RE: APPROVE TASK ORDER OPTION TO RENEW LETTER#2 FOR MATERNAL AND
CHILD HEALTH PROGRAM AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Task Order Option to Renew Letter#2
for Maternal and Child Health Program between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Weld County
Department of Public Health and Environment, and the Colorado Department of Public Health
and Environment, commencing October 1, 2003, and ending September 30, 2004, with further
terms and conditions being as stated in said renewal letter, and
WHEREAS, after review, the Board deems it advisable to approve said renewal letter, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Task Order Option to Renew Letter#2 for Maternal and Child
Health Program between the County of Weld, State of Colorado, by and through the Board of
County Commissioners of Weld County, on behalf of the Weld County Department of Public
Health and Environment, and the Colorado Department of Public Health and Environment be,
and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said renewal letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 6th day of August, A.D., 2003.

State Fiscal Year 2003-04. Maternal and Child Health Proliram(MCH)
This Task Order Option to Renew Letter is issued pursuant to Part F.5.of the Master Contract identified by contract routing number
00 FAA 00008 and paragraph ES.of the Task Order identified by contract routing number 02 FAA 00544 and contract
encumbrance number PO FAA MCH0200544. This Task Order Option to Renew Letter is between the
COLORADO DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT and BOARD OF COUNTY
COMMISSIONERS OF WELD COUNTY. The Task Order has been amended by Task Order Option to Renew
Letter 01 contract routing number 03 FAA 00454,if any. The Task Order,as amended, if applicable,is referred to as
the"Original Task Order". This Task Order Option to Renew Letter is for the renewal term of October 1,2003,
through September 30,2004. The maximum amount payable by the State for the work to be performed by the
Contractor during this renewal term is ONE HUNDRED FOURTEEN THOUSAND EIGHT HUNDRED
TWENTY-THREE DOLLARS ($114,823.00)for an amended total financial obligation of the State of THREE
HUNDRED FORTY-FOUR THOUSAND FOUR HUNDRED SIXTY-NINE DOLLARS ($344,469.00). This is
an increase of ONE HUNDRED FOURTEEN THOUSAND EIGHT HUNDRED TWENTY-THREE DOLLARS
($114,823.00)of the amount payable from the previous term. The Scope of Work for this renewal term is
incorporated herein by this reference,made a part hereof,and attached hereto as"Attachment 1". The first sentence
in part C of the Original Task Order is hereby modified accordingly. All other terms and conditions of the Original
Task Order are reaffirmed. This Task Order Option to Renew Letter is effective upon approval by the State
Controller,designee,or on October 1,2003 whichever is later.

As with most publicly funded agencies, we are challenged with shrinking resources and
increasing demand for services. In order to meet this challenge, the child health team continues
to be creative in our approach to reaching parents in Weld County. We are represented on the
planning committee for the Weld County Children's Festival and saw approximately 700+
children and their parents at this year's event in which we provided information on nutrition,
healthy snacks and dental care. We have had the opportunity to collaborate with other
community partners such as the Realtors' Association, Home Depot, and RSVP (Retired and
Senior Volunteer Program) to help meet some of the needs of the families we serve through
donations and volunteer work. The child health team also is represented on the HeadStart Health
Advisory Board and has collaborated to do lead testing for the Migrant HeadStart Centers and
participated in last year's Migrant Health Fair. Weld County is fortunate to have so many
community partners interested in serving children. Our partners also include primary health care
providers and we've seen a growing trend toward limited access for well-child examinations due
to the need for acute illness appointments. Also,time spent educating parents at well-child
exams is shrinking in an effort to serve more families with fewer resources. We stopped offering
well-child clinics here at the health department due to time and budget constraints and are
concerned with the loss of these educational opportunities.
We are proposing using existing immunization clinics to restore some of the education
pieces that have been lost from the childhood check up visits. Our immunization clinics are
offered three days a week in Greeley, one day a week at our Southwest County Complex on both
an appointment and walk-in basis. This provides an excellent opportunity to have contact with
parents and present them with educational materials focused on injury prevention, safety,
nutrition, and other health related topics.
We plan to use the MCH grant to supplement a percentage of staff FTE to be devoted to
the development of written materials in both English and Spanish that can be utilized by the
nurses working the clinic. The materials would be given to the parents of children getting
immunizations to give them practical information on keeping their children healthy and safe.

