SUBJECT: USR-1353 HS Gathering, LLC
Environmental Health Services has reviewed this proposal for an oil and gas support service facility (Natural gas
compressor station with associated inlet and outlet pipelines). We have no objections to the proposal, however, we
do recommend that the following conditions be part of any approval:
t/We recommend that the following requirements be met prior to allowing the plat to be recorded:
1. Submit an Air Pollution Emission Notice (A.P.E.N.)and Emissions Permit application to the Air Pollution
Control Division, Colorado Department of Health and Environment for emissions of criteria, hazardous or
odorous air pollutants. Sources of such emissions include but are not limited to the following: sandblasting
operations, mining, spray paint booths, dry cleaners, haul road traffic, composting, boilers and incinerators.
2. In the event the applicant will be disturbing greater than 5 acres at any given time, the applicant shall obtain a
stormwater discharge permit from the Water Quality Control Division of the Colorado Department of Public
Health and Environment.
We recommend that the following requirements be incorporated into the permit as"development standards":
1. All liquid and solid wastes(as defined in the Solid Wastes Disposal Sites and Facilities Act, 30-20-100.5,
C.R.S., as amended) shall be stored and removed for final disposal in a manner that protects against surface
and groundwater contamination.
2. No permanent disposal of wastes shall be permitted at this site. This is not meant to include those wastes
specifically excluded from the definition of a"solid waste" in the Solid Wastes Disposal Sites and Facilities
Act, 30-20-100.5, C.R.S., as amended.
3. Waste materials shall be handled, stored, and disposed in a manner that controls fugitive dust, blowing
debris, and other potential nuisance conditions.
4. Fugitive dust shall be controlled on this site.
5. This facility shall adhere to the maximum permissible noise levels allowed in the Industrial Zone as
delineated in 25-12-103 C.R.S., as amended.

6. All pesticides, fertilizer, and other potentially hazardous chemicals must be stored and handled in a safe
manner. Reference 25-8-205.5 Colorado Water Quality Control Act and Rules and Regulations thereof.
7. The facility shall utilize bottled water for drinking during construction of the project.
8. The facility shall provide water for employees upon completion of the construction at the adjacent

10. The facility shall comply with the Rules and Regulations of the Oil and Gas Commission.
11. The facility shall comply with the above Ground Storage Tank Regulations (7CCR1101-14)
12. Portable toilets shall be used during construction of the site and removed upon completion.
13. Bathroom facilities shall be available for employees upon completion of the construction at the
adjacent compressor site.

The Weld County Public Works Department has reviewed this proposal; this project falls primarily under the
purview of the Use by Special Review Standards, Weld County Code, Chapter 23, Article II, Division 4, Section
23. Our comments and requirements are as follows:
COMMENTS:
WCR 22 is designated on the Transportation Plan Map as a collector status road, which requires an 80-foot right-
of-way at full build out. There is presently a 60-foot right-of-way. This road is maintained by Weld County.
Pursuant to the definition of SETBACK in the Weld County Code, Chapter 23, Article III, Section 23-3-50, the
required setback is measured from the future right-of-way line.
The average daily traffic count (ADT) for WCR 22 between WCR 31 and WCR 37 reflects 937 vehicles. This
count was taken on September 21, 2000.
REQUIREMENTS:
The Weld County Public Works Department recommends approval of this application.
Future Right-of-Way: The applicant should reserve an additional ten feet of property parallel to WCR 22 right-of-
way for future build out.✓
Access: While visiting with the applicant on site, we agreed upon one main access to the facility. The main
access point has adequate sight distance in both directions and shall have a minimum width of at least 20 feet to
accommodate two-way traffic and any emergency vehicles that may need to access the site.
Off-Street Parking and Circulation: The off-street parking spaces at the main office parking lot, including the
access drive and unloading areas shall be surfaced with gravel, asphalt, concrete, or equivalent, and shall be
graded to prevent drainage problems.
Storm Water Drainage:The applicant will utilize collection ponds and silt fences as needed to control erosion both
during construction and final operation in accordance with a storm water discharge plan and permit. This portion
of the permit falls under the jurisdiction of the State of Colorado or Storm Water Discharge.

In consultation with the Natural Resources Conservation Service (NRCS) ,
we have reviewed the above referenced case and have the following
comments .
SOILS/LIMITATIONS : The application package contained information on the
soils found on the site derived from the "Soil Survey of Weld County,
Colorado" and is accurate . The soils are classified as Valent loamy
sand and Otero sandy loam. These soils are very sandy and caution
should be exercised in any disturbances . Permeability of both soils is
moderately rapid with moderate available water holding capacity.
PRIME FARMLANDS : These soils would not be considered as "prime
farmland" , even if they were irrigated.
SOIL EROSION POTENTIAL: The potential for water erosion is moderate to
high for the Otero soils but low for the Valent sands . The potential
for wind erosion is moderate to high.
OTHER: The applicant has contacted Ron Broda, Weld County Vegetation
Management Specialist, for assistance in developing a weed management
plan. A copy of that plan was included and appears to be adequate. The
revegetation plan looks specific, as well as accurate, however, some
tree plantings may also be advised as per previous recommendations . The
buffer on the south and east side is good and it might be good to extend
this along the north side along Weld County Road 22 .
The District welcomes the opportunity to review and comment on proposals
such as this . Conservation and wise use of our natural resources are

DECEMBER 30, 1997.
d. The following permitting/reporting requirements applied to pits constructed prior to December
30, 1997:
(2) An Application For Permit For Unlined Pit, Form 15 was required for the following:
A. Unlined production pits and special purpose pits in sensitive areas constructed
prior to July 1, 1995, and not closed by December 30, 1997.
B. Unlined production pits AND SPECIAL PURPOSE PITS outside sensitive
areas constructed after July 1, 1995 and not closed by December 30, 1997.
RECLAMATION REGULATIONS
1004. FINAL RECLAMATION OF WELL SITES AND ASSOCIATED PRODUCTION FACILITIES
a. Well sites. Upon the plugging and abandonment of a well, all pits, mouse and rat holes and
cellars shall be backfilled. aAll debris, abandoned gathering line risers and flowline risers, and
surface equipment SHALL BE removed WITHIN THREE (3) MONTHS OF PLUGGING A WELL.
. All access roads to plugged and abandoned wells and
associated production facilities shall be closed, graded and recontoured. Culverts and any other
obstructions that were part of the access road(s) shall be removed. Well locations, access roads
and associated facilities shall be reclaimed. As applicable, compaction alleviation, restoration, and
revegetation of well sites and access roads shall be performed to the same standards as
established for interim reclamation under Rule 1003. Material may be burned or buried on the
premises only with the prior written consent of the surface owner, and with prior written notice to the
surface tenant. Such burning or burial shall be subject to applicable state law.All such reclamation
work shall be completed within three (3) months on crop land and twelve (12) months on non-crop
land after plugging a well. The Director may grant an extension where unusual circumstances are
encountered, but every reasonable effort shall be made to complete reclamation before the next
local growing season.
FLOWLINE REGULATIONS
1103. ABANDONMENT
Each flowline abandoned in place must be disconnected from all sources and supplies of natural
gas and petroleum, purged of liquid hydrocarbons, depleted to atmospheric pressure, CUT OFF AT A
MINIMUM OF THREE(3)FEET BELOW GROUND SURFACE and sealed at the ends.
