RE: APPROVE APPOINTMENT OF PRIVACY OFFICERS UNDER HEALTH INSURANCE
PORTABILITY AND ACCOUNTABILITY ACT
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, by Resolution#2003-0902,the Board of County Commissioners approved the
Health Insurance Portability and Accountability Act(HIPAA)Compliance Plan for Weld County for
Dental,Vision, and Flexible Spending Plans, and the HIPAA Compliance Plan for the Departments
of Public Health and Environment, Paramedic Services, and Human Services as health care
providers, and
WHEREAS, said HIPAA Compliance Plans require HIPAA Privacy Officers be appointed,
and
WHEREAS,the Board deems it advisable to appoint Donald D.Warden, Director of Finance
and Administration, the HIPAA Privacy Officer for the Weld County Dental, Vision, and Flexible
Spending Plans, Cheryl Weinmeister for the Weld County Department of Public Health and
Environment's HIPAA Plan, David Bressler for the Weld County Paramedic Services' HI PAA Plan,
and Eva Jewell for the Department of Human Services, Area Agency on Aging Division's HIPAA
Plan.
NOW, THEREFOR BE IT RESOLVED by the Board of Weld County Commissioners that
Donald D.Warden, Director of Finance and Administration, be, and hereby is,appointed the HIPAA
Privacy Officer for the Weld County Dental, Vision, and Flexible Spending Plans.
BE IT FURTHER RESOLVED by the Board that Cheryl Weinmeister be, and hereby is,
appointed the HIPAA Privacy Officer for the Weld County Department of Public Health and
Environment's HIPAA Plan.
BE IT FURTHER RESOLVED by the Board that David Bressler be, and hereby is,
appointed the HIPAA Privacy Officer for the Weld County Paramedic Services' HIPAA Plan.
BE IT FURTHER RESOLVED by the Board that Eva Jewell be, and hereby is, appointed
the HIPAA Privacy Officer for the Department of Human Services,Area Agency on Aging Division's
HIPAA Plan.

The above and foregoing Resolution was, on motion duly made and seconded, adopted by
