RE: APPROVE REVISED NOTIFICATION OF FINANCIAL ASSISTANCE AWARD FOR
LIFESKILLS AND AUTHORIZE CHAIR TO SIGN - LUTHERAN FAMILY SERVICES
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a revised Notification of Financial
Assistance Award for Lifeskills between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Department of Social
Services, and Lutheran Family Services, commencing January 1, 2003, and ending May 31,
2003, with further terms and conditions being as stated in said revised award, and
WHEREAS, after review, the Board deems it advisable to approve said revised award, a
copy of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, ex-officio Board of Social Services, that the revised Notification of
Financial Assistance Award for Lifeskills between the County of Weld, State of Colorado, by
and through the Board of County Commissioners of Weld County, on behalf of the Department
of Social Services, and Lutheran Family Services, be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said revised award.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 20th day of January, A.D., 2003, nunc pro tunc January 1, 2003.

RE: Revision of Notification of Financial Assistance Award with
Lutheran Family Services
Enclosed for Board approval is revision of Notification of Financial Assistance Award
(NOFAA) with Lutheran Family Services. The NOFAA was discussed and reviewed at
the Board's Work Session of January 14, 2003.
The major provisions of the revised NOFAA are:
1. The term of the NOFAA remains the same ending May 31, 2003, with the revised
terms becoming effective January 1, 2003.
2. Lutheran Family Services will revise its current supervised visitation program by
providing three levels of service at the request of the Department. These levels of
service vary in the intensity of service.
3. The Department agrees to reimburse Lutheran Family Services according to rates
corresponding to their three levels of service: Supervised Visitation at a rate of
$59.00 per hour, Therapeutic Visitation at a rate of$90.00 an hour, and
Transportation at a rate of$40.00 an hour.
