RE: APPROVE AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES
AND AUTHORIZE CHAIR TO SIGN - NORTH RANGE BEHAVIORAL HEALTH
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Amendment to the Child Protection
Agreement for Services between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Department of Social
Services, and North Range Behavioral Health, commencing June 1, 2002, and ending May 31,
2003, with further terms and conditions being as stated in said amendment, and
WHEREAS, after review, the Board deems it advisable to approve said amendment, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, ex-officio Board of Social Services, that the Amendment to Child
Protection Agreement for Services between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Department of
Social Services, and North Range Behavioral Health be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said amendment.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 28th day of May, A.D., 2003, nunc pro tunc June 1, 2002.

AMENDMENT TO CHILD PROTECTION AGREEMENT FOR SERVICES
BETWEEN THE WELD COUNTY DEPARTMENT OF SOCIAL SERVICES AND
NORTH RANGE BEHAVIORAL HEALTH
THIS Amendment made and entered into thi ay of May 2003,by and between the Board of Weld
County Commissioners,on behalf of the Weld County Department of Social Services,hereinafter referred to as
"Social Services,"and North Range Behavioral Health,hereinafter referred to as"North Range Behavioral Health."
WITNESSETH:
WHEREAS,the parties desire to modify their current Agreement(FY02-12000) in order to increase
funding to North Range Behavioral Health and eliminate funding for the Foster Parent Consultation Program.
WHEREAS, Social Services and North Range Behavioral Health entered into a Child Protection
Agreement for Services on June 6, 2002,to provide psychological evaluation and assessment services for families,
children,and adolescents referred by Social Services to North Range Behavioral Health; and
WHEREAS, Social Services has increased the number of referrals to North Range Behavioral Health for
services;and
WHEREAS, Social Services agrees to reimburse North Range Behavioral Health to cover its expenses
associated with increased referrals by Social Services; and
WHEREAS,North Range Behavioral Health agreed,under the original Child Protection Agreement for
Services,to transfer an amount of Seventeen Thousand Dollars($17,000)to the Foster Parent Consultation Program;
and
WHEREAS, Social Services and North Range Behavioral Health agree that resources no longer exist to
transfer any monies to the Foster Parent Consultation Program under the Agreement.
NOW THEREFORE,in consideration of the premises,the parties hereto covenant and agree as follows:
1. Exhibit B,Payment Schedule,Item 1,Funding and Method of Payment,will be revised as
follows:
Social Services agrees to increase reimbursement to North Range Behavioral Health,in
consideration for work and services performed, for a total amount not to exceed Ninety-seven
Thousand and Five Hundred Forty-four Dollars($97,544.00),under Child Welfare Services and
Family and Children's Program Funding.
Expenses incurred by North Range Behavioral Health, if Family and Children's Program funds,
whether in whole or in part, are subject to and contingent upon the continuing availability of
Family and Children's Program funds for the purpose hereof. In the event that said funds, or any
part thereof,become unavailable as determined by Social Services, Social Services may
immediately terminate this Contract, or amend it accordingly.
2. Exhibit B,Payment Schedule,Item 2,Fees for Service,will be revised as follows:
Under Psychological Evaluation and Caseworker Consultation Services, Social Services agrees to
pay North Range Behavioral Health for the costs of services at an hourly rate of $83.20,not to
exceed Ninety-seven Thousand and Five Hundred Forty-four Dollars($97,544.00).
This Amendment,together with the original Child Protection Agreement for Services,constitutes
the entire understanding among the parties with respect to the subject matter hereof, and may not
be changed or modified except as stated in the Child Protection Agreement for Services.

RE: Amendment of Child Protection Agreement or Services Between eld
County Department of Social Services and North Range Behavioral
Health
Enclosed for Board approval is an Amendment Child Protection Agreement for Services between
the Weld County Department of Social Services (Department)and North Range Behavioral
Health. The Amendment was discussed and reviewed at the Board's Work Session of May 12,
2003.
The Department is requesting approval of an Amendment to the current Agreement with North
Range Behavioral Health,which is funded with Core Services Funding. The purpose of the
Amendment is to increase funding to North Range Behavioral Health to a maximum funding
level from$80,544 to $97,544 and at an hourly rate of$83.20. North Range Behavioral Health
will provide:
A. Psychological evaluations and assessment services for families, children, and adolescents
who are non-Medicaid eligible,but who require child welfare services; and
B. Consultation services,through a licensed or licensed eligible psychologist, for case
workers regarding the integration of assessment results into overall care plans; and
C. Court testimony for clients who were provided services under the Agreement and have
either open or closed cases.
Partial funding of$17,000 for the Foster Care Consultation Program was deleted from this
Agreement and funded wholly under its categorical Core Services funding. All other provisions
of the original Agreement remain the same including the term of the Agreement, which is June 1,
2002 through May 31, 2003.
