RE: APPROVE 2000 TRANSPORTATION GRANT AND AUTHORIZE CHAIR TO SIGN -
COLORADO DEPARTMENT OF TRANSPORTATION
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a 2000 Transportation Grant between
the County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Department of Human Services, and the Colorado Department of
Transportation, with terms and conditions being as stated in said grant, and
WHEREAS, after review, the Board deems it advisable to approve said grant, a copy of
which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the 2000 Transportation Grant between the County of Weld, State
of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of
the Department of Human Services, and the Colorado Department of Transportation be, and
hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said grant.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 30th day of April, A.D., 2001.

Enclosed for Board approval is the 2000 CDOT Grant (5310-5311) with the Colorado
Department of Transportation.
Included are Certification of Use Form A and Form B, Annual Performance Report Form
C, the Charter Service Certification letter, a copy of the Human Service Agency Audit, and
a copy of the Certificate of Participation Insurance Form. These complete the 2000 Grant.

Dear Ms. Hershberger:
In compliance with the Federal Transit Administration (FTA) charter regulations, Weld County
Board of County Commissioners, Human Services Transportation Department does not intend to
provide charter service with Section 5311-funded vehicles in 2000.
If Weld County Board of County Commissioners decides that it wishes to provide any charter
service with Section 5311-funded vehicles in 2000, Weld County Board of County
Commissioners will notify CDOT as soon as possible and complete a notification process for
such. No charter service will be provided until such process is successfully completed.

CERTIFICATION OF USE OF SECTION 5310 PROJECT EOUIPMENT
As Chair, Weld County Board of County Commissioners, I hereby certify that the equipment
purchased with funds authorized under Section 5310 of the Federal Transit Act of 1997,
as amended:
1. Is being used for the purpose for which the grant was approved, that is for the
Transportation of elderly persons and persons with disabilities;
2. Is being used in accordance with the scope of work set forth in out contract with the
State of Colorado;
3. Does not exceed that which is needed for transit operations (i.e. , the equipment is not
in excess of that needed to fulfill the purposes other than those specified in the grant),
is in regular, continuing, and full time use, and that no vehicle is used as a spare or
backup vehicle.
4. Has been insured in the minimal amounts required in the grantee's contract with the
State of Colorado, with the State being listed as loss payee for that equipment that still
has a Federal interest;
5. Has not been sold, liened, used as collateral, damaged, or otherwise taken out of transit
service;
6. Is titled in the name of the above organization, with the original title provided to the
State, unless otherwise indicated by written agreement with the Colorado Department
of Transportation; and
7. Includes the vehicles or other equipment as described in the inventory page provided
by the Colorado Department of Transportation, and that any errors in the provided
information about said equipment is noted on said inventory page and submitted along
with this form to the State.

over the past year(i.e., service expansion, eligibility changes, additional vehicles. etc.)
We have begun to increase services to the very rural part of Weld County. Our trip numbers.
both miles and passengers. are less than 1999, as we are spending more time in the rural
settings.
The total driving hours are near the same as previous years. due to the increasing number of
one-on-one transportation, such as additional clients to doctors appointments and dialysis
centers. We have been successful in coordination of the group type transportation. therefore
increasing the number of clients on the route but in doing so. we are filling those times in with
the one-on-one trips. We are seeing exceeding number of dead-head miles. We continue in our
efforts in the coordinating of many rural routes with existing programs and we can see some
results in saved miles at the beginning or ending of the newer routes. We are often unable to
utilize the vehicle and driver with other routes in both the beginning and/or end of established
routes.

Colorado Counties Casualty and Property Pool
For the Coverage Period January 1, 2001 through December 31, 2001
WELD COUNTY
Colorado Counties Casualty and Property Pool(CAPP)hereby certifies that Weld County is a participating
Member of CAPP for the period beginning January 1, 2001 through December 31,2001. The coverages,
conditions of membership, and other provisions applicable to members of CAPP are as described in CAPP's
Bylaws and Intergovernmental Agreement and in the applicable excess policies, Policy Statements and
endorsements thereto, copies of which have been or will be provided to Weld County.
The types and monetary limits of the coverages provided to Weld County through membership in CAPP, in
consideration of the payment of its contributions, are limited, as of the date of this certificate, to those which
are shown below. The scope, terms,conditions, and limitations of coverages are governed by the
aforementioned agreement and policies.
I. The types of coverages,subject to the limit on CAPP's liability in Section II below,are as follows:

II. CAPP Retention,Aggregate Limits,and Member Deductibles
For the coverages described in Section I,CAPP shall be liable only for payment of the self-insured
retention and only to a total annual aggregate amount for members of CAPP as a whole of the amount of
the CAPP loss fund for the coverage period. CAPP's per claim/occurrence retention are limited to the
following for the foregoing coverages:
A. $150,000 per claim/occurrence property
B. $250,000 per claim/occurrence liability(per coverage line)
C. $150,000 per claim/occurrence crime
D. $5,000 per loss boiler and machinery
In'addition, there is a maintenance deductible of $500.00, which applies to each of the County's first
party claims/occurrences/losses. Payment of the deductible reduces the amount otherwise payable
under the applicable CAPP retention.
Pertaining to all liability losses, CAPP has purchased an aggregate retention protection of$4 million.
Coverages in excess of the foregoing retention are provided only by the excess insurers in applicable
excess policies,and are payable only by those excess insurers. The limits of coverage provided by the
excess insurers are as follows:
A. Liability
From $250,000 per claim/occurrence to$2 million per claim/occurrence except for auto which
is non-aggregated,subject to the following:
All liability coverages are provided on a claims-made coverage form except for automobile
liability,which is on an occurrence form. In no event shall the maximum per
claim/occurrence payment exceed the following for general liability,auto liability,public
entity management liability, law enforcement liability,employee benefits liability or medical
professional liability claims subject to the Governmental Immunity Act: $150,000 per person
and$600,000 per occurrence. Additional limits, sublimits and aggregates apply as provided in
the applicable excess policies.
B. Crime
None
C. Boiler and Machinery
From $5,000 to the cost of repair/replacement for each actual loss sustained
D. Property
From $150,000 to $50,000,000 each occurrence. "All Risk"basis with sublimits of
$1,000,000 newly acquired property and$5,000,000 new construction each occurrence.
$500,000 unscheduled locations.Newly acquired property must be reported within 60 days.
