RE: APPROVE PURCHASE OF SERVICES AGREEMENT FOR LONELINESS
OUTREACH PROGRAM AND AUTHORIZE CHAIR TO SIGN - LOCHBUIE AFTER
GLOWS
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Purchase of Services Agreement for
the Loneliness Outreach Program between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Department of
Human Services, Area Agency on Aging, and the Lochbuie After Glows, with terms and
conditions being as stated in said agreement, and
WHEREAS, after review, the Board deems it advisable to approve said agreement, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Purchase of Services Agreement for Loneliness Outreach
Program between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, Area Agency
on Aging, and the Lochbuie After Glows be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 29th day of January, A.D., 2001.

COLORADO SUBJECT: Loneliness Outreach Agreement with Lochbuie
After Glows
Enclosed for Board approval is an agreement for $1,000 between the Weld County
Area Agency on Aging and the Lochbuie After Glows Senior Citizens. The purpose of
this agreement is to promote extensive outreach into the Lochbuie Community to
identify lonely and isolated seniors to connect them with the activities of the Lochbuie
senior program. Volunteers will be recruited to help with this outreach project.
Questions should be directed to Linda E. Piper, Director Weld County Area Agency on
Aging at extension 3320.

Purchase of Services Agreement
THIS AGREEMENT,made and entered into this 15th day of January, 2001 by and between the County of Weld, State of
' - Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Division
of Human Services'Area Agency on Aging, hereinafter referred to as "AAA" and the Lochbuie After Glows hereinafter
referred to as"Lochbuie."
WITNESSETH:
WHEREAS, "AAA"develops and administers the Area Plan for a comprehensive and coordinated system of services,
and
WHEREAS, "AAA"considers senior centers a vital link in the county's system of services to older adults, and
WHEREAS, "Lochbuie" is a senior program providing recreational,nutritional, and educational opportunities for senior
citizens; and
WHEREAS, "Lochbuie"proposes to implement a senior outreach program to involve local seniors in the social activities
at the Lochbuie Senior Center;
NOW THEREFORE, it is mutually agreed:
1. The"AAA" hereby agrees to award grant monies to the "Lochbuie" in the sum of$1,000.
2. "Lochbuie"agrees to use one on one contacts, a welcome letter, one time free meal coupons along with other
outreach activities to recruit new participants to the Lochbuie Senior Program.
3. "Lochbuie"agrees to document contacts made. "Lochbuie"understands that the grant of$1,000 may be used to
implement the senior outreach program and to advance the overall Lochbuie senior recreational program.
4. "Lochbuie"understands this is a one time only incentive award.
IN WITNESS WHEREOF,the parties hereto have executed this Agreement on the day first written above.
