RE: APPROVE AMENDMENT TO COLORADO WORKS PROGRAM AGREEMENT FOR
SERVICES AND AUTHORIZE CHAIR TO SIGN - ISLAND GROVE REGIONAL
TREATMENT CENTER, INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Amendment to the Colorado Works
Program Agreement for Services between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Department of
Social Services, and Island Grove Regional Treatment Center, Inc., commencing upon full
execution of said amendment, and ending March 31, 2004, with further terms and conditions
being as stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, ex-officio Board of Social Services, that the Amendment to the
Colorado Works Program Agreement for Services between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Department of Social Services, and Island Grove Regional Treatment Center, Inc., be, and
hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 4th day of June, A.D., 2003.

AMENDMENT
COLORADO WORKS PROGRAM AGREEMENT FOR SERVICES
BETWEEN THE WELD COUNTY DEPARTMENT OF SOCIAL SERVICES AND
ISLAND GROVE REGIONAL TREATMENT CENTER, INC.

This AMENDMENT to Colorado Works Program Agreement for Services,made and entered into
the OCday of June 2003,by and between the County of Weld,a political subdivision of the State of
Colorado,by and through the Board of County Commissioners of the County of Weld, sitting as the Board
of Social Services for the Weld County Department of Social Services,hereinafter referred to as"Social
Services";and Island Grove Regional Treatment Center,Inc.,hereinafter referred to as"Contractor."
WITNESSETH
WHEREAS, Social Services and the Contractor entered into a Colorado Works Program
Agreement for Services on June 26,2002,to expand educational access in Greeley and South Weld County
for the benefit of both Colorado Works clients and Social Services staff through a collaborative
environment with the Contractor and to solve a technology barrier that prevents such access expansion
under a one-year phase-in plan;and
WHEREAS,the Contractor experienced delays in completing its new Greeley facility that would
enable the technology link to become operational with South Weld County;and
WHEREAS,the Contractor expects to move into its new Greeley facility by June 1,2003;and
WHEREAS,Social Services and the Contractor desire to extend the length of the Agreement and
certain work outcomes to enable the technology project to be positively completed.
NOW,THEREFORE,in consideration of the premises,the parties hereto covenant and agree as
follows:
1. Item 1,Term,of Agreement will be extended and will end March 31,2004.
2. Exhibit A,Scope of Services, Item 4,Role of Contractor,B, shall be amended by
deleting the date of December 31,2002,and adding the date of December 31,2003. The
date of June 30,2003,will retain to provide an initial survey of clients.
3. Exhibit A,Scope of Services,Item 4,Role of Contractor C,2),shall be amended to
delete the date of July 31,2002,regarding the requirement of a benchmark survey with
Social Services survey. The date of June 15,2003,will be retained to provide for an
initial survey. The date of March 15,2004,will be added to provide for a fmal
benchmark survey.

4. This Amendment,together with the original Colorado Works Program Agreement for
Services,constitutes the entire understanding among the parties with respect to the
subject matter hereof, and may not be changed or modified except as stated in the
Colorado Works Program Agreement.

RE: Amendment of Colorado Works Program Agreement for Services
Between Weld County Department of Social Services and Island Grove
Regional Treatment Center, Inc.
Enclosed for Board approval is an Amendment of a Colorado Works Program Agreement for
Services between the Weld County Department of Social Services (Department) and Island Grove
Regional Treatment Center, Inc., (Island Grove). The Amendment was discussed and reviewed at
the Board's Work Session of May 28, 2003.
The Depa,tment is requesting approval of an Amendment to the current Agreement with Island
Grove,which was funded with Colorado Works(TANF)Funding. The purpose of the
Amendment is to extend the term of Agreement from June 30, 2003, to March 31, 2004, and to
change deadline dates for certain client and Social Services staff surveys.
The original Agreement provided partial funding of$2,795 of a$6,500 project for better linkage
between the South County(Ft. Lupton) and Greeley Microwave links of Island Grove and Weld
County. This technology link was hampered by the building construction delays of Island Grove
Greeley facility in the Weld Business Park. All other provisions of the original Agreement
remain the same
