SUBJECT: AMUSR-488 Mobile Premix Concrete,
Inc., Department of Planning Services
Environmental Health Services has reviewed this proposal for this Open Pit and Materials
Processing plant. We have no objections to the proposal,however, we do recommend that the
following conditions be part of any approval:
We recommend that the following requirements be met prior to allowing the plat to be recorded:
1. Submit a dust abatement plan to the Environmental Health Services, Weld County
•
Department of Public Health& Environment, for approval prior to operation.
2. If applicable,the applicant shall amend the existing Emission Permit for the modification
in the operation if the Colorado Department of Public Health and Environment
determines that such a modification represents a significant change in emissions or
production.
3. All septic systems located on the property shall have appropriate permits from the Weld
County Department of Public Health& Environment. The Environmental Health
Division of the Weld County Department of Public Health and Environment was unable
to locate a septic permit,for the existing septic system that served a residence (but has
since burnt down) located in the northeast part of this USR. Any existing septic system
which is not currently permitted through the Weld County Department of Public Health&
Environment will require an I.S.D.S. Evaluation prior to the issuance of the required
septic permit. In the event the system is found to be inadequate,the system must be
brought into compliance with current I.S.D.S. regulations.
4. If necessary,the applicant shall amend the existing CPDS Permit from the Water Quality
Control Division of the Colorado Department of Health.
,/ 5. The applicant shall submit a waste handling plan, for approval,to the Environmental
Health Services Division of the Weld County Department of Public Health&
Environment. The plan shall include at a minimum,the following:

1) A list of wastes which are expected to be generated on site (this should include
expected volumes and types of waste generated).
2) A list of the type and volume of chemicals expected to be stored on site.
3) The waste handler and facility where the waste will be disposed (including the
facility name, address, and phone number).
We recommend that the following requirements be incorporated into the permit as"development
standards":
1. The facility shall operate in accordance with the approved dust control plan. The facility
shall have sufficient equipment available to implement appropriate dust control.
Additional control measures shall be implemented as required by the Weld County Health
Officer.
2. All liquid and solid wastes(as defined in the Solid Wastes Disposal Sites and Facilities
Act, 30-20-100.5, C.R.S., as amended) shall be stored and removed for final disposal in a
manner that protects against surface and groundwater contamination.
3. No permanent disposal of wastes shall be permitted at this site. This is not meant to
include those wastes specifically excluded from the definition of a"solid waste"in the
Solid Wastes Disposal Sites and Facilities Act, 30-20-100.5, C.R.S., as amended.
4. Waste materials shall be handled, stored, and disposed in a manner that controls fugitive
dust,blowing debris, and other potential nuisance conditions.
5. Fugitive dust shall be controlled on this site.
v 6. This facility shall adhere to the maximum permissible noise levels allowed in the
Industrial Zone as delineated in 25-12-103 C.R.S., as amended.
7. The applicant shall remove, handle, and stockpile overburden, soil, sand and gravel from
the facility area in a manner that will prevent nuisance conditions.
, 8. The facility shall utilize the existing public water supply. (Left Hand Water District)
9. If applicable, the applicant shall obtain a stormwater discharge permit from the Colorado
• Department of Public Health& Environment, Water Quality Control Division.
10. The operation shall comply with all applicable rules and regulations of the Colorado
Division of Minerals and Geology.

/ 11. All fuel tanks, septic tanks,temporary buildings, and any other hazardous items that
might wash away during flooding shall be securely anchored and adequately flood
proofed to avoid creation of a health hazard.
12. Adequate handwashing and toilet facilities shall be provided for employees.
13. The operation shall comply with all applicable rules and regulations from the Colorado
Department of Labor and Employment, Oil Inspection Section.
14. The operation shall comply with the Occupational Safety and Health Act(OSHA).
15. The operation shall comply with the Mine Safety and Health Act(MSHA).
16. Portable toilets may be utilized on sites that are temporary locations of the working face
and portable processing equipment, etc. for up to six months at each location.
17. Any vehicle washing area(s) shall capture all effluent and prevent discharges from drum
washing and the washing of vehicles in accordance with the Weld County Code, the
Rules and Regulations of the Water Quality Control Commission, and the Environmental
Protection Agency.

