RE: APPROVE GRANT APPLICATION FOR WELD COUNTY PLATICA PROJECT FOR
BREAST CANCER AND AUTHORIZE CHAIR TO SIGN -SUSAN G. KOMEN BREAST
CANCER FOUNDATION
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Grant Application for Weld County
Platica Project for Breast Cancer between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Department of
Public Health and Environment, and Susan G. Komen Breast Cancer Foundation, commencing
April 1, 2003, and ending March 31, 2004, with further terms and conditions being as stated in
said application, and
WHEREAS, after review, the Board deems it advisable to approve said application, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Grant Application for Weld County Platica Project for Breast
Cancer between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Department of Public Health and
Environment, and Susan G. Komen Breast Cancer Foundation be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said application.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 27th day of January, A.D., 2003.

Abstract
In the space below, please provide a short abstract, not to exceed 200 words, written in
lay terms for release to the general public should this application be chosen for funding.
Weld County Department of Public Health and Environment is requesting funding to
introduce an innovative approach to educating women about breast cancer. It is based on
the results of a survey funded by an American Cancer Society grant, which indicated
Hispanic women preferred receiving information either one-on-one or in small groups.
This grant would fund platicas, discussion groups, which would be held in homes and
one-on-one education, which will be done in the waiting area at the Social Services
building.
The Project Director will provide information about resources available in Weld County
for women to be screened for breast cancer, as well as educate younger women about
doing breast self-exams. Hostesses will be recruited to invite family and friends for an
educational afternoon or evening. These more personal approaches should result in an
increased number of low-income Hispanic women receiving mammograms, as well as
more knowledge about breast cancer.

