RE: APPROVE TASK ORDER OPTION TO RENEW LETTER#4 FOR COLORADO
WOMEN'S CANCER CONTROL INITIATIVE AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Task Order Option to Renew Letter#4
for the Colorado Women's Cancer Control Initiative between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Weld County Department of Public Health and Environment, and the Colorado Department of
Public Health and Environment, commencing July 1, 2003, and ending June 29, 2004, with
further terms and conditions being as stated in said task order option to renew letter, and
WHEREAS, after review, the Board deems it advisable to approve said task order option
to renew letter, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that Task Order Option to Renew Letter#4 for the Colorado Women's
Cancer Control Initiative between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Weld County Department of
Public Health and Environment, and the Colorado Department of Public Health and
Environment be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said task order option to renew letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 16th day of June, A.D., 2003.

Task Order Renewal Letter Number 4 Contract Routing Number 04-00131
State Fiscal Year 2003-2004, CWCCI Program
This Task Order Option to Renew Letter is issued pursuant to paragraph F.1.of the Master Contract identified by
contract routing number 00-00008 and paragraph G. of the Task Order identified by contract routing number 01-
00030 and contract encumbrance number PPG0100030. This Task Order Option to Renew Letter is between the
COLORADO DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT and the BOARD OF
COUNTY COMMISSIONERS OF WELD COUNTY. The Contract has been amended by Task Order Option to
Renew Letter 3, contract routing number 03-00187,Task Order Option to Renew Letter 2,contract routing number
03-00153 and Task Order Option to Renew Letter 1,routing number 02-00107.The Contract,as amended,if
applicable, is referred to as the"Original Task Order". This Task Order Option to Renew Letter is for the renewal
term of July 1,2003,through June 29,2004. The maximum amount payable by the State for the work to be
performed by the Contractor during this renewal term is Fifty-One Thousand,Nine Hundred and Forty Dollars
($51,940.00)for an amended total financial obligation of the State of ONE HUNDRED AND THIRTY-THREE
THOUSDAND,SIX HUNDRED AND FOURTEEN DOLLARS($133,614.00). This is an increase of
($51,940.00)of the amount payable from the previous term. The first sentence in paragraph C_!.of the Original
Task Order is hereby modified accordingly. The revised Work Plan,which is attached hereto as"Attachment 1",
and the revised Budget, which is attached hereto as"Attachment 2", and the revised Duties of the State,which is
attached hereto as "Attachment 3",are incorporated herein by this reference and made a part hereof. All other
terms and conditions of the Original Task Order are hereby reaffirmed. This Task Order Option to Renew Letter is
effective upon approval by the State Controller, or designee, or on July 1,2003 whichever is later.

program. If never have had a Pap and/or mammogram ask why and

in more than 5 years OR have never discomforting but that the screenings can save her life. If client doesn't
been screened know what a mammogram is,explain that a mammogram is an X-ray of
each breast that can detect breast cancer at its earliest stage. If client
doesn't know what a Pap test is, explain that it's a small procedure in
which a few cervical cells are brushed off from the uterus opening. A
lab will check these cells for cancer or other problems. If cancer is
found early,it is easier to cure.
2B.Eligible women referred for screenings will receive a letter thanking N/A Ongoing Contact Log Data
them for calling and using the program. They will be reminded of the Screening Referral List/Screening Data
clinic they were referred and a certificate for a free a Pap test and/or
mammogram with the provider's phone number will be incl/in the letter.
2C. If a women has reservations about her first mammogram,a hand- N/A Ongoing Contact Log Data
out on facts and myths of mammograms and stats regarding the Screening Referral List/Screening Data
number of mammograms that actually end up showing breast cancer
will be sent. Hopefully,this will encourage women to keep their
appointment for a mammogram.

will be asked if she plans to go with the woman to her appointment. If
she doesn't,other options will be discussed.

