RE: APPROVE AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES
AND AUTHORIZE CHAIR TO SIGN - NORTH RANGE BEHAVIORAL HEALTH
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to the Child Protection
Agreement for Services between the County of Weld,State of Colorado,by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Social Services, and
North Range Behavioral Health, commencing upon full execution,and ending May 31, 2004,with
further terms and conditions being as stated in said amendment, and
WHEREAS,after review,the Board deems it advisable to approve said amendment,a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado,ex-officio Board of Social Services,that the Amendment to the Child Protection
Agreement for Services between the County of Weld,State of Colorado,by and through the Board
of County Commissioners of Weld County, on behalf of the Department of Social Services, and
North Range Behavioral Health be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be,and hereby is,authorized to
sign said amendment.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 31st day of December, A.D., 2003.

AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES
BETWEEN THE WELD COUNTY DEPARTMENT OF SOCIAL SERVICES
AND NORTH RANGE BEHAVIORAL HEALTH
This Amendment,made and entered into the day of December 2003,by and between the Board of
Weld County Commissioners,on behalf of the Weld County Department of Social Services,hereinafter referred to
as"Social Services",and North Range Behavioral Health,hereinafter referred to as"North Range Behavioral
Health."
WITNESSETH
WHEREAS,the parties desire to modify their current Agreement(FY 03-12000)effective October 1,2003,
to modify what service hours are paid for,at what point the request for payment is made,and how many days a
referral lasts;and
WHEREAS,required approval,clearance,and coordination have been accomplished from and with
appropriate agencies.
NOW THEREFORE,in consideration of the premises,the parties hereto covenant and agree as follows:
1. Exhibit D,Special Conditions,Item B,will be revised as follows:
The hourly rate will be paid for all direct and indirect hours to complete the service.
2. Exhibit D,Special Conditions,Item G,will be added as follows:
Request for payment must be made when the entire evaluation is completed. Such evaluation
must be submitted to the Social Services prior to or at the date of billing.
3. Exhibit D,Special Conditions,Item H,will be added as follows:
Referrals are considered active for 90 days from the issuance date.
Except as modified herein,the terms and conditions of the Original Agreement,together with all
attachments of the Original Agreement,remain in full force and effect.
IN WITNESS WHEREOF,the arties ere have duly executed the Agreement as of the day, month, and year first
above written.

RE: Amendment to Child Protection Agreement for Services Between
the Weld County Department of Social Services and North Range
Behavioral Health
Enclosed for Board approval is an Amendment to Child Protection Agreement for
Services between the Weld County Department of Social Services (Department) and
North Range Behavioral Health (North Range). This Amendment was discussed and
reviewed at the Board's Work Session of December 29, 2003.
The major reasons for the Amendment are to:
1. Pay North Range for direct and indirect hours needed to complete a psychological
evaluation.
2. Reimburse North Range only when the entire evaluation is completed and
submitted to the Department for payment.
3. Define a referral as "active" or good for 90 days from the issuance date by the
Department.
All other provisions remain the same including the reimbursement level of$97,544 and
the term of the Agreement, ending May 31, 2004.