Weld County Public Works has reviewed this proposal; this project falls primarily under the purview of the se by
Special Review Standards, Weld County Code, Chapter 23, Article II, Division 4, Section 23. Our comments and
requirements are as follows:
COMMENTS:
WCR 20.5 is designated on the 1-25 Mixed Use Development Area Structural Plan Map 2.2, Structural Transportation
Network, as a four-lane undivided-secondary road, which requires a 100-foot right-of-way at full build out. There is
presently a 60-foot right-of-way. This road is maintained by Weld County. Pursuant to the definition of SETBACK in
the Weld County Code,Chapter 23,Article Ill,Section 23-3-50,the required setback is measured from the future right-
of-way line.
Both WCR 1 and WCR 7 are also designated on the 1-25 Mixed Use Development Area Structural Plan Map 2.2 ,
Structural Transportation Network,as arterial four-lane with painted medium-secondary road,which requires a 100-foot
right-of-way at full build out.
The average daily traffic count (ADT)for WCR 25 reflect 1,726 vehicles in a 24-hour period taken October 30, 1998.
WCR 1 (County Line Road)count reflects 3,604 taken in a 24-hour period December 29, 1999. WCR 7 count reflects
1,080 vehicles taken February 18, 1999.
The applicant currently has a Road Maintenance Improvements Agreement dated January 24, 1999. This agreement
is with multiple heavy haulers in the Renn area. The agreement includes utilizing the designated haul route set up for
the Renn area including WCR 20.5 between WCR 1 and WCR 7, WCR 7 between SH 119 and SH 52, WCR 18 west
of WCR 7, and the paved portion of WCR 5 at that intersection.
REQUIREMENTS:
The applicant shall dedicate an additional 20 feet of right-of-way adjacent to the subject property next to WCR 20.5 to
accommodate acceleration and deceleration lanes associated with the new access point on to WCR 20.5.
Traffic Impact Study: All new commercial developments with expected ADT>200 will be required to submit a traffic
study as part of the review process. This study shall include a description of the project, vehicle trip generation and
distribution,verification of adequate acceleration,deceleration and storage facilities at all impacted intersections,review
of bridge capacity, intersection level of service analysis for existing and proposed conditions and recommendation for
needed upgrades to transportation infrastructure to support the development.
Weld County Public Works will review your submitted traffic study. The traffic study needs to be submitted no later than
two weeks prior to scheduling with the rift ii,;„gCep i iui to allow our traffic engineer time to review and comment.

Off-Street Parking: The off-street parking spaces at the main office parking lot, including the access drive, shall be
surfaced with asphalt, concrete or equivalent, and shall be graded to prevent drainage problems.
The applicant shall adhere to all legal load limits for restricted bridges.
Storm Water Drainage: The applicant is indicating all drainage around the structures will be diverted into the settling
and de-watering ponds. Any discharge point shall be identified on the plat. This portion of the permit falls under the
jurisdiction of the State of Colorado/DMG.

Dear Ms. Lockman:
I have reviewed the submitted material pertaining to the Special Use Permit for open mining and
materials processing, located 2002 Weld County Road 20.5 (Case Number: tea AMUSR-488,
Case Name: Mobile Premix Concrete). The Fire District has no objections to the project if the
requirements of the District can be met.
Weld County has ratified the 1994 Edition of the Uniform Fire Code, within the territorial
boundaries of the Fire District and the activities of the mining operation will be covered by this
Code. The principal applications of the Code to a this type of operation will be for tank
installations for flammable or combustible liquids, fueling operations, and any use of hazardous
materials.
Tanks installed for flammable or combustible liquid storage or dispensing, either temporary or
permanent, need to be installed in accordance with the requirements of the Uniform Fire Code and
plans for the installation must be reviewed and approved by the Fire District prior to installation.