CWCCI eligibility criteria:
1. Women who are age 40 years and older;
2. Women whose household income is less than or equal to 250%of the current federal guidelines established by the United States Department of Health and
Human Services. Current percent of poverty and federal poverty guidelines have been provided to the Contractor and when the guidelines and/or percents
are updated in future years that information will be provided to the Contractor by the State. The state will inform the contractor the effective date of the new
guidelines.
3. Women who do not have health insurance that would cover these services(women with Medicare-Part B coverage are not eligible for these services); and
4. Women who have not had a mammogram or Pap test in the past twelve(12)months.
Special efforts will be made to reach the rarely and never screened women. Efforts will also be made to reach priority populations who are racial, ethnic,and
cultural minorities,i.e., American Indians,African Americans,Asian/Pacific Islanders and Hispanics; lesbians; women with disabilities;and women who live in
hard to reach urban and rural areas, including those counties where less than 5%of CWCCI eligible women receive services.
ATTACHMENT 1: Workplan Part B.
1. Within fifteen(15)days following the end of the month during which services were rendered Contractor must complete and submit to the State,in a manner
and format determined by the State,a copy of all outreach data which includes,but is not limited to:
a) Outreach activities and meetings, including the dates,the types of activities,the location,the hours spent and number of women reached through one-to-
one contact, and the total number of individuals reached through outreach efforts. The total number hours worked will also be documented. For media,
the type and channel of media activity will be recorded as well as the number of media exposures.This information was previously recorded on the
Community Coordinator Contact Log.
b) Summary of the minutes from community coalition meetings and/or breast cancer advocacy group meetings.This summary will be documented on an
electronic form provided by CWCCI.The number of hours volunteered by members while attending meetings and/or conducting outreach activities will
be documented in the database.This information was previously recorded on the Community Coordinator Contact Log.
c) Hotline calls received regarding program services. The calls will be documented according to the transaction type(screening referral, face-to-face
encounter, third party referral or other). Where or how the consumer heard about the program will also be recorded.This information was previously
recorded on the Summary of Coordinator Hotline Calls form and will be used to track and evaluate outreach efforts.
d) Eligible women referred to CWCCI screening sites. Name, date of birth, address,phone number and eligibility information(household income,
insurance status,date of last exam)will be collected. Additional demographic information such as social security number,race and ethnicity is
encouraged unless refused by a woman. This information was previously recorded on the Screening Referral List and will be used to determine if
women have followed-through with recommended screening exams.
e) Documentation of in-kind or cash contributions received by the Contractor while conducting public education, outreach and media activities. Estimated
in-kind dollars will be reported in a manner and format determined by the State(eg.Affidavit of In-Kind Donation/Match form or monthly Contract
Reimbursement Statement). The types of contributions that are allowed may be in cash or in-kind,including equipment,services,or clinical
services. Matching funds may not include treatment,indirect or overhead costs,or services subsidized by the Federal government. Matching
funds information is required to maintain the covenant of the federal grant from the Centers for Disease Control and Prevention. The State shall require
a summary of this information at the end of the fiscal year(June 30, 2003).
2. Contractors are required to acknowledge the CWCCI on any informational,outreach or media materials developed and distributed through contract
activities.
3. Contractors are required to participate in one(I)conference call and two(2)training meetings,dates/locations to be determined by the State.
ATTACHMENT 2: Budget
This attachment replaces the Budget(C1.Duties and Obligations of the State) in the Original Contract.
The State shall, in consideration of those services satisfactorily and timely performed by Contractor under this Contract, cause to be
paid to the Contractor a sum not to exceed fifty-one thousand, nine hundred and forty dollars for this renewal term. Of the total
financial obligation of the State referenced above, $51,940.00 are identified as attributable to a funding source of the United States
government and $0.00 are attributable to a funding source of the state of Colorado.
The following budget shall govern the expenditure of funds by the Contractor as well as the subsequent reimbursement by the State.

ATTACHMENT 3: Duties and Obligations of the State
Section C. of the Original Contract is modified as outlined below:
C3. The State shall produce a limited number of CWCCI promotional materials (brochures, posters, pink ribbon cards), training
and technical assistance for the Contractor's personnel in conducting public education and outreach activities. The State shall
distribute statewide media releases to complement local media activities in Contractor's region. The Contractor is encouraged
to develop and utilize specific materials and strategies that are targeted for women in their community.
C4. The State shall schedule and originate one (1) conference call for the Contractor staff. The State shall also schedule and
convene two (2) training meetings.
C8. The State shall provide a list of women referred by the Contractor who have not received recommended screening exams so
that Contractor can conduct follow up activities with those women in their community. This list will be provided on a monthly
basis starting approximately six months after the first month of the contract year.
C9. The State shall pay Contractor an amount not to exceed $5.00 per woman that receives one or more screening exams (initial or
rescreen) at a CWCCI service provider. These screening referrals will be documented and submitted monthly to the State in a
manner and format determined by the State. Reimbursements will be provided to contract agency within 90 days after the end
of the contract year, June 30, 2004.

SUBJECT: Task Order Renewal Letter#4 for the
CWCCI Program
Enclosed for Board review and approval is Renewal Letter Number Four to the task order
between Colorado Department of Public Health and Environment and Weld County Department
of Public Health and Environment for the outreach program of Colorado Women's Cancer
Control Initiative.
This letter will provide renewal funding for WCDPHE to conduct public education and outreach
activities to recruit women for breast and cervical cancer screening services. It is the intent of the
task order to provide these services to women age 40 years and older whose household income is
less than or equal to 250% of the current federal poverty guidelines, who do not have health
insurance to cover these services, and who have not had a mammogram or Pap test in the past
twelve months. This task order renewal letter will fund a .75 Health Education Specialist Ito
continue the outreach program. For these services, WCDPHE will be reimbursed an amount not
to exceed $51,940 for the time period July I, 2003 through June 29, 2004.
