RE: APPROVE AMENDMENT TO PURCHASE OF SERVICES AGREEMENT AND
AUTHORIZE CHAIR TO SIGN - RVNA HOME HEALTH CARE, INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to the Purchase of
Services Agreement between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Human Services,
and RVNA Home Health Care, Inc., commencing February 1, 2001, and ending June 30, 2001,
with further terms and conditions being as stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Amendment to the Purchase of Services Agreement between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Department of Human Services, and RVNA Home Health Care,
Inc., be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 12th day of February, A.D., 2001, nunc pro tunc February 1, 2001.

SUBJECT: Amendment of the RVNA Purchase of Service
Agreement
For Board approval is an amendment of the RVNA Purchase of Services agreement. The
amendment will increase the grant amount from $25,000 to $30,000 to purchase 1,575 units of
in-home health care services. This change in grant amount is due to the increasing need for in-
home services in south county.

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS, AMENDMENT made and entered into this 1st day of February, 2001 by and between the
Weld County Division of Human Services' Area Agency on Aging, hereinafter referred to as
"AAA" and RVNA Home Care Services, Inc., hereinafter referred to as "RVNA."
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase home health care services from
"ANNA" to provide in-home care services to south Weld County concentrating on the
communities of Fort Lupton, Hudson, Keenesburg, Dacono, Frederick and Firestone, and
WHEREAS, the "AAA" and "RVNA" are desirous of amending the original agreement of January
2, 2000,
NOW THEREFORE, it is mutually agreed:
1. The "AAA" will reimburse "RVNA" a total of $30,000 for 1,575 units of home health
care for 25 persons.
2. The "AAA"will reimburse "RVNA" $10.85 per unit of service for general in-home
services.
3. The"AAA" will reimburse"RVNA" $12.94 per unit of services for personal care
preceptor services.
4. The "AAA" will reimburse "RVNA" up to $7,900 for program startup and ongoing nursing
case management services
2. All other terms and conditions of the original agreement shall remain in effect unless
otherwise stated in this Amendment.
3. The terms of this Amendment extend from February 1, 2001 through June 30, 2001. This
Amendment will be terminated immediately in the event of loss of funding.
IN WITNESS WHEREOF, the parties hereto have executed this Amendment on the day first
written above.
