RE: APPROVE AMENDMENT TO PURCHASE OF SERVICES AGREEMENT FOR IN-
HOME RESPITE SERVICES AND AUTHORIZE CHAIR TO SIGN - RVNA HOME
HEALTH CARE, INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to the Purchase of
Services Agreement for In-Home Respite Services between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Department of Human Services, and RVNA Home Health Care, Inc., commencing February 1,
2001, and ending June 30, 2001, with further terms and conditions being as stated in said
amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Amendment to the Purchase of Services Agreement for In-
Home Respite Services between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Human
Services, and RVNA Home Health Care, Inc., be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 12th day of February, A.D., 2001, nunc pro tunc February 1, 2001.

SUBJECT: Amendment of the RVNA Purchase of Services
Agreement
For Board approval is an amendment of the RVNA Purchase of Service agreement.
The amendment will decrease the grant amount from $10,447 to $5,447 to purchase
562 units of in-home respite services. This change in grant amount is due to the under
use of in-home respite services in the Greeley area.

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS, AMENDMENT made and entered into this 1st day of February, 2001 by and between the Weld
County Division of Human Services' Area Agency on Aging, hereinafter referred to as "AAA" and
RVNA Home Care Services, Inc., hereinafter referred to as "RVNA."
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase home health care services from
"RVNA" to provide in-home respite services to elders and their families.
WHEREAS, the "AAA" and "RVNA" are desirous of amending the original agreement of February 1,
2001,
NOW THEREFORE, it is mutually agreed:
1. The "AAA" will reduce the original grant to "RVNA"from $10, 447 to $5,447.
2. "RVNA" agrees to prove 562 units of in-home respite services for 8 60 plus elders and their
families.
2. The "AAA"will reimburse"RVNA" $10.85 per unit of service for in-home respite services.
3. All other terms and conditions of the original agreement shall remain in effect unless otherwise
stated in this Amendment.
4. The terms of this Amendment extend from February 1, 2001 through June 30, 2001. This
Amendment will be terminated immediately in the event of loss of funding.
IN WITNESS WHEREOF,the parties hereto have executed this Amendment on the day first written
above.
