RE: APPROVE AMENDMENT TO PURCHASE OF SERVICES AGREEMENT AND
AUTHORIZE CHAIR TO SIGN - MEALS ON WHEELS, INC.
WHEREAS,the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with an Amendment to Purchase of Services
Agreement between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County,on behalf of the Department of Human Services,Area Agency on
Aging,and the Meals on Wheels, Inc., commencing July 1,2003,and ending June 30, 2004,with
further terms and conditions being as stated in said amendment, and
WHEREAS,after review,the Board deems it advisable to approve said amendment,a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado,that the Amendment to Purchase of Services Agreement between the County
of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County,
on behalf of the Department of Human Services,Area Agency on Aging,and the Meals on Wheels,
Inc., be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be,and hereby is,authorized to
sign said amendment.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 14th day of July, A.D., 2003, nunc pro tunc July 1, 2003.

PURCHASE OF SERVICES AGREEMENT AMENDMENT
THIS, AMENDMENT made and entered into this 1 day of July, 2003 by and between the Weld
County Division of Human Services' Area Agency on Aging, hereinafter referred to as "AAA," and
Meals on Wheels, Inc., hereinafter referred to as "MOW."
WITNESSETH:
WHEREAS, the "AAA" desires to continue to purchase home delivered meals from "MOW", and
WHEREAS, the "AAA" and "MOW" are desirous of amending the original agreement of
January 1, 2003,
NOW THEREFORE, it is mutually agreed:
1. The "AAA" will reimburse " MOW" $34,000 for 24,000 home delivered meals for 175 persons,
with a goal of reaching 50 rural elders, 30 Hispanic seniors and 175 disabled homebound seniors.
"MOW"may use up to $3,000 to maintain or purchase equipment needed in order to continue to
provide meals to homebound at risk seniors of Weld County.
2. All other terms and conditions of the original agreement shall remain in effect unless otherwise
stated in this Amendment.

3. The terms of this Amendment extend from July 1, 2003 through June 3-1 2003. This
Amendment will be terminated immediately in the event of loss of finding.

SUBJECT: Amendment to the Purchase of Service Agreement
between the Weld County Area Agency on Aging and Meals on
Wheels.
Enclosed for Board approval is an amendment to the Purchase of Service agreement between the
Weld County Area Agency on Aging and Meals on Wheels. The original contract for services
was$31,000 and this amendment will increases the service agreement to$34,000. A total
of 24,000 meals will be provided for 175 frail and homebound Weld County Seniors during
the contract period which will run from July 1, 2003 through June 30, 2004.
