RE: APPROVE PURCHASE OF SERVICES AGREEMENT FOR LONELINESS
OUTREACH PROGRAM AND AUTHORIZE CHAIR TO SIGN - PIERCE SENIOR
CENTER
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Purchase of Services Agreement for
the Loneliness Outreach Program between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Department of
Human Services, Area Agency on Aging, and the Pierce Senior Center, commencing upon full
execution of said agreement, March 12, 2001, and ending June 30, 2001, with further terms
and conditions being as stated in said agreement, and
WHEREAS, after review, the Board deems it advisable to approve said agreement, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Purchase of Services Agreement for the Loneliness Outreach
Program between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Human Services, Area Agency
on Aging, and the Pierce Senior Center be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said agreement.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 30th day of April, A.D., 2001, nunc pro tunc, March 12, 2001.

COLORADO SUBJECT: Purchase of Service Agreement - Loneliness
Outreach
Enclosed for Board approval a Purchase of Service agreements with Pierce Senior Center to
purchase outreach activities. This grant for$1,000 is in response to needs data which shows that
a high percentage of Weld seniors are lonely and isolated from community life. The Pierce
Senior Center has agreed to redouble their efforts to reach out to these seniors by implementing
one on one visitations, free meal coupons to attract new seniors to the centers and a variety of
other outreach activities. The agreement is in effect from 3/12/01 through 6/30/01.

Purchase of Services Agreement
THIS AGREEMENT,made and entered into this 12th day of March,2001 by and between the County of Weld, State of Colorado,by
and through the Board of County Commissioners of Weld County,on behalf of the Weld County Division of Human Services'Area
Agency on Aging,hereinafter referred to as "AAA" and the Pierce Senior Center hereinafter referred to as"Pierce."
WITNESSETH:
WHEREAS,"AAA"develops and administers the Area Plan for a comprehensive and coordinated system of services,and
WHEREAS,"AAA"considers senior centers a vital link to seniors who are lonely and isolated,and
WHEREAS,"Pierce" is a program providing recreational,nutritional, and educational opportunities for senior citizens, and
WHEREAS,"Pierce"proposes to implement an"Abolish Senior Loneliness"to reach out to seniors in their community to involve
them in the social activities at the Pierce Senior Center;
NOW THEREFORE, it is mutually agreed:
1. The"AAA"hereby agrees to award grant monies to"Pierce"in the sum of$1,000.
2. "Pierce"agrees to mail invitations to all(senior)box holders in the Pierce area. The invitation will invite seniors to come to the
center and will include a"free meal"coupon for the first senior dinner."Pierce"expects to recruit 10 new members to their
program.
3. "Pierce"agrees to implement a volunteer mentor program.
4. "Pierce"agrees to increase the community's awareness of the center's activities through by utilizing the Senior Market Place
News and the North Weld Herald.
3. "Pierce"agrees to document activities and results of this outreach effort. "Pierce"understands that the grant of$1,000 may be
used to pay for the costs of the outreach program and to advance the overall Pierce Senior Center Program.
4. "Pierce"understands this is a one time only incentive award and the terms of this agreement are valid through June 30,2001.
IN WITNESS WHEREOF,the parties hereto have executed this Agreement on the day first written above.