Perinatal Plan for Federal Fiscal year 2004
Priority Need: Increase the number of women in Weld County who receive prenatal care in the
first trimester of pregnancy.
A large portion of our MCH funds are used for the funding of prenatal care. This is due to the
large number of undocumented women in Weld County who do not qualify for Medicaid. The O
Street Clinic (which is a satellite clinic for SCHC), receives the funds for the provision of
diagnostics, consultation, operational costs, and salaries.
Poor immigrant women are more likely to delay or postpone prenatal care due to financial needs
and system distrust. The O Street Clinic provides culturally competent prenatal care with
minimal charges to the client.
Program objective: By September 30, 2004 70% of women who present to the health
department(before 13 weeks of gestation) in need of prenatal care will be seen by a
prenatal care provider in the first trimester as measured by gestational age data collection
during a follow-up contact with clients for care.
Activities:
1. One best practice is the maintenance of Medicaid Presumptive Eligibility Sites to assist
women with enrollment into Medicaid. We will provide onsite PE during department
business hours and refer a minimum of 175 pregnant women for prenatal care services to
the "O Street Clinic"by September 30,2004.
2. Contract at least $89,562.00 of the MCH funds to the "O Street Clinic" for the provision
of prenatal care for a minimum of 150 uninsured women by September 30, 2004.
3. Establish a follow-up system to assure that clients have accessed prenatal care following
referral and record the gestational age at the time the client has been seen by the provider.
4. Obtain Weld County Prams and WCDPHE data by July 31, 2003. Factors contributing to
delayed prenatal care will be obtained. One new strategy or system change will be
highlighted to decrease prenatal care barriers by September 30, 2004.
5. Prams and WCDPHE data will be presented to each member of the Perinatal Advisory
Board by September 30, 2004.
Types of Activities: Enabling and Infrastructure Building
Monitoring:
1. Number of women who receive PE/referral services.
2. Number of women who receive prenatal care at the "O Street Clinic"
3. Data results from the Prams over sample and WCDPHE prenatal barriers survey results
will be reported to the Perinatal Advisory Board.

Evaluation:
All pregnant women presenting to the health department for PE Medicaid applications will have
prenatal care status assessed at intake and entered into client record. (one month follow-up will
be made with all clients to assess number/percentage in care by end of first trimester. Results
recorded in case file.

Priority need: Reduce the teen fertility rate in Weld County
Objective: At the completion of the Sexuality Training, 75% of parents participating
In the training will indicate increased knowledge and skills in communicating with their teen or
preteens on sexuality issues.
Activities:
1. Identify "best practices" sexuality training cirriculum for teens and parents, which has
been shown to increase parent/child communication re: sexuality.
2. Conduct training for 1-2 staff members to teach cirriculum.
3. Pilot cirriculum with parent group and develop evaluation.
4. Identity and obtain reference materials for parents to use at home after presentations.
5. Provide presentations to 100 parents in"best practice" cirriculum.
6. Analyze pre-post class evaluation survey to determine percentage of parents exhibiting an
improved knowledge and skills.
Outcome Evaluation: Change in knowledge and skills of parents as measured by pre-post
survey.

SUBJECT: Task Order Renewal Letter for the Maternal
and Child Health Services Program
Enclosed for Board review and approval is the Maternal and Child Health Task Order Renewal
Letter#2 between the Colorado Department of Public Health and Environment (CDPHE) and
Weld County Department of Public Health and Environment (WCDPHE).
This renewal letter will provide continuation funding for the Prenatal and Child Health programs
at WCDPHE. The funding will be used to provide prenatal and postpartum care, including
education and counseling,to increase early entry into prenatal care, and to assist women with
enrollment in Medicaid by maintaining the Health Department's status as a Presumptive
Eligibility Site. Funding for the Child Health Program will be used to provide continuing
education for car seat safety, car seat safety inspection check points, written materials for parents
with messages of injury prevention, developmental stages, and age appropriate guidance, and one
on one education to parents in immunization clinics regarding these concerns for children.
For these services, WCDPHE will receive a total reimbursement of$114,823 for the time period
October 1, 2003 through September 30, 2004.
