RE: APPROVE OPTION TO EXERCISE RENEWAL AND AUTHORIZE CHAIR TO SIGN -
UNIVERSITY OF NORTHERN COLORADO
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with an Option to Exercise Renewal
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the University of Northern Colorado, commencing January 1, 2003, and
ending January 1, 2005, with further terms and conditions being as stated in said renewal, and
WHEREAS, after review, the Board deems it advisable to approve said renewal, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Option to Exercise Renewal between the County of Weld,
State of Colorado, by and through the Board of County Commissioners of Weld County, on
behalf of the Weld County Department of Public Health and Environment, and the University of
Northern Colorado be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said renewal.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 29th day of January, A.D., 2003, nunc pro tunc January 1, 2003.

In accordance with Paragraphs 3.7 and 3.8 of the Agreement for Internship dated February 24, 2000, entered into
between the STATE OF COLORADO acting by and on behalf of the BOARD OF TRUSTEES OF THE UNIVERSITY OF
NORTHERN COLORADO FOR THE BENEFIT OF THE COLLEGE OF HEALTH& HUMAN SCIENCES, GUNTER HALL 1000, Greeley,
Colorado, hereinafter referred to as "UNC", and Weld County Public Health and Environment, 1555 N. 17th Avenue,
Greeley, Colorado 80631, hereinafter referred to as "AGENCY", covering the period January 1, 2000 through
January 1, 2003, UNC hereby exercises this option for an additional two-year period from January 1, 2003 through
January 1, 2005.

SUBJECT: Option to Exercise Renewal to Non-
financial Agreement with University of
Northern Colorado for Clinical Experience
and Education
Enclosed for Board review and approval is an option to exercise renewal to the non-financial
agreement between Weld County Department of Public Health and Environment and the
University of Northern Colorado (UNC).
The original agreement outlined each agency's responsibilities regarding UNC students from the
College of Health and Human Science who will be receiving clinical or internship education and
experience at the Health Department. This option to exercise renewal will extend the time period
for this agreement for another two years from January 1, 2003 through January 1, 2005.
