereby reaffirmed. This amendment to the Original
Task Order is intended to be effective as of June 1,2001. However, in no event shall this amendment be deemed
valid until it shall have been approved by the State Controller or such assistant as he may designate.
Please sign,date, and return all 4 originals of this Task Order Change Order Letter by May 1,2001, to the attention
of: Lisa Ellis,Colorado Department of Public Health and Environment,4300 Cherry Creek Drive South,
Denver,Colorado 80246,Mail Code: DCEED-A3. One original of this Task Order Change Order Letter will be
returned to you when fully approved.

SUBJECT: Change Order Letter#1 to the Chlamydia
Project Task Order
Enclosed for Board review and approval is Change Order Letter#1 to the Chlamydia Project task
order between the Colorado Department of Public Health and Environment and Weld County. In
November 2000, you approved the original task order that provided funding in the amount of
$7,996 for the testing and treatment of Chlamydia. Of this funding, $542 was to be used to
purchase Azithromycin to treat this disease.
Since the signing of the task order, CDPHE has determined they can purchase the medication at a
much lower price than local health departments. Therefore, they are processing this change order
letter to reduce the total reimbursement of the task order by$542, and they in turn will purchase
the medication and dispense it directly to Weld County Department of Public Health and
Environment at no cost. The revised budget for this task order will be $7,254.
I recommend your approval of this change order letter.
