KOMEN BREAST CANCER FOUNDATION WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a 2004 Grant Application for Weld County Platicas Project for Breast Cancer between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Department of Public Health and Environment, and the Susan G. Komen Breast Cancer Foundation, commencing April 1, 2004, and ending March 31, 2005, with further terms and conditions being as stated in said application, and WHEREAS, after review, the Board deems it advisable to approve said application, a copy of which is attached hereto and incorporated herein by reference. Weld County Department of Public Health and Environment is requesting funding to continue an innovative approach to educating women about breast cancer. It is based on the results of a survey funded by an American Cancer Society grant, which indicated Hispanic women preferred receiving information either one-on-one or in small groups. This grant would fund platicas, or discussion groups, that are held in the women's homes and one-on-one education that is done in the waiting area at several local agencies including the Department of Social Services, the Health Department and also at health fairs. The Project Director will provide information about resources available in Weld County for women for breast cancer screenings as well as education for younger women about breast self- exams. Women will be recruited to host family and friends for an educational afternoon or evening to learn about breast cancer prevention. This more personal approach will result in an increased number of low-income Hispanic women who receive information and education about breast cancer. This project will provide education about breast self-exam and mammography screenings to low-income Hispanic women in Weld County through one-on-one contact at local agencies and small gatherings, called platicas, held in homes. The Platicas project has been effective and well received by Hispanic women in the Greeley-Evans area for the last nine months. In fact, it is so popular that more platicas are being requested. For the 2004-2005 grant period, one-on-one contact will continue at the Weld County Department of Social Services (DSS), the Weld County Department of Public Health and Environment (WCDPHE) and community health fairs. We will then expand the outreach education to new agencies including Our Lady of Peace church, Centennial BOCES and Billy Martinez elementary school. Although the primary outreach is to Hispanic women, the project director provides education and information to all women in the waiting areas at these sites and at the health fairs. Those women who meet the criteria(50 years of age and older or 40-49 with signs and symptoms of breast cancer or a first degree relative with breast cancer) are offered an appointment for a free mammogram funded by the Colorado Women's Cancer Control Initiative (CWCCI). Women 40 or younger are invited to host a platicas in their home to educate relatives and friends in the community. A new video, Mona's Story, about a local breast cancer survivor has been shown at the platicas and has been very well received. The project director will accompany clients to appointments for screening, if needed, for interpretation. 2. Data from the Colorado Central Cancer Registry show there were eleven reported cases of breast cancer in Hispanic women in Weld County in 2001 and six in 2002. According to the Susan G Komen Breast Cancer Foundation National Hispanic/Latino Advisory Council, breast cancer is less common among Hispanic/Latino women, approximately 70 per 100,000. However it is the leading cause of death in this population. The mortality rate for breast cancer for Colorado females decreased 6% from 1995-99 to 2000; the rate was down 5% among non-Hispanic whites and did not change among Hispanics, according to the Centers for Disease Control and Prevention. The five-year survival rates remain lower for Hispanic/Latino women than those in other ethnic groups. The reason may be attributable to the fact that Latinos are more likely to be diagnosed at a later stage when tumors are larger and harder to treat. Education about breast cancer screening is a particular challenge in this population because there are few bilingual education programs and many women do not have health insurance or screening is not covered. There are also cultural barriers that prevent Hispanic women from getting screened. Breast self- examination, male physician check ups and fear of pain with mammography are examples. . The project addresses these barriers and takes breast cancer education to the Hispanic woman in a culturally acceptable format, one on one and in their homes. Twenty-nine percent of Weld County's population is Hispanic and the female Hispanic population in Weld County was 23,143 in 2000. This population is growing each year and it is a challenge in Weld County to build on the progress that's been made in breast cancer education and screening. • 3. The people to be served with this grant are low-income Hispanic women in Weld County. During 2003, 731 contacts were made at the DSS, WCDPHE and in hostess's homes. Of those, 43 have qualified for a free mammogram through the CWCCI. It is estimated that a minimum of 45 women will have one-on-one contact with the program director each week in the waiting areas at the DSS, WCDPHE, and the new venues described above. The platicas discussion groups will be held monthly with a minimum of six women attending. Through this program, 1700 low-income Hispanic women will be contacted personally about breast cancer screening during the upcoming grant year. This program will help to increase knowledge of breast cancer prevention and may lead to earlier detection and higher survival rates among Latino women in Weld County. 4. Objective 1: 45 women a week will be contacted one-on-one about breast self-exam and mammograms in the waiting areas of the Social Services building, the Health Department and occasional health fairs. Of the 45, at least five will sign up for mammograms and PAP tests offered by CWCCI and at least three will sign up to host a platica. Measure of accomplishment: The project director will spend four hours per week, three weeks each month, contacting women at DSS, WCDPHE and health fairs. Records will be kept on the number of women contacted, screened and scheduled to host platicas. Objective 2: 12 discussion groups, or platicas, will be held in homes of Hispanic women during the grant year. Women who attend the platicas will receive educational materials, i.e., Do It For Life brochure, Breast Health Learn the Facts, Shower Card: Steps to Breast Self Examination,Questions About Cancer bookmark, HPV and Cervical Cancer Screening brochure, A Women's Guide to Breast Care brochure, and Latina News: a quarterly bilingual publication of Y-ME National Breast Cancer Organization and the Health Department clinic hours schedule. Measure of accomplishment: Records of the groups, attendance sheets, and evaluations will be kept, along with number of women who sign up for mammograms and view the videos. 