RE: APPROVE RESCINDING RESOLUTION #2003-1482, DATED JUNE 9, 2003, FOR A
CONTRACT FOR EARLY AND PERIODIC SCREENING, DIAGNOSIS, AND
TREATMENT(EPSDT)SERVICES; HIPAA BUSINESS ASSOCIATE ADDENDUM;AND
ATTACHMENT A TO HIPAA BUSINESS ASSOCIATE ADDENDUM
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,by Resolution#2003-1482,dated June 9,2003,the Board approved a Contract
for the Early and Periodic Screening, Diagnosis and Treatment (EPSDT) Services Program; a
HIPAA Business Associate Addendum; and Attachment A to the HIPAA Business Associate
Addendum, on behalf of the Department of Public Health and Environment, and the Colorado
Department of Public Health and Environment, and
WHEREAS,after review,the Board deems it advisable to rescind Resolution#2003-1482,
dated June 9, 2003.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County,Colorado,that Resolution#2003-1482,dated June 9,2003,fora Contractforthe Early and
Periodic Screening, Diagnosis and Treatment (EPSDT) Services Program; a HIPAA Business
Associate Addendum;and Attachment A to the HIPAA Business Associate Addendum between the
County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Public Health and Environment, and the Colorado
Department of Public Health and Environment be, and hereby is, rescinded.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 8th day of December, A.D., 2003.
