RE: APPROVE AGREEMENT TO ADMINISTER GRANT FUNDS AND AUTHORIZE CHAIR
TO SIGN - UNITED WAY OF WELD COUNTY
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Agreement to Administer Grant
Funds between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and United Way of Weld County, commencing September 15, 2000, and ending
September 15, 2001, with further terms and conditions being as stated in said agreement, and
WHEREAS, after review, the Board deems it advisable to approve said agreement, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Agreement to Administer Grant Funds between the County of
Weld, State of Colorado, by and through the Board of County Commissioners of Weld County,
on behalf of the Weld County Department of Public Health and Environment, and United Way
of Weld County be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted

SUBJECT: Agreement with United Way of Weld County
Enclosed for Board review and approval is an agreement between United Way of Weld County
and the Board of Weld County Commissioners on behalf of the Weld County Department of
Public Health and Environment (WCDPHE).
United Way has been awarded a grant to provide outreach to Weld County residents to inform
and assist them in application for the Child Health Plan Plus (CHP+) program. Through this
grant United Way is collaborating with Monfort Children's Clinic, Salud Clinic, Sunrise
Community Health Center, North Range Behavioral Health, and Weld County Departments of
Social Services and Public Health and Environment.
In the provisions of this agreement, WCDPHE agrees to administer the program by providing a
full-time staff person to serve as a Child Health Plan Plus Advocate. This person will provide
CHP+ information through community presentations to agencies working with families, increase
CHP+ enrollment through the application and referral process, and develop a system to provide
CHP+ enrollment to support WCDPHE clients accessing our services.
For these services, WCDPHE will receive an amount not to exceed $32,500 for the time period
September 15, 2000 through September 15, 2001.

AGREEMENT TO ADMINISTER GRANT FUNDS
This Agreement is entered into by and between United Way of Weld County ("United Way") and
the Board of Weld County Commissioners, on behalf of the Weld County Department of Public
Health and Environment ("the Health Department").
BACKGROUND INFORMATION
A. United Way has been awarded a grant to provide outreach to Weld County
residents to inform and assist them in application for the Child Health Plan Plus
program ("the Grant").
B. Health Department has agreed to collaborate with United Way in providing the
services pursuant to the Grant.
C. The parties wish to set forth their agreement in writing.
NOW THEREFORE, in consideration of the mutual promises contained herein, the
parties agree as follows:
AGREEMENT
1. Recitals. The above set forth recitals are hereby incorporated into this
Agreement.
2. Acceptance of Funds. United Way agrees to provide funds from the Grant in the
amount of$ 32,500.00 to Health Department, and Health Department agrees to
accept said funds to provide outreach to residents of Weld County and to assist
them in application for the Child Health Plan Plus ("CHP+").
3. Term. The term of this Agreement is from September 15, 2000 through
September 15, 2001.
4. Administration of Grant. The Health Department agrees to administer the Grant
in compliance with the proposal submitted by United Way (which is attached
hereto and incorporated herein as "Exhibit A"). Health Department shall provide
a full-time staff person to serve as Child Health Plan Plus Advocate. The duties
for the position shall include the duties delineated in the job description attached
hereto and incorporated herein as "Exhibit B." The Child Health Plan Plus
Advocate shall be employed by Health Department, and, shall therefore be a Weld
County employee, subject to the responsibilities and benefits of the Weld County
Employee Personnel Policy Handbook. Health Department shall abide by the
"General Conditions for Child Health Plan Plus Marketing and Outreach

Assistance Awards," as more particularly set forth in "Exhibit C," attached hereto
and incorporated herein.
5. Support From United Way. United Way agrees to do the following:
a. Support the Health Department, and the Collaborative Partners, Sunrise
Health Center, Monfort Children's Clinic, and Salud Health Center.
b. Provide a .25 FTE Administrative Assistant to support the Collaborations
work and manage the program database.
c. Abide by the "General Conditions for Child Health Plan Plus Marketing
and Outreach Assistance Awards." as set forth in Exhibit C.
d. Work with the CHP+ Advocate to generate all necessary reports and
documentation.
6. Use of Funds. The funds provided to Health Department by United Way
pursuant to this Agreement (sometimes referred to as "Grant funds"), are
restricted to the purpose specified in Exhibit A and are to be spent in accordance
with the proposed budget as set forth in Exhibit A. Any part of the Grant funds
not so used within the term of this Agreement must be returned to United Way
within 30 days after the completion of the project or upon the expiration of the
term of this Agreement.
7. Grant Accounting and Records.
a. Health Department agrees to maintain adequate accounting records and to
obtain documentation for all Grant expenditures.
b. Health Department agrees to maintain records and accounts consistent
with generally accepted accounting principles, and to provide for such
fiscal control as is necessary to assure proper disbursing of, and
accounting for, project Grant funds.
c. Health Department will make a good faith effort to provide information
concerning accounts and documentation relating to the Grant project
expenditures which will be adequate to permit an accurate and expeditious
audit, should an audit be requested by the Grant funding source. Any
audit requested may be conducted during regular business hours and upon
24 hours notice.
8. Changes in activities or expenses. Any significant change in project activities or
expenses must receive prior written approval from United Way. These changes
include, but are not limited to, changes in the project's scope, purpose or
activities, changes in expenses of more than 20 percent of an approved budget

