Subject: Senior Nutrition Agreement with Trinity Episcopal Church
Enclosed for Board approval is a Non-Financial Agreement between the
Division of Human Resources' Senior Nutrition Program and the Trinity
Episcopal Church.
The Agreement provides for the Trinity Episcopal Church to be a congregate
meal site in conjunction with the Eldergarden PLvyLam. The Senior Nutri-
tion Program will have use of the church's facilities as a neal site Monday
through Friday of each week from 10:00 a.m. to 2:00 p.m.

MEMORANDUM OF AGREEMENT
t
THIS AGREEMENT, made and entered into this 3rd day of January ,
1983, by and between the Weld County Division of Human Resources Senior
Nutrition Program, hereinafter, designated as the "Project," and the Trinity
Episcopal Church, hereinafter referred to as the "Site. "
WITNESSETH:
WHEREAS, the parties hereto desire to work together for the express
purpose of providing nutrition services to the senior citizens in the
Eldergarden Program; and
WHEREAS, the parties desire to provide a congregate meal site for
senior citizens in the Eldergarden Program; and
WHEREAS, to implement these goals, the parties desire to establish
an Agreement for the use of the Trinity Episcopal Church for the Weld
County Senior Nutrition Program;
NOW THEREFORE, in consideration of the premises, the parties hereto
covenant and agree as follows:
1. The Site agrees to allow the Project to use its facilities at no
cost as a congregate meal site on Monday, Tuesday, Wednesday, Thursday,
and Friday of each week, 10:00 a.m. to 2:00 p.m.
2. The Project will work in conjunction with the University of
Northern Colorado to provide meals.
3. Representatives of the Colorado State Department of Social Services
may make inspections of congregate meal sites at any reasonable time.
4. This Agreement may be amended at any time with the consent of both
parties.
5. This Agreement may be terminated upon thirty (30) days written
notice by either the Project or the Site.
