RE: APPROVE REVISION TO NOTIFICATION OF FINANCIAL ASSISTANCE AWARD FOR
SEX ABUSE TREATMENT AND AUTHORIZE CHAIR TO SIGN - ADOLESCENT AND
INDIVIDUAL DEVELOPMENT
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with a Revision to the Notification of Financial
Assistance Award for Sex Abuse Treatment between the County of Weld,State of Colorado,by and
through the Board of County Commissioners of Weld County,on behalf of the Department of Social
Services, and Adolescent and Individual Development, commencing June 1, 2003, and ending
May 31, 2004, with further terms and conditions being as stated in said revision, and
WHEREAS,after review,the Board deems it advisable to approve said revision, a copy of
which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, ex-officio Board of Social Services, that the Revision to the Notification of
Financial Assistance Award for Sex Abuse Treatment between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Department of Social Services, and Adolescent and Individual Development, be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be,and hereby is,authorized to
sign said revision.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 20th day of October, A.D., 2003, nunc pro tunc June 1, 2003.

RE: Revision of Notification of Financial Assistance ward (NOF A)
with Adolescent & Individual Therapy
Enclosed for Board approval is a revision to a Notification of Financial Assistance Award
(NOFAA) with Adolescent & Individual Therapy. The revised NOFAA was discussed
and reviewed at the Board's Work Session held on October 6, 2003.
The major revisions to the NOFAA are to:
A. Lower their individual session rate from $55.06 to $50.00 per hour.
B. Lower their group rate from $55.06 per hour to $35.00 per hour.
C. Provide for more specificity regarding their testing and evaluation rates.
All other terms of the NOFAA remain the same.
