Subject: Request for Proposal - Nursing Services
As you are aware, Weld County issued a request for proposal (RFP) to contract
out nursing services in the jail as discussed in the budget hearings. Based
upon the RFP, one response was received that conformed to the RFP. An evaluation
of the contract services RFP was done by Dick Barrett, Dan Sandy, and myself
that indicates there is only a "savings" of $4,959 over a twelve month period.
However, the "savings" is predicated on the fact that only 6044 hours/year of
service would be provided compared to 7200 hours/year with the current staff of
four nurses. Thus, there is truly no cost savings in going to contract.
The situation is further complicated by the fact that the physician involved and
Sheriff's management have many reservations concerning the use of contract
services in a jail environment as detailed in previous discussions and memos
to you.
Therefore, it is recommended that the proposal be rejected by the Board.
However, the recommendation to reject the proposal is done with the following
additional recommendations to the Sheriff and Board of County Commissioners :
1 . Recognition that the nature of the program will continue to cause
high turnover rates in the employment of jail nurses, resulting in
periods of lengthy vacancies (historically averaging over 90 days) .
2. Cost analysis was based upon assumption of no overtime compensation
to nurses, thus requiring the Sheriff's Office to manage scheduling
with vacancies with comp time and salary limits.
3. Funds in the amount of $17,412 must be transferred from the Contingency
to the Jail Budget.

Nurse Placement Service, located at 1802 16th Street, legally authorized
to operate in Weld County in the State of Colorado by proof of Trade
Name Affidavit recorded November 27, 1979, with the Weld County Clerk
and Recorder, Mary Ann Feurstein (see exhibit A) respectfully submits a
bid for nursing service for the Weld County Jail.
Those persons authorized to represent Nurse Placement Service are the
partners of this service, Margaret Clemons, R.N., and Sara R. Asmus, R.N.
We are willing to sign the "Disclosure Statement" as per your request
for proposal.
In reviewing this bid, if you so desire, we would be available at any
time to discuss and answer any questions your evaluation committee may
have regarding our proposal.

1. STATEMENT OF INTENT
Nurse Placement Service is willing and able to provide nursing
personnel and service as outlined in the specific program components
of the request for proposal (section III, page 2 through 5).
CONDITIONS
A. Nurse Placement Service has in force an insurance policy through
St. Paul Insurance Company which meets the Condition A — Indemnification—
Insurance (see Exhibit B)
B. Nurse Placement Service shall maintain adequate records in accordance
with this condition and under the supervision of our Accredited Medical
Records Director.
C. Nurse Placement Service shall furnish the appropriate bond to comply
with this section and will sign the ownership and control interest <
disclosure statement.
D. Nurse Placement Service agrees to this condition.
E. Nurse Placement Service respectfully requests a cost of living adjustment
to be negotiated by both parties 60 days prior to the termination of the
contract.
F. — G. Nurse Placement Service understands and agrees to these conditions.
H. Nurse Placement Service agrees to this condition but wishes to elaborate upon
the supervision. Nurse Placement Service shall provide a registered nurse
with administrative and management skills to supervise these services. The
R.N. will also be certified to do C.P.R.

PROPOSAL #1
WELD COUNTY NURSING SERVICE
I. Supervisor Service - registered nurse 40 hours per week. 1910 hours
in 11 months - 2/1/82 to 12/31/82
II. L.P.N. Service - 72 hours per week. 3454 hours in 11 months -
2/1/82 to 12/31/82
III . A professional nurse will be available on clinic days, oral boards,
disciplinary boards, quarterly meeting with sheriff, and during
training programs.
IV. On call coverage - there will be 16 hours of on call coverage available
per month from 11: 00 p.m. to 7:00 a.m. 183 hours of on call service
will be available in the 11 month period.
If there are more than 183 hours of on call service used in the 11 month
period a rate of $15.00 per hour will be billed to Weld County.
V. The above services will be provided from 2/1/82 through 12/31/82 at
a cost of $67,892.00. The billing will be $6,172.00 per month.
PROPOSAL #2
WELD COUNTY NURSING SERVICE
I. Supervisor service - registered nurse 8 hours per week. 384 hours
in 11 months - 2/1/82 to 12/31/82
II . L.P.N. Service - 112 hours per week. 5376 hours in 11 months -
2/1/82 to 12/31/82
III. A professional nurse will be available on clinic days, oral boards,
disciplinary boards, quarterly meeting with sheriff, and during
training programs.
IV. On call coverage - there will be 16 hours of on call coverage available
per month from 11:00 p.m. to 7:00 a.m. 183 hours of on call service
swill be available in the 11 month period.
If there are more than 183 hours of on call service used in the 11 month
period a rate of $15.00 per hour will be billed to Weld County.

a cost of $64,724.00 per 11 months. The billing will be $5,884.00
per month.
