Re: Claim for Damages -Eugene Sutliff _
TO WHOM IT MAY CONCERN
Pursuant to applicable Colorado Law,Eugene Sutliff makes the following claim for damages:
(a) The name and address of the claimant and the name and address of his attorney, if any;
Claimant: Eugene Sutliff- 1008 Lilac Drive, Brighton, Colorado 80603

(b) A concise statement of the factual basis of the claim, including the date, time, place, and
circumstances of the act, omission, or event complained of;
On or about May 22, 2003, at approximately 4:00 p.m., the Claimant was driving his 1992
Dodge Spirit Automobile near the intersection ofLocus and Lilac Drive,in the Township of Lochbuie.
Apparently, at the time, there was street construction being performed. At this stage in the
construction, the street was a dirt road. Mr. Sutliff was heading in a northwesterly direction on
Locust, when he was directed to drive in certain direction by a member of the construction crew. He
followed the directions which caused his automobile to drive over a elevated man hole. As a result,
his vehicle's front right tire dropped into the man hole causing extensive damage to the undercarriage.
(c) The name and address of any public employee involved, if known;
It is unknown at this time whether the Township of Lochbuie, Street Maintenance or Weld
County Street Maintenance or a private contractor is responsible for these damages.
