HomeMy WebLinkAbout20013317.tiff RESOLUTION RE: APPROVE TASK ORDER RENEWAL LETTER#1 FOR STD PROGRAM AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with Task Order Renewal Letter#1 for the STD Program between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Department of Public Health and Environment, and the Colorado Department of Public Health and Environment, commencing January 1, 2002, and ending December 31, 2002, with further terms and conditions being as stated in said Task Order Renewal Letter#1, and WHEREAS, after review, the Board deems it advisable to approve said Task Order Renewal Letter#1, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that Task Order Renewal Letter#1 for the STD Program between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Department of Public Health and Environment, and the Colorado Department of Public Health and Environment be, and hereby is, approved. BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to sign said Task Order Renewal Letter#1. If approved, this renewal letter will become part of the master contract between the Colorado Department of Public Health and Environment (CDPHE) and Weld County. This renewal letter provides for Weld County to be reimbursed for chlamydia testing in the sexually transmitted disease and family planning clinics. The reimbursement rate is $7.80 per test not to exceed $7,254 for the time period January 1, 2002 through December 31, 2002. I recommend your approval of this renewal letter. Wallace, D, MPH•Director ra Printed on Recycled Paper Attachment 1 CY 2002 Chlamydia Weld County Health Department The Contractor will participate in the Chlamydia Project and agrees to the following: The Contractor will submit Chlamydia Project epidemiologic data (using the mutually agreed upon data collection form) for men and women tested for chlamydia in the STD Clinic and for women tested for chlamydia in the Family Planning Clinic. The Contractor will assure the quality of chlamydia control activities (screening, treatment, and partner management) in the STD Clinic through chart review activities to be agreed upon in conjunction with the State. The Contractor will assure that Azithromycin obtained with State funds will be used in the STD clinic and Family Planning clinic according to State guidelines. Budget Chlamydia Testing: A. STD Clinic: 120 @ $7.80 each $ 936.00 B. Family Planning Clinic: 810 @ $7.80 $6,318.00 TOTAL $7,254.00 Hello
