RE: APPROVE TASK ORDER RENEWAL LETTER#1 FOR EPIDEMIC AND
COMMUNICABLE DISEASE CONTROL PROGRAM AND AUTHORIZE CHAIR TO
SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Task Order Renewal Letter#1 for the
Epidemic and Communicable Disease Control Program between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Weld County Department of Public Health and Environment, and the Colorado Department of
Public Health and Environment, commencing July 1, 2001, and ending June 30, 2002, with
further terms and conditions being as stated in said task order renewal letter, and
WHEREAS, after review, the Board deems it advisable to approve said task order
renewal letter, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Task Order Renewal Letter#1 for the Epidemic and
Communicable Disease Control Program between the County of Weld, State of Colorado, by
and through the Board of County Commissioners of Weld County, on behalf of the Weld County
Department of Public Health and Environment, and the Colorado Department of Public Health
and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said task order renewal letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 20th day of June, A.D., 2001.

State Fiscal Year 2001-2002, Task Order Renewal Letter Number 1, Contract Routing Number 02-00059
Epidemic&Communicable Disease Control Program
Pursuant to Part F.5.of the Master Contract with contract routing number 00-FAA-00008 and paragraph E.2 of the Task Order
with contract routing number 01-00076 and contract encumbrance number EPI0100076,hereinafter referred to as the "Original
Task Order"(a copy of which is attached hereto and by this reference incorporated herein and made a part hereof)between the
State of Colorado,Department of Public Health and Environment and the BOARD OF COUNTY COMMISSIONERS
OF WELD COUNTY(a political subdivision of the state of Colorado),for the use and benefit of the WELD COUNTY
DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT, for the renewal term from July 1,2001,through June
30, 2002,the parties agree that the maximum amount payable by the State for the eligible services referenced in paragraph B
of the Original Task Order is increased by EIGHTEEN THOUSAND FIFTY NINE DOLLARS,($18,059.00) for a new total
financial obligation of the State of SEVENTY THOUSAND EIGHT HUNDRED EIGHTY ONE DOLLARS, ($70,881.00).
The budget, which is outlined below, is incorporated herein by this reference and made a part hereof. The first sentence in
paragraph C of the Original Task Order is hereby modified accordingly. All other terms and conditions of the Original Task
Order are hereby reaffirmed. This amendment to the Original Task Order is intended to be effective as of July 1,2001.
However, in no event shall this amendment be deemed valid until it shall have been approved by the State Controller or such
assistant as he may designate.
Please sign,date, and return all 4 originals of this Task Order Renewal Letter by June 15,2001,to the attention of: Lisa Ellis,
Colorado Department of Public Health and Environment,4300 Cherry Creek Drive South,Denver,Colorado 80246,
Mail Code: DCEED-A3. One original of this Task Order Letter will be returned to you when fully approved.

SUBJECT: Task Order for West Nile Virus Funds
Enclosed for Board review and approval is a Task Order Renewal Letter from the
Colorado Department of Public Health and Environment for continued monitoring and
surveillance of the West Nile Virus. The original Task Order (attached) was signed by
the Commissioners last summer.
The Renewal Letter increases the original Task Order by $18,059. Of this amount,
$10,653 is for Personnel, $6,000 for supplies, and $1,406 for indirect costs. The term
for this Renewal Letter is July 1, 2001, through June 30, 2002.
I recommend your approval of this Task Order Renewal Letter.
