RE: APPROVE THREE REVISIONS TO NOTIFICATION OF FINANCIAL ASSISTANCE
AWARDS FOR FOSTER PARENT CONSULTATION WITH VARIOUS VENDORS
AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with three Revisions to Notification of
Financial Assistance Awards for Foster Parent Consultation between the County of Weld, State
of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of
the Department of Social Services, and the following parties, commencing June 1, 2003, and
ending May 31, 2004, with further terms and conditions being as stated in said revisions:

Ackerman and Associates, P.C.
Lori Kochevar
WHEREAS, after review, the Board deems it advisable to approve said revisions, copies
of which are attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, ex-officio Board of Social Services, that the three Revisions to
Notification of Financial Assistance Awards for Foster Parent Consultation between the County
of Weld, State of Colorado, by and through the Board of County Commissioners of Weld
County, on behalf of the Department of Social Services, and the above-listed parties be, and
hereby are, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said revisions.

RE: Revision of Notifications of Financial Assi Lance Awards
(NOFAAs) with Vendors—Foster Parent Consultation
Enclosed for Board approval are revisions to Notifications of Financial Assistance
Awards (NOFAAs) with vendors for Foster Parent Consultation. The vendors are:
Lutheran Family Services, Lori Kochevar, and Ackerman & Associates. The revised
NOFAAs were discussed and reviewed at the Board's Work Session held on October 6,
2003.
The major revisions to the NOFAAs are to increase the number of foster parent groups
from a maximum of 4 to 5 per month and to lower the number of foster parents who
make up a group from 8 to 10 participants to 6 to 10 participants.
All other terms of the NOFAA remain the same.
