RE: APPROVE HIPAA BUSINESS ASSOCIATES STANDARD AGREEMENTS AND
DELEGATE AUTHORITY TO SIGN AGREEMENTS TO THE PRIVACY OFFICERS
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,by Resolution#2003-0902, the Board of County Commissioners approved the
Health Insurance Portability and Accountability Act (HIPAA) Compliance Plan for Weld County for
Dental,Vision, and Flexible Spending Plans, and the HIPAA Compliance Plan for the Departments
of Public Health and Environment, Paramedic Services, and Human Services as health care
providers, and
WHEREAS, included in said HIPAA Compliance Plans are copies of standard agreements
to be executed by Privacy Officers on behalf of Weld County for any Business Associate having
access to HIPAA protected information and doing business with Weld County, and
WHEREAS, after review, the Board deems it advisable to approve the standard
agreements, copies of which are included in the HIPAA Compliance Plans approved by
Resolution #2003-0902.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the standard agreements to be executed on behalf of Weld County for any
Business Associate having access to HIPAA protected information and doing business with Weld
County, which are included in the HIPAA Compliance Plans be, and hereby are, approved as to
content.
BE IT FURTHER RESOLVED by the Board that authority to sign said agreements on behalf
of Weld County be, and hereby is, delegated to the HIPAA Privacy Officers, as appointed, for the
various Weld County HIPAA Plans.

The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 7th day of April, A.D., 2003.