category, and changes in the duration of the grant period.
9. Payment of Grant Funds. United Way shall pay the Grant funds as follows:
The payments shall be made quarterly with the first quarterly payment due and
payable to the Health Department upon receipt by United Way of a signed
duplicate original of this Agreement. The second payment is due and payable on
January 30, 2001; the third payment is due and payable on April 30, 2001; and the
final payment is due and payable on July 30, 2001.
10. Termination. In the event that the Health Department fails to meet the terms of
this contract,United Way may terminate this Agreement upon thirty (30) business
days written notice to the Health Department. Health Department may terminate
this Agreement for any reason upon thirty (30) business days written notice to
United Way.
In the event that this Agreement is terminated prior to the end of the Agreement
term, Health Department shall, within 30 days of the effective date of the
termination, repay to United Way all unexpended Grant funds. Health
Department shall provide to United Way a full and complete accounting of all
receipts and disbursements of funds and expenditures incurred under this
Agreement through the effective date of termination of this Agreement.
11. No Waiver of Governmental Immunities. Notwithstanding the provisions
stated herein, nothing in this Agreement is intended as a waiver by Health
Department of governmental immunity rights pursuant to state or federal law.
12. Budgeting, Nothing in this Agreement shall be construed to require Weld
County to expend funds not previously budgeted.
13. Independent Parties. The parties to this Agreement intend that the relationship
between them contemplated by this Agreement is that of independent entities
working in mutual cooperation. No employee, agent, or servant of one party shall
be or shall be deemed to be an employee, agent, or servant of another party to this
Agreement.
14. Agreement not Assignable. This Agreement shall not be assignable to any third
party.
15. Notices. Any notice provided for in this Agreement shall be in writing and shall
be served by personal delivery or by certified mail, return receipt requested,
postage prepaid, at the addresses set forth in this Agreement, until such time as
written notice of a change is received from the party wishing to make a change of

address. Any notice so mailed and any notice served by personal delivery shall be
deemed delivered and effective upon receipt or upon attempted delivery.

16. Entire Agreement-- Modification. This Agreement contains the entire
Agreement and understanding between the parties to this Agreement and
supersedes any other agreements concerning the subject matter of this transaction,
whether oral or written. No modification, amendment, novation, renewal, or other
alteration of or to this Agreement shall be deemed valid or of any force or effect
whatsoever, unless mutually agreed upon in writing by the undersigned parties.
No breach of any term, provision, or clause of this Agreement shall be deemed
waived or excused, unless such waiver or consent shall be in writing and signed
by the party claimed to have waived or consented. Any consent by any party
hereto, or waiver of, a breach by any other party, whether express or implied, shall
not constitute a consent to, waiver of, or excuse for any other different or
subsequent breach.
17. Provisions Severable. If any term or condition of this Agreement shall be held to
be invalid, illegal, or unenforceable, this Agreement shall be construed and
enforced without such a provision, to the extent this Agreement is then capable of
execution within the original intent of the parties.
18. No Third Party Enforcement. It is expressly understood and agreed that
enforcement of the terms and conditions of this Agreement, and all rights of
action relating to such enforcement, shall be strictly reserved to the undersigned
parties, and nothing contained in this Agreement shall give or allow any claim or
right of action whatsoever by any other person not included in this Agreement. It
is the express intention of the undersigned parties that any entity, other than the
undersigned parties, receiving services or benefits under this Agreement, shall be
deemed an incidental beneficiary only.

