Dear Sir or Madam:
Pursuant to the provisions of C.R.S. 1973 §24-10-109, as amended, and pursuant to any
applicable City Charter provisions,county provisions,city or county ordinances,or any other body
of law, you are hereby given notice of injuries and claim for said injury as follows:
1. The injured persons and claimants are Leonardo Quintana, Steven Quintana,Nancy
Quintana, and Stacy Quintana, 312 Stockton Street, Gilcrest, Colorado, 80623. The claimants are
represented by the law firm of Beem & Mann, P.C., 1700 Lincoln Street, Suite 3901, Denver,
Colorado 80203, telephone number 303-894-8100.
2. The basis of the claim is that on or about July 21, 2000, Leonard Quintana, natural
father of the above-named claimants, was shot and killed by members of the Weld County Drug
Task Force, the Greeley Police Department, La Salle Police Department, Milliken Police
Department, Platteville Police Department, and Gilcrest Police Department.
3. As a result of the incident, the claimants have suffered the wrongful death of their
father,the loss of his economic support,his comfort,companionship and affection,together with the
mental and emotional grief, pain and suffering caused by his shooting and death.

4. The claimants seek damages for the injuries set forth above in the amount of
$250,000.00 each, for a total of$1,000,000.00.
