SITE SPECIFIC DEVELOPMENT PLAN AND USE BY SPECIAL
REVIEW (USR) QUESTIONNAIRE
The following questions are to be answered and submitted as part of the USR application. If a question does not
pertain to your use, please respond with "not applicable", with an explanation as to why the question is not
applicable.
1. Explain, in detail, the proposed use of the property.
2. Explain how this proposal is consistent with the intent of the Weld County Code, Chapter 22
(Comprehensive Plan).
3. Explain how this proposal is consistent with the intent of the Weld County Code, Chapter 23 (Zoning)and
the zone district in which it is located.
4. What type of uses surround the site? Explain how the proposed use is consistent and compatible with
surrounding land uses.
5. Describe, in detail, the following:
a. How many people will use this site?
b. How many employees are proposed to be employed at this site?
c. What are the hours of operation?
d. What type and how many structures will be erected (built)on this site?
e. What type and how many animals, if any, will be on this site?
f. What kind (type, size, weight) of vehicles will access this site and how often?

USE BY SPECIAL REVIEW QUESTIONNAIRE
1. The property will be initially used for a church. Future uses may include
daycare/private school, parsonage and playgrounds.
2. Consistent with Weld County Code, Chapter 22, Comprehensive Plan. Does not take
prime agricultural land since it is only 5 acres and it is at the end of the pivot.
3. This special use would comply with the special use permitted in 23-3-40 Section E (1).
4. Most of the land surrounding the area is agricultural. One mile West of the land is the
Hawkstone Subdivision of Eaton. There are also approximately 10 homes between 1-
2 miles south of the property. The church would be available to the Eaton community
as well as the rural community.
5. a. Initially we are a congregation of about 40 people. We are hoping to facilitate up
to 350 people.
b. Approximately 10 employees. Most workers would be volunteers.
c. We would use the facilities mainly on Sunday 9:00 a.m.-9:00 p.m. and Wednesday
Evenings from 6:00 p.m.-9:00 p.m. Once we had a daycare/school the hours
would have to change to daily hours 6:30 a.m.- 6:30 p.m.
d. Building 1: 50'x100' building to start a church in. Later building could be used
for daycare/school.
Building 2: Main Sanctuary approximately 80' x 80'.
Building 3: Parsonage for family approximately 50' x 30'.
e. Only family pets for parsonage (maximum 2) •
£ Mainly passenger cars and vehicles. Future use may include Passenger
Vans/buses.

h. North Weld has water tap set aside for this property. (We have letter from them.)

6. Trees and shrubs with a minimal amount of grass for children to play on. Drought
conditions would be considered in determining what types of other landscaping would be
used.

8. The North and East Side of the property have ditches in which the storm water can
drain.
9. Building #1 will be ready to go as soon as permitted. Landscaping will also start at
that time. We hope to be in the building by Fall of 2003. Other buildings will be
completed when needed and funds are available.

Michelle,
I have found some information regarding the maximum number of students and
teachers for the private school as requested.
First, I contacted the Department of Education who gave me Wayne Warren's
phone number at Colorado Department of Labor& Employment Oil & Public Safety
Division Public Safety Section at 303-318-8499.
Mr. Warren left me a message stating we were to use the Uniform Building Code
1997 Edition in order to figure out our maximum occupancy for our school.
According to Table 10-A the most that could occupy the school at a time is 159
students.
Square footage of rooms used for school =3182
Divided by the Occupant Load Factor* 20

The Colorado Department of Education also has staff-child ratios depending on
the age of the children so these regulations would be followed in determining
how many teachers and children we would have.
Hopefully, this information is adequate. Thanks for your help and let us
know if you need anything else before our meeting with the county commissioners.
