RE: APPROVE TASK ORDER RENEWAL LETTER #1 FOR TUBERCULOSIS PROGRAM
AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Task Order Renewal Letter#1 for the
Tuberculosis Program between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Weld County Department of
Public Health and Environment, and the Colorado Department of Public Health and
Environment, commencing January 1, 2002, and ending June 30, 2002, with further terms and
conditions being as stated in said Task Order Renewal Letter#1, and
WHEREAS, after review, the Board deems it advisable to approve said Task Order
Renewal Letter#1, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Task Order Renewal Letter#1 for the Tuberculosis Program
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby
is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said Task Order Renewal Letter#1.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 17th day of December, A.D., 2001.

SUBJECT: Change Order Letter Number 1 to the
Tuberculosis Outreach and Control Task
Order
Enclosed for Board review and approval is Change Order Letter Number 1 to the Tuberculosis
Outreach and Control Task Order between the Colorado Department of Public Health and
Environment and Weld County.
Under the provisions of the task order, WCDPHE will provide TB treatment, preventive
treatment, provide for and interpret chest x-rays and other medical evaluation services, conduct
contact investigations and TB skin test screening programs. WCDPHE will also follow-up with
active patients to ensure they receive appropriate information and education and they adhere to
the completion of their TB therapy.
Through this change order letter, CDPHE is providing additional funding in the amount of
$4,357 for outreach activities. This funding will bring the total reimbursement for the time
period January 1, 2001 through June 30, 2002 to $99,795.
I recommend your approval of this change order letter.

Pursuant to Part F. of the Master Contract with contract routing number 00 FAA 00008 and paragraph E_1.of the
Task Order with contract routing number 01 00158 and contract encumbrance number PO FHA EPI0100158 (as
amended by Task Order Renewal Letter 01,contract routing number 02 00036,)hereinafter referred to as the
"Original Task Order" (a copy of which is attached hereto and by this reference incorporated herein and made a part
hereof), between the State of Colorado,Department of Public Health and Environment and the Board of County
Commissioners of Weld County,for the use and benefit of the Weld County Department of Public Health and
Environment the parties agree that the maximum amount payable by the State for the eligible services referenced in
paragraph B.of the Original Task Order for the teen from January 1,2002 through June 30,2002, is increased by
FOUR THOUSAND,THREE HUNDRED FIFTY SEVEN DOLLARS,($4,357.00)for a new total financial
obligation of the State of NINETY NINE THOUSAND, SEVEN HUNDRED NINETY-FIVE DOLLARS,
($99,795.00)for that term in accordance with the supplemental budget as outlined below.The first sentence in
paragraph C.1. of the Original Task Order is hereby modified accordingly. All other terms and conditions of the
Original Task Order are hereby reaffirmed. In accordance with section 24-30-202(1),C.R.S.,as amended,the
effective date of this amendment is the date the State Controller approves this amendment.