Project Description:
1. This project is designed to provide education about breast self-exam and mammography
screenings to low-income Hispanic women in Weld County through one-on-one contact
and small gatherings for discussions, platicas, in homes. Women will be contacted in the
waiting area of the Weld County Social Services "Walton" Building, 315 N. 11`h Avenue,
Greeley an average of four hours a week during three weeks each month. Although the
primary outreach will be to Hispanic women, the project director will provide the
education and information to all women who are in the waiting room at Social Services.
Those women who meet the criteria(50 years of age and older or 40-49 with a relative
with breast cancer) will be offered an appointment for a free mammogram funded by the
Colorado Women's Cancer Control Initiative (CWCCI). In addition, those women
younger than 40 will be invited to view a video about breast self-exams in a small
conference room near the waiting room. They will also be given information about
CWCCI for their older relatives. Recruitment for hostesses for platicas in their homes will
be done at the same time. Once the video about breast cancer services in Weld County is
completed (if funding is provided for that grant), it will be shown at the discussion groups.
The project director would also accompany clients, as needed, to interpret at appointments.
2. Twenty-three women died of breast cancer in Weld County in 2000. According to a
National Health Interview Survey in 1992, Latinas face barriers to getting mammograms
such as lack of physician recommendation, lack of access to mammogram facilities,
language barriers, and cultural beliefs and values that do not emphasize preventive medical
care. Breast cancer rates are increasing faster for Latinas than for other women. The five-
year cancer survival rate is 70% for Latinas,versus 84% for white women. The reason for
this discrepancy is not well documented, but it may be that Latinas are more likely to be
diagnosed later, after the disease has spread. The goal of this project, working with other
projects in Weld County, is to promote knowledge that will lead to earlier detection and
higher survival rates among Latinas. Twenty-nine percent of Weld County's population is
Hispanic.
3. The people to be served with this grant are low-income Hispanic women in Weld
County. It is estimated that 30 women will have one-to-one contact with the program
director each week in the waiting area at the Social Services. Approximately once a month
a discussion group will be held in a home, with an average of six women attending. 36
weeks x 30 women = 1080 women. 12 months x 6 women= 72 women. More than 1100
low-income Hispanic women will be contacted personally about breast cancer screening
through this grant.
4. Objective 1: 30 women a week will be contacted one-on-one about breast self-exam
and mammograms in the waiting area of the Social Services building. Out of the 30, at
least five will sign up for mammograms and pap tests offered by CWCCI, and 10 will view
the breast self-exam video.
Measure of accomplishment: The project director will spend four hours per week, three
weeks each month, contacting women at Social Services. Daily records will be kept.
Objective 2: 12 discussion groups, or platter's; will be held in homes of Hispanic women
during the grant year.
Measure of accomplishment: Records of the groups and attendance will be kept, along
with numbers related to women who sign up for mammograms and view the videos.
5. This project will be an expansion of the Colorado Women's Cancer Control Initiative
(CWCCI)program currently housed locally at the Weld County Department of Public
Health and Environment. CWCCI provides funding for community outreach, as well as
mammograms and pap tests for eligible women. Funds from the Komen grant would allow
the project director to spend more time doing one-on-one consultation, as well as introduce
the new program of discussion groups, or platicas. Platicas have been used successfully in
Larimer County for health education with Hispanic women. The project director would
meet with the Larimer county director to gain insight and tips for successfully using the
platica/discussion group method of health education.
6. Initial contact has already been made with the Department of Social Services and a
letter supporting this collaboration is included with the grant. During the past year, the
project director spent eight hours talking with women in the waiting area, and during that
time signed up nine (9) for mammograms. Although the basic plan is to do primarily one-
to-one education, it may develop that the discussion groups are more successful, and later
in the year more platicas will be organized. The project is ready to be implemented once
notification of funding is received.
7. The Colorado Women's Cancer Control Initiative will provide information, as well as
mammograms or pap tests for eligible women(50 years of age or 40 years old with risk
factors). This program will expand the education outreach to women of all ages. If the
video highlighting resources for cancer screening and treatment in Weld County is
developed (by another group requesting funding from a Komen grant) then that video will
become a part of the educational materials used with this project. The Department of
Social Services is happy to provide the opportunity for their clients to receive this
education, as well as provide space to meet individually and show the video.
8. We expect the Colorado Women's Cancer Control Initiative to continue to provide
funding and will also apply for other grants. Education about the program will be provided
to public health nurses and educators so they can inform clients during clinic visits or
community presentations.
9. There are other programs in Weld County that provide some education and screenings
for low-income Hispanic women. For example, in addition to the screenings mentioned
previously provided by CWCCI, Sunrise Community Health Center provides breast cancer
screenings for women 40-49 with no risk factors. During the last funding cycle, the
American Cancer Society received a Komen grant to survey Hispanic women. This
program has been developed based on the results of that survey, which support the need for
one-on-one or small group education in Spanish.
10. The program's success will be measured by the number of women contacted one-on-
one for education, as well as the number of women who get mammograms. This will
indicate an increase in interest in breast cancer screenings among Hispanic women.
11. The results of this project could be incorporated into statewide cancer control
programs such as CWCCI. If one-on-one and small group education appears to be doable
and successful, then more funds could be used for that. The project director will approach
the Greeley Tribune about an article featuring the discussion groups,platicas, as an
innovative method of health education. KGRE will also be approached about doing a talk
show to recruit hostesses for the platicas.

Budget Justification
Project Director: Olivia Martinez has been the coordinator of the Colorado Women's
Cancer Control Initiative in Weld County for two years. She is currently employed 20
hours a week at the Weld County Department of Public Health and Environment for the
CWCCI position. Olivia is bilingual. With this grant, Olivia would work an additional
six hours a week, building on and expanding the current program. The budgeted amount
reflects the cost of the six additional hours per week.
Supplies: Each plktica/discussion group hostess will be given $50 for her effort and to
provide simple refreshments for the women who attend the gathering in her home.
Additionally, a few healthy snacks such as small boxes of fruit juice and animal crackers
will be purchased to serve to the women and their children while they are watching the
breast-self exam video at Social Services.
Equipment: Funds will be used to purchase a combination TV/VCR to show the breast
self-exam video at Social Services, as well as used in the homes for the discussion groups
if there is no TV/VCR available.
Travel: Mileage for travel to homes, to pick up supplies, to Loveland to discuss plkticas
with the group that is currently using that health education model, to transport clients to
appointments and interpret for them, as needed. Current mileage reimbursement rate is
$.34 per mile. The grant allows for 800 miles for the year.
Indirect costs: County policy requires indirect costs to be allocated for each grant. For
this grant, it will cover telephone, office space, computer access, and support staff, and
will also cover the expenses of staff meetings and other"in-house" activities that will not
need to be billed as work time for the grant.

Breast Cancer Foundation grant. The contract period would be for 12 months,

Weld County Department of Public Health and Environment will use this funding
to provide one-on-one and small group education and information about breast
cancer prevention and screenings.