WELD COUNTY CHP+ COLLABORATION
GRANT APPLICATION
Desc— r�on
United Way of Weld County is submitting this Child Health Plan Plus grant and if
funded,will facilitate this project on behalf of all S.E.D. sites/Child Health Plan Plus providers
in Weld County. We propose providing outreach to all of Weld County through a coordinated
approach using two strategies—we will use the existing network of service providers as
outreach workers to educate and enroll families. We will also concentrate on enrollment through
the Weld County Health Department, which through immunization clinics, well-baby checks,
family planning services to women with children, and E.P.S.D.T. comes in contact with 2,000
potentially eligible clients, but does not currently have adequate staffing to handle enhanced
enrollment efforts. We will follow up on Medicaid denied applicants to inform and assist them
in getting their applications processed for the CHP+program. We are creating an Outreach/
Tracking form (attached)that will not only allow us to do more extensive follow-up with
eligible families but will be our evaluation tool.
Need
Weld County children could greatly benefit from this program. While other Front Range
communities are improving in the Colorado Children's Campaign Kid's Count indicators, Weld
County is actually loosing ground. According to the 1999 Kids Count Report Card, Weld
County's indicators have worsened since 1991, including rates for low birth weight, infant
mortality and teen births. The 1990 census also shows Weld County's poverty level for children
at 18.9%compared to a state rate of 15%. Weld County compared less favorably to neighboring
Front Range counties child poverty rates (Lorimer— 10.4% and Adams 14.1%). According to
1990 census, the median family income in Weld County was $30,800 compared to a state family
median income of$35,930. Poverty remains high and wages have remained low. While CHP+
lists Weld County with 1,782 unenrolled eligible children, the number is much higher. Weld
County's growth since 1990 has resulted in its' population doubling over the last decade.
Currently Northern Colorado is growing at one of the fastest rates in the state. We estimate the
number of uninsured CHP i eligible children to between 6,000 to 7,000 compared to the 1990
census figures. We know, according to a study done by the Colorado Coalition for Medically
Uninsured, that Weld County ranks the highest in the state for the number of uninsured children
(20%).
Because our project includes all of the current S.E.D. sites several barriers have already
been identified. They include inability to follow-up with eligible clients who did not have all
their documentation,resubmission to CHP after denial of Medicaid and general concerns that the
system is too cumbersome. We are writing into our unique outreach/tracking form a survey
process that will identify additional barriers by asking questions at initial contact and after
enrollment.This information will be used to refine our process and to support further projects.
Several of our partners are also applying for mini-grants to enhance their own outreach efforts_ If
funded, they will use our outreach/tracking system and those additional efforts will be integrated
into this collaborative.
By using Weld County's existing network of agencies to educate clients about CHP+and
to support the application process, minimal new or additional staffing will be required. We
propose one full-time position to serve as a CHP+ Advocate/Ombudsman for all sites. The
ombudsman will be located at the Health Department to provide coordinated support and follow-
up to their existing application process. An additional .25 FTE clerical position will be located at
UWWC WC to manage the.outreach tracking database. United Way of Weld County will provide
grant oversight and facilitation of the collaboration as an in-kind contribution to the project.
Impact
•
Weld County is fortunate to have an extensive network of service providers; many doing
case-management, outreach and/or home visits to many families with children. Many of those
agencies are only minimally aware of CHP+, few provide clients information about the program
and only the enrollment technicians at the clinics fully understand the process. Many eligible
clients have been frustrated by the complicated application and lengthy process and have given
up. Our approach,we hope,will provide more accurate information to eligible clients up front
and support throughout the process. Our program will provide a wider network of agencies
educating our community about CHP+ and a key group of agencies actually assisting individuals
with filling out the applications and obtaining support documentation. This hopefully will result
in more individuals at S.E.D. sites being certified on their first visit.
The ombudsman using our outreach/tracking form will be able to support each individual
who has completed an application. We will use our outreach/tracking form to create a database
that will allow us to follow-up with individuals, track our results and to also provide CHP+
valuable information on the CHP+programs internal barriers and perhaps provide insight into
ways of improving access.
As an incentive to support the additional staff time required of the ten entities actually
helping their clients fill-out applications, we will pay a$5.00 stipend for each application
completed.
Priority
This project is a joint project of the North Colorado Medical Center Foundation's
Monfort Children's Clinic, Salud Clinic, Sunrise Community Health Center, Weld County
Department of Health and Environment,Weld County Department of Social Services, North
Range Behavioral Health and United Way of Weld County.This network serves all of Weld
County.This collaborative effort will increase enrollment and will enhance cooperation and
collaboration among the participating agencies.