5. This project is an expansion of the Colorado Women's Cancer Control Initiative (CWCCI)program currently housed locally at the Weld County Department of Public Health and Environment. CWCCI provides funding for community outreach, as well as mammograms and PAP tests for eligible women. Funds from the Komen grant allow the project director to spend more time doing one-on-one consultation, as well as conduct the discussion groups, or platicas for women of all ages. Platicas have been well received in the Greeley-Evans area. Through the platicas evaluations, Hispanic women have expressed their gratitude and interest to learn more about breast cancer. They have also requested education on other health topics such as prevention of Sexually Transmitted Diseases (STDs) in this setting. The platicas'project has been featured in several media • venues during the past year, i.e., the Greeley Tribune, Senior Market Place News, and LatiNoticias (1st Spanish newspaper in Weld County). 6. Over the past nine months, a good working relationship has been established with the Department of Social Services and the Health Department. Initial contacts have been made with Our Lady of Peace, Centennial BOCES, Strengthening Latino Families, and Billie Martinez School parent groups to expand the one-on-one contacts and platicas'project. Letters supporting this collaboration are included with the grant. Implementation will begin as soon as funding is received. 7. The Colorado Women's Cancer Control Initiative provides information, as well as mammograms or PAP tests for eligible women (50 years of age or 40 years old with risk factors). The Komen grant has allowed us to expand the education outreach to women of all ages. The video highlighting resources for cancer screening and treatment in Weld County has become a part of the educational materials used with this project. The video was developed by the American Cancer Society with Komen funding. The Department of Social Services, the WCDPHE, Our Lady of Peace, Centennial BOCES, Strengthening Latino Families and Billie Martinez School parents group are delighted to provide the opportunity for their clients to receive this education. They will provide the space for their clients to view the video and speak privately to the coordinator. 8. Funding from the Colorado Women's Cancer Control Initiative was increased this grant year and this has allowed more time for one-on-one contacts. Education about CWCCI and the Komen program is provided to WCDPHE public health nurses and health educators so they can inform clients during clinic visits or community presentations. We have partnered with other organizations like Our Lady of Peace, Centennial BOCES, and Strengthening Latino Families and the Department of Social Services who want to offer the breast health education to their Hispanic and migrant families who desperately need education on health topics and available community resources. 9. The platicas project is unique in the sense that it offers the opportunity for Hispanic women to receive education, express their fears and share their personal and family experiences with breast health problems. This project encourages women of all ages who are not performing regular breast self-exams to start taking charge of their health. They are encouraged to seek the professional medical assistance if they notice an abnormality even if they do not qualify for CWCCI. There are other programs in Weld County that provide some education and screenings for low-income Hispanic women. In addition to the screenings provided by CWCCI, Sunrise Community Health Center provides breast cancer screening for women younger than 40 with no risk factors at the time they have a PAP test. The Greeley X-ray Group is funding a free mammogram per month to women younger than 40 with ls`degree family history of breast cancer but not for women with signs and symptoms of breast cancer. The American Cancer Society received funding to develop a video in both English and Spanish that highlights the story of a local breast cancer survivor and the resources available in the community. • 10. Currently, the platicas project reflects an expansion of CWCCI and the collaboration with other agencies. The project's success is being measured by the number of women contacted one-on-one for education, the number of women who indicate they perform regular breast self exams and also the number who qualify for free mammograms through the CWCCI program. For the next funding year, pre and posttest questions will be included in the evaluation. Questions on the frequency of breast self-exam and how the presentation/discussion influenced the women to do regular breast self exams and seek screening will be included. This information will help us to measure the intention to do breast self-exams and seek breast cancer screenings among Hispanic women. 11. The results of this project could be incorporated into statewide cancer control programs such as CWCCI. If one-on-one and small group education appears to be successful more funds can be used for that. The project director has approached the Greeley Tribune, the Senior Market Place News, and LatiNoticias about an article featuring the discussion groups,platicas, as an innovative method of health education. This method is working well and can work for other health messages such as STDs in Hispanic communities. We would like to offer free mammograms for women who do not fit the CWCCI criteria. Currently, the Greeley X-ray Group is funding a free mammogram per month to women younger than 40 with 15`degree family history of breast cancer but not for women with signs and symptoms of breast cancer. She is currently employed 30 hours a week at the Weld County Department of Public Health and Environment for the CWCCI position and 6 hours a week for the Komen position. Olivia is bilingual. With this grant, Olivia would continue to work an additional six hours a week, building on and expanding the current programs. The budgeted amount reflects the cost of the six (6) additional hours per week. Supplies: Eachpl/tica/discussion group hostess will be given $50 for her effort and to provide simple refreshments for the women who attend the gathering in her home. Equipment: none Travel: Mileage for travel to homes, to pick up educational materials, for outreach at DSS and community health fairs. Current mileage reimbursement rate is 35.5 cents per mile. The grant allows for 302 miles for the year. Indirect costs: County policy requires indirect costs to be allocated for each grant.
