RE: APPROVE TASK ORDER CHANGE ORDER LETTER#1 FOR WEST NILE VIRUS
AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Task Order Change Order Letter#1
for West Nile Virus between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Weld County Department of Public
Health and Environment, and the Colorado Department of Public Health and Environment,
commencing July 1, 2002, and ending June 30, 2003, with further terms and conditions being
as stated in said change order, and
WHEREAS, after review, the Board deems it advisable to approve said change order, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Task Order Change Order Letter#1 for West Nile Virus
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby
is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said change order.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 26th day of March, A.D., 2003, nunc pro tunc July 1, 2002.

This Task Order Change Order Letter is issued pursuant to paragraph F. of the Master Contact identified as contract routing
number 00 FAA 00008 and paragraph E.1. of the Task Order identified as contract routing number 01 FHA 00076 and contract
encumbrance number PO FHA EPI0100076. This Task Order Change Order Letter is between the COLORADO
DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT and the BOARD OF COUNTY COMMISSIONERS OF
WELD COUNTY,(a political subdivision of the state of Colorado),for the use and benefit of the WELD COUNTY
DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT. The Task Order has been amended by Task Order Option
to Renew Letter 1,contract routing number 02 FHA 00059,and Task Order Option to Renew Letter 2,contact routing number
03 FHA 00062. The Task Order,as amended,is referred to as the"Original Task Order". This Task Order Change Order
Letter is for the current term of July 1,2002 through June 30,2003. The maximum amount payable by the State for the work
to be performed by the Contractor during this current term is increased by SIX THOUSAND SIX HUNDRED NINETY ONE
DOLLARS,f$6,691.001 for an amended total financial obligation of the State of ONE HUNDRED NINE THOUSAND
EIGHT HUNDRED TWELVE DOLLARS,($109,812.00). The revised Budget is incorporated herein by this reference,made
a part hereof,and attached hereto as"Attachment A".The first sentence in paragraph C of the Original Task Order is modified
accordingly. All other terms and conditions of the Original Task Order are reaffirmed. This change to the Task Order shall be
effective upon approval by the State controller,or designee,or on March 17.2003 whichever is later.
Please sign,date,and return all 3 originals of this Task Order Change Order Letter by March 10,2003,to the attention of:Lisa

SUBJECT: Task Order Revision for West Nile
Virus
Enclosed for Board review and approval is a Task Order Change Order Letter from the
Colorado Department of Public Health and Environment for monitoring and surveillance
of West Nile Virus. The Task Order (attached) was signed by the Commissioners on
May 29, 2002.
The Task Order Change Order Letter increases the original Task Order by $6,691, for a
total of$38,931. Of this amount, $16,252 is for Personnel, $19,025 for supplies, and
$3,654 for indirect costs. The additional funds are considered "supplemental" due to
the increase in West Nile Virus surveillance activities conducted by staff. The funds are
passed through the Colorado Department of Public Health & Environment from the
Centers for Disease Control. The term for the Task Order remains from July 1, 2002,
through June 30, 2003.
I recommend your approval of this Task Order Renewal Letter.
