Subject: Contract for Purchase of Transportation Services by Parent Child
Center Food Service Program -
Attached is an agreement to contract transportation services from the Weld County
Division of Human Resources. The Parent Child Center Food Service Program is in
need of a vehicle and staff person to transport commodity foods from the USDA
Denver warehouse to their LaSalle address. These services are needed on an every
other month basis.
The Food Service Program has agreed to pay for the transportation services at
the rate of $6.03 per hour for the driver utilized and .65Q per mile driven. In
addition, the Food Service Program will compensate Human Resources an additional
20% of the combined cost of the driver and mileage to cover the costs of admini -
stration, overhead, gas, oil , repair, maintenance, and insurance.
It should be noted that no new staff or vehicles will be needed on the part of
Human Resources in order to honor this agreement.

Contract for Purchase of Services Agreement
This Agreement made and entered into this 30th day of July
1984, by and between the Weld County Division of Human Resources, here-
inafter referred to as "Human Resources," and the Parent Child Center
Food Service Program, hereinafter referred to as "Food Service Program."
WI TN ES SETH:
WHEREAS, Human Resources administers a Commodity Supplemental Food
Program, and a Transportation Program, and
WHEREAS, the Food Service Program will be receiving commodity items
in Weld County, and
WHEREAS, the Food Service Program will be required to transport the
commodities from the Distribution Commodity Warehouse in Denver to Weld
County, and
WHEREAS, Human Resources has a fleet to vehicles with the capacity
to meet a variety of transportation needs, and
WHEREAS, the Food Service Program is desirous of utilizing Human
Resources ' staff and vehicles for the express purpose of transporting
the commodity items from the Denver warehouse to 123 N. 1st Street,
LaSalle, Colorado, in Weld County.
NOW THEREFORE, the parties hereto mutually agree as follows:
1. Human Resources will provide an appropriate staff person and
vehicle for the express purpose of transporting commodity
foods for the Food Service Program from the Denver warehouse
location to 123 N. 1st Street, LaSalle, Colorado, in Weld
County.
2. Human Resources will make one (1) trip every other month for
the purpose of transporting the commodity foods for the Food
Service Program.
3. A record of the date of usage, hours worked, and miles
traveled will be kept by Human Resources.
4. Human Resources agrees to transport the commodities for the
rate of $6.03 per hour for the staff person utilized and .65Q
per mile driven.
5. The Food Service Program agrees to compensate Human Resources
at the rate identified in item #4 and additionally agrees to
compensate Human Resources an additional 20% of the combined
cost of staff person and mileage to cover the costs of
administration, overhead, gas, oil , repair, maintenance, and
insurance.
6. Human Resources agrees to bill the Food Service Program on an
every other month basis in coordination with delivery.
7. Human Resources and the Food Service Program agree to review
the costs at the end of six (6) months. Necessary amendments
or adjustments would be made at that time.
8. All requests for additional transportation and staff for the
Food Service Program must be accompanied by a formal written
request and agreement to pay additional costs incurred, if
any.
9. It is agreed between the parties that either party hereto has
the right to terminate this Agreement within thirty (30) days
after service is instituted upon thirty (30) days written
notice given to the other party.
10. This Agreement may be amended with the written consent of both
parties.
IN WITNESS WHEREOF, the parties hereto have placed their hands as
of the date first written above.
