RE: APPROVE TASK ORDER CHANGE ORDER LETTER#1 FOR EPSDT PROGRAM
AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Task Order Change Order Letter#1
for the EPSDT Program between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Weld County Department of
Public Health and Environment, and the Colorado Department of Public Health and
Environment, commencing July 4, 2000, and ending June 30, 2001, with further terms and
conditions being as stated in said task order change order letter, and
WHEREAS, after review, the Board deems it advisable to approve said task order
change order letter, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Task Order Change Order Letter#1 for the EPSDT Program
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby
is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said task order change order letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 20th day of June, A.D., 2001, nunc pro tunc July 1, 2000.

State Fiscal Year 2000-2001, Task Order Change Order Letter Number 1, Contract Routing Number 01-01220
Pursuant to Part F.3.of the Master Contract with contract routing number 00-FAA 00008 and paragraph 16 of the Task Order
with contract routing number 01-00214 and contract encumbrance number FAA EPS 0100214 hereinafter referred to as the
"Original Task Order"(a copy of which is attached hereto and by this reference incorporated herein and made a part hereof as
"Attachment 1")between the State of Colorado,Department of Public Health and Environment and the BOARD OF
COUNTY COMMISSIONERS OF WELD COUNTY(a political subdivision of the state of Colorado)for the use and
benefit of the Weld County Department of Public Health and Environment,for the term from July 4,2000,through June
30,2001,the parties agree that the maximum amount payable by the State for the eligible services referenced in part B of the
Original Task Order is increased by SEVEN THOUSAND FIVE HUNDRED FIFTY DOLLARS,($7,550.00)for a new
total financial obligation of the State of EIGHTY SEVEN THOUSAND FOUR HUNDRED FIFTY THREE DOLLARS
($87,453.00). The revised budget,which is attached hereto as"Attachment 2-1",are incorporated herein by this reference and
made a part hereof. The first sentence in paragraph C_of the Original Task Order is hereby modified accordingly. All other
terms and conditions of the Original Task Order are hereby reaffirmed. This amendment to the Original Task Order is intended
to be effective as of June 01,2001. However,in no event shall this amendment be deemed valid until it shall have been
approved by the State Controller or such assistant as he may designate.
Please sign,date,and return all 4 originals of this Task Order Change Order Letter by May 28,2001 to the attention of:Lee
Joseph,FCHSD A-4,Colorado Department of Public Health and Environment,4300 Cherry Creek Drive South,
Denver,Colorado 80246. One original of this Task Order Change Order Letter will be returned to you when fully approved.

SUBJECT: Change Order Letter#1 to Task Order for
EPSDT Program
Enclosed for Board review and approval is Change Order Letter Number One to the Task Order
between the Colorado Department of Public Health and Environment and Weld County
Department of Public Health and Environment for the EPSDT Program.
The EPSDT Program provides case management, outreach and support services for children age
0 to 21 who are on Medicaid. This change order will provide additional funding in the amount of
$7,550 which will bring the total amount of funding for the time period July 4, 2000 through
June 30,2001 to $87,453. The additional funding will be used to purchase a shredder, to
upgrade the computer that is shared between the EPSDT and Department of Social Service
Medicaid Outstation workers, and to upgrade the EPSDT team leader to an Office Technician III.
I recommend your approval of this change order letter.
