RE: APPROVE CONTRACT FOR SERVICES FOR YOUTH RISK BEHAVIOR SURVEY
DATA PROJECT AND AUTHORIZE CHAIR TO SIGN - OMNI INSTITUTE
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with a Contract for Services for the Youth Risk
Behavior Survey Data Project between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Weld County Department of
Public Health and Environment, and the OMNI Institute, commencing June 1, 2003, and ending
May 31, 2004, with further terms and conditions being as stated in said contract, and
WHEREAS,after review,the Board deems it advisable to approve said contract,a copy of
which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado,that the Contract for Services for the Youth Risk Behavior Survey Data Project
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the OMNI Institute be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is,authorized to
sign said contract.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 10th day of December, A.D., 2003, nunc pro tunc June 1, 2003.

CONTRACT FOR SERVICES
Between OMNI Institute and
Weld County Department of Public Health & Environment
Entered into on June 1, 2003
The Weld County Department of Public Health & Environment ("Weld") has requested
research services from the OMNI Institute ("OMNI"). These services are outlined in an
attached document entitled "Scope of Work."
The contract period will begin June 1, 2003 and terminate May 31, 2004. By the contract
end date, all requested services, as specified in the Scope of Work, will have been
provided to Weld.
Weld has agreed to pay OMNI a total of$6,690.00 over the contract period for the
requested research services. In exchange for these services, OMNI will bill Weld in two
installments. The first request for payment will be for$4,000.00 in exchange for services
specified in points one through four in the Scope of Work. OMNI will bill Weld two
weeks prior to the first survey date. Payment is due upon receipt of the invoice. The last
and final payment will be due upon completion of the work outlined.
Parties'Relationship: The parties to this Agreement intend that the relationship between
them contemplated by this Agreement is that of independent entities working in mutual
cooperation. No employee, agent, or servant of one party shall be,or shall be deemed to be,
an employee, agent, or servant of another party to this Agreement.
Modification: This Agreement may be modified only upon written agreement by both
parties.
Each party agrees to keep any and all records and information confidential, in compliance
with all laws and regulations concerning the confidentiality of such records. Ownership of
data,reports or other works produced for Weld shall be the property of Weld with OMNI
being credited with authorship. Should OMNI and Weld decide that it would be
advantageous to publish the reports or summary or any portion of the reports,the
determination of this may be made jointly by both parties.
Either party may terminate this Agreement without cause upon thirty (30) days' prior
written notice to the other party. In the event of termination without cause, payments
shall be adjusted so that OMNI is compensated for all work relating to the Project
performed and delivered by OMNI prior to the effective date of termination. If payment
beyond the $4000.00 initial payment is owed by Weld, such payment shall be due upon
the earlier of(i) when due under the terms of this Agreement, or(ii) within thirty (30)
days following the date of an invoice therefor. OMNI shall refund any overpayments
within thirty(30) days following the date of an invoice therefore.

SCOPE OF WORK FOR RESEARCH SERVICES

The following scope of work outlines proposed services to be provided by the OMNI
Institute ("OMNI") to the Weld County Department of Public Health & Environment
("Weld").
For the contract period, June 1, 2003 through May 31, 2004, OMNI proposes to work
closely with Weld to develop a sampling approach for a countywide student survey.
OMNI will provide two sampling designs. OMNI will provide support to Weld
facilitators administering the survey, including coordination of the YRBS over-sample.
County-wide surveys will be scanned and data cleaned by OMNI. A custom report,
based on the YRBS survey instrument, will be created and used to present the data.
1. Create Survey Sampling Design
Weld requires a complex design to meet the required over-sample of the Greeley 6
School District, participation of schools in the Statewide (Integrated and Tobacco)
surveys, and a county-wide representative sample. OMNI will provide two sampling
designs, enabling Weld to choose from two cost and precision options.
2. Coordinate with CDPHE on CDC Grant Requirements
OMNI will work closely with the grant and/or survey coordinator at CDPHE to
ensure survey requirements for the CDC Steps Towards a Healthy U.S. are met. This
includes documenting required forms and procedures, strategizing about improving
student response rates, and overseeing shipping of forms to CDPHE/CDC. It may
also require OMNI to adjust the sampling strategy to include the required YRBS
instrument.
3. Provide Survey Administration Support
OMNI staff will provide guidance on assembling survey packets for each school
participating in the survey. These packets should include surveys/answer sheets,
classroom-level enrollment and attendance sheets, facilitator instructions, and return
envelopes. Sample consent letters will also be provided. (Note: Weld is responsible
for printing costs of the YRBS booklets.)
4. Collect, Scan, and Clean Data
OMNI will provide shipping labels (FedEx Ground) for the county contact to return
completed surveys for processing. OMNI will separate completed YRBS instruments
from Greeley 6 to be forwarded to CDPHE. OMNI will enter the countywide YRBS
data using its optical scan equipment. Data will be cleaned by out-of-range and
dishonesty criterion. (Note: OMNI will scan the 1500 expected surveys from
Greeley 6 as an in-kind contribution.)
5. Create Custom Report
OMNI staff will seek input from Weld to adapt current YRBS reports. At a
minimum, reports will include frequencies of all items, with details by grade and
gender.
6. Present Findings
OMNI staff will share findings with Weld and any invitees at a 1.5 hour presentation
in Weld County. This may also be used as a train-the-trainer session.

Enclosed for Board review and approval is the agreement between(the Weld County
Department of Public Health and Environment (WCDPHE) and the OMNI Institute for
collection, analysis and reporting of the Youth Risk Behavior Survey data for Weld
County schools for 2003- 2004. The total amount due for completion of the contract
services is $6690.02
The Youth Risk Behavior Survey is conducted every two years by the Centers for
Disease Control and Prevention(CDC) and the Colorado Department of Public Health
and Environment in grades 9— 12. The data is used to evaluate trends in prevalence of
high risk and related health behaviors such as violence, teen sex, smoking, alcohol,
drugs, poor nutrition and lack of physical activity on a national, state and local level.
Project planning was begun in June, 2003, and the data collection and reporting will be
completed by May 1, 2004. The funding for this project comes from general county
funds budgeted in 2003 and 2004 in Health Promotion Programs. These funds are part
of the matching funds required for the Health and Human Services Steps to a Healthier
Colorado Grant received by WCDPHE in October, 2003.
