RE: APPROVE TASK ORDER RENEWAL LETTER#1 FOR STD PROGRAM AND
AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Task Order Renewal Letter#1 for the
STD Program between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Weld County Department of Public
Health and Environment, and the Colorado Department of Public Health and Environment,
commencing January 1, 2002, and ending December 31, 2002, with further terms and
conditions being as stated in said Task Order Renewal Letter#1, and
WHEREAS, after review, the Board deems it advisable to approve said Task Order
Renewal Letter#1, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Task Order Renewal Letter#1 for the STD Program between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of
Weld County, on behalf of the Weld County Department of Public Health and Environment, and
the Colorado Department of Public Health and Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said Task Order Renewal Letter#1.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 5th day of December, A.D., 2001.

SUBJECT: Task Order Renewal Letter#1 for the
Chlamydia Testing Program
Enclosed for the Board review and approval is Task Order Renewal Letter#1 for the chlamydia
testing project. If approved, this renewal letter will become part of the master contract between
the Colorado Department of Public Health and Environment (CDPHE) and Weld County.
This renewal letter provides for Weld County to be reimbursed for chlamydia testing in the
sexually transmitted disease and family planning clinics. The reimbursement rate is $7.80 per
test not to exceed $7,254 for the time period January 1, 2002 through December 31, 2002.
I recommend your approval of this renewal letter.

State Fiscal Year 2001 -2002, Task Order Renewal Letter Number 1, Contract Routing Number 02-00093
Pursuant to Part F_5.of the Master Contract with contract routing number 00 FAA 00008 and paragraph C.4 of the Task Order
with contract routing number 01-00090 and contract encumbrance number PO FHA EPI0100090,(as amended by Task Order
Change Order Letter 1 contract routing number 01-00180), hereinafter referred to as the"Original Task Order"(a copy of
which is attached hereto and by this reference incorporated herein and made a part hereof)between the State of Colorado,
Department of Public Health and Environment and the BOARD OF COUNTY COMMISSIONERS OF WELD
COUNTY, for the use and benefit of the Weld County Department of Public Health and Environment,the parties agree
that the maximum amount payable by the State for the eligible services referenced in paragraph B of the Original Task Order
for the renewal term from January 1,2002 through December 31,2002, is increased by SEVEN THOUSAND TWO
HUNDRED FIFTY FOUR DOLLARS, ($7,254.00)for a new total financial obligation of the State of FOURTEEN
THOUSAND FIVE HUNDRED EIGHT DOLLARS ($14,508.00).The budget and work plan, which is attached hereto as
"Attachment 1",is incorporated herein by this reference and made a part hereof. The first sentence in paragraph C of the
Original Task Order is hereby modified accordingly. All other terms and conditions of the Original Task Order are hereby
reaffirmed. In accordance with section 24-30-202(1),C.R.S.,as amended,the effective date of this amendment is the date the
State Controller approves this amendment.
Please sign, date,and return all 3 originals of this Task Order Renewal Letter by December 1,2001 to the attention of:Lisa

CY 2002 Chlamydia
Weld County Health Department
The Contractor will participate in the Chlamydia Project and agrees to the following:
The Contractor will submit Chlamydia Project epidemiologic data (using the mutually
agreed upon data collection form) for men and women tested for chlamydia in the STD
Clinic and for women tested for chlamydia in the Family Planning Clinic.
The Contractor will assure the quality of chlamydia control activities (screening,
treatment, and partner management) in the STD Clinic through chart review activities to
be agreed upon in conjunction with the State.
The Contractor will assure that Azithromycin obtained with State funds will be used in
the STD clinic and Family Planning clinic according to State guidelines.
Budget
Chlamydia Testing:
A. STD Clinic: 120 @ $7.80 each $ 936.00
B. Family Planning Clinic: 810 @ $7.80 $6,318.00
TOTAL $7,254.00