Qualification of Organzation
United Way of Weld County for the last 12 years has facilitated the Weld County Child
Abuse Coalition. Those efforts resulted in the creation of an advocacy center and a C.A.S.A.
program. Both originally operated by United Way have now been spun off as A Kids Place. We
are in our fourth year as facilitator and fiscal agent for the Weld County Early Family
Intervention Collaboration funded through Y.C.P,I. We currently have partnerships with School
District 6 and Aims Community College for program delivery. United Way of Weld County also
funds and monitors 47 programs, which provide 76 services in Weld County.
Collaboration
We will be building on the successful collaborative efforts of both the Child Abuse
Coalition and the Weld County Early Intervention Collaboration by using them as outreach
networks. This collaboration is also working on a joint application to implement an intensive
nurse home-visiting program. The partners in this project will meet regularly to review progress
and refine our program to meet goals & objectives.
,Evaluation
The agencies check list forms and our database will provide the information required for both
tracking and reporting purposes.
• By training the agencies to help support families in filling out their application and gathering
the necessary documents, these process will be faster and easier for them at the s.e.d. sites
and decrease the need for additional appointments to complete this process.
• With the ombudsman's will follow up with eligible clients and the s.e.d. sites to monitor and
track the success rate of applicants to the originating referral sources.
• The ombudsman will communicate with the applicants,the s.e.d. sites, the Department of
Social Services, and the CHP+office,resolving many enrollment problems.
• Community relationships will be strengthened through the efforts and communications of the
ombudsman with the county's agencies and through our collaborative grantee meetings.
• The clerical staff position will dispense CHP+program informational materials to the
information tier groups and will track enrollments.
• 3,000 families will receive CHP+information through our efforts; this would reflect a
duplicated count as families could receive information from more than one source.
• The correlation of the number of information fliers distributed, the number of applications
supported by trained agencies, the number of applicants who get to the s.e.d. sites and the
actual number of approved applications will be tracked and compared for both the evaluation
and reporting processes.
• The primary care providers will conduct a survey of enrollees to reveal problems they may
have experienced as enrollees.

BUDGET.JUSTIFICATION
The ombudsman's salary is based on Weld County salary classification for a comparable
position.
The supply and operation cost covers both United Way and the Weld County Public Health
Department's costs. Printing costs would include the cost of printing the CHP+check list forms
on NCR paper in triplicate and the Weld County Application Process and S.E.D. site information
sheets. Postage would include mailings to agencies, S.E.D. sites and applicants. Travel costs
would cover staff mileage to the Salud Clinic in Ft. Lupton.
The incentive stipend would reimburse the ten agencies who begin assisting clients in filling out
their applications. We would reimburse the agency$5.00 for each application generated at their
site and submitted to a S.E.D. site.
Office space and equipment will be in-kind donations of United Way of Weld County and the
Weld County Public Health Department.

DESCRIPTION OF WORK
General Statement of Duties: To impact and increase Child Health Plan Plus (CHP+) enrollments
by providing community training,public application assistance, and program advocacy to CHP+
providers and Weld County families.
Supervision Received: Works under the Weld County Department of Public Health and
Environment(WCDPHE) administrative policies,United Way of Weld County(UWWC), CHP+
Collaboration Grant guidelines,and CHP+general enrollment procedures.
Supervision Exercised: Serves as a leader in the community regarding CHP+services.
EXAMPLES OF DUTIES: (Any one position may not include all of the duties listed nor do the
listed examples include all duties which may be found in grant position.)
Provide CHP+information through community presentations to agencies working with
families as outlined in the Weld County CHP+Collaboration grant.
Increase CHP+ enrollment through application and referral process.
Develop a system to provide CHP+enrollment to support WCDPHE clients accessing
services. Provide CHP+enrollment through internet.

UWWC for data collection.

MINIMUM QUALIFICATIONS
Required Knowledge,Skills, and Abilities: Ability to exercise initiative and considerable
amount of independent judgement;has excellent verbal and written communication skills;
Bi-lingual in Spanish and English; computer knowledge of software systems to enhance
development of program enrollment process,tracking system, and group training presentations;
establish and maintain effective working relationships with community agencies and families
identified for CHP+services. Knowledge of community resources essential. Utilization of office
equipment,practices, and procedures.Personal insured vehicle and current Colorado driver's
license.

literary or education purposes. Award funds and income earned thereon may be expended only
for the purpose(s) indicated on the applicant's formal request and as outlined in the award letter.
No substantial variance may be made without prior written approval from Child Health Plan
Plus.
Any award funds, or income earned thereon, not expended or committed for the purposes of the
award must be returned to Child Health Plan Plus.
Awardee shall comply with Child Health Plan Plus requests for information and reports.
If an award is to provide funds for a special project and if the special project being funded
includes any attempts to influence legislation, none of the award funds may be used for any
attempt to influence legislation.
With an award from Child Health Plan Plus in the amount of$46,500, the United Way of Weld
County will meet the project goals stated in your proposal. The duration of the grant shall be
September 15, 2000 through September 15, 2001.
Additional tracking, reporting, evaluation and payment requirements will be established by the
United Way of Weld County Project Supervisor and the Child Health Advocates Community
Enrollment Projects Manager at a later date.
