RE: APPROVE TASK ORDER FOR PUBLIC HEALTH PREPAREDNESS AND
RESPONSE FOR BIOTERRORISM PROGRAM AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Task Order for Public Health
Preparedness and Response for Bioterrorism Program between the County of Weld, State of
Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the
Weld County Department of Public Health and Environment, and the Colorado Department of
Public Health and Environment, commencing September 1, 2003, and ending August 31, 2004,
with further terms and conditions being as stated in said task order, and
WHEREAS, after review, the Board deems it advisable to approve said task order, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Task Order for Public Health Preparedness and Response for
Bioterrorism Program between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Weld County Department of Public
Health and Environment, and the Colorado Department of Public Health and Environment be,
and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said task order.
The above and foregoing Resolution was, on motion duly made and seconded, adopted

TASK ORDER
Public Health Preparedness and Response for Bioterrorism Program
This Task Order is made this 29TH day of AUGUST, 2003,by and between:the state of Colorado,acting by and
through the COLORADO DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT,whose address or
principal place of business is 4300 Cherry Creek Drive South,Denver,Colorado 80246 hereinafter referred to
as"the State";and,the BOARD OF COUNTY COMMISSIONERS OF WELD COUNTY(a political
subdivision of the state of Coloradol,whose address or principal place of business is 915 106 Street,3r°Floor,
Greeley,Colorado 90631,for the use and benefit of the Weld County Department of Public Health and
Environment, whose principal place of business is 1555 North 17th Avenue,Greeley,Colorado,80631
hereinafter referred to as"the Contractor".
FACTUAL RECITALS
The United State Department of Health and Human Service, Centers for Disease Control and prevention(CDC)has
awarded the State federal funds under Notice of Cooperative Agreement Award("NCAA")number
U90/CCU816827-04-2 to fund"Public Health Preparedness and Response for Bioterrorism"activities.(See,
Catalog of Federal Domestic Assistance("CFDA")number 93.283.) The funds from this NCAA are intended to
assist the State to upgrade the ability of state and local public health agencies to prepare for,and respond to:
bioterrorism incidents,outbreaks of infectious disease, and other public health threats and emergencies.These
federal funds are intended for use in Focus Area A: "Preparedness Planning and Readiness Assessment",
"Strategic National Stockpile"("SNS"),Focus Area B: "Surveillance and Epidemiology Capacity",Focus
Area C: "Laboratory Capacity:Biologic Agents",Focus Area E:"Health Alert Network("HAN")
Communications and Information Technology",and Focus Area F: "Risk Communication and Public Health
Dissemination".The State has formulated a comprehensive State plan,with associated budgets, to disburse these
federal funds throughout the state of Colorado. Under this comprehensive State plan,the State shall allocate these
federal funds to qualified entities to assist those entities in responding to public health preparedness and response to
bioterrorism incidents,outbreaks of infectious disease,and other public health threats and emergencies.
Section 18(2)(a)of Article XIV of the Colorado Constitution and section 29-1-203 of the Colorado Statutes, C.R.S.
as amended,authorize governments to make the most efficient and effective use of their powers and
responsibilities by cooperating and contracting with each other to the fullest extent possible to provide any function,
service,or facility lawfully authorized to each of the cooperating or contracting entities. Additionally, all state of
Colorado contracts with its political subdivisions are exempt from the state of Colorado's personnel rules and
procurement code.

The Contractor is a political subdivision of the state of Colorado. The State and the Contractor mutually agree that
the most efficient and effective way to provide the above-described services is at the local level. The State and the
Contractor previously entered into a Master Contract with contract routing number 00 FAA 00008. This Task Order
is issued pursuant to the terms and conditions of that Master Contract.
As to the State,authority exists in the Law and Funds have been budgeted,appropriated,and otherwise made
available,and a sufficient uncommitted balance thereof remains available for subsequent encumbering and payment
in Fund Code 100 Organizational Unit Code 3650 Appropriation Code 369 Program Code 9012,Function Code
FLWT, and Object Code 5420 under Contract encumbrance number 04 FHA EPI0400103. All required approvals,
clearances,and coordination have been accomplished from and with all appropriate agencies.
NOW,THEREFORE,in consideration of their mutual promises to each other,stated below,the parties hereto
agree as follows:
A. PERIOD OF PERFORMANCE AND TERMINATION. The proposed effective date of this Contract is
September 1,2003. However,in accordance with section 24-30-202(1),C.R.S.,as amended,this Contract
is not valid until it has been approved by the State Controller,or an authorized designee thereof. If the
underlying NCAA authorizes the State to pay all allowable and allocable expenses of a contractor as of the
effective date of that NCAA,then the State shall reimburse the Contractor for any allowable and allocable
expenses of the Contractor that have been incurred since the proposed effective date of this Contract. If the
underlying NCAA does not authorize the State to pay all allowable and allocable expenses of a contractor
as of the effective date of that NCAA,then the State shall only reimburse the Contractor for those
allowable and allocable expenses of the Contractor that were incurred on or after the effective date of this
Contract. The initial term of this Contract shall commence on its effective date and continue through and
including August 31,2004. If the State receives additional federal funding for the purposes of this
Contract,then this Contract shall automatically renew for the period covered by those additional federal
funds. This Contract is funded entirely with federal funds. Therefore,the State's liability for payment for
all services rendered,or goods supplied,under this Contract is contingent upon the State's receipt of those
federal funds. If the State does not receive all,or any part,of the federal funds necessary to pay its
obligations under this Contract,then the State may immediately amend or terminate this Contract without
further liability to the State. The total term of this Contract, including all automatic renewals,bilateral
options to renew,and extensions,may not exceed five(5)years.
13. DUTIES AND OBLIGATIONS OF THE CONTRACTOR. The Contractor,in accordance with the
terms and conditions of the Master Contract and this Task Order,shall perform and complete, in a timely
and satisfactory manner,all work items described in SNS Workplan,Focus Area A Workplan,Focus
Area B Workplan,Focus Area C Workplan,Focus Area E Workplan,and Focus Area F Workplan.
All of these Workplans are incorporated herein by this reference,made a part hereof and attached hereto
as"Attachments A,B,C,D,E and F".

C. DUTIES AND OBLIGATIONS OF THE STATE.
1. In consideration for those services satisfactorily and timely performed by the Contractor under this
Task Order the State shall cause to be paid to the Contractor a sum not to exceed THREE
HUNDRED TWENTY EIGHTY THOUSAND THIRTY FIVE DOLLARS ($328,035.001 for
the initial term of this Task Order. Of the total financial obligation of the State referenced above,
$328,035.00 are identified as attributable to a funding source of the United States government.

The Contractor shall be compensated in accordance with the budgets set forth in Attachments A,
B,C,D,E and F hereto.
2. If this Task Order requires the Contractor to submit a"Task Order Reimbursement Statement",
then the Contractor shall submit a signed,duplicate monthly Task Order Reimbursement
Statement within sixty(60)calendar days of the end of the billing period for which goods were
delivered or services were performed . A sample Task Order Reimbursement Statement is
incorporated herein by reference,made a part hereof,and attached hereto as"Attachment G".
Expenditures shall be in accordance with those items identified in Attachments A,B,C,D,E and
F. These items may include,but are not limited to: the Contractor's salaries,fringe benefits,
supplies,travel,operating,and indirect costs which are allowable and allocable expenses related to
its performance under this Task Order.
Each Task Order Reimbursement Statement shall reference the related Master Contract and this
Task Order by their respective contract routing numbers. The contract routing numbers are
located on page one of these documents. Each Task Order Reimbursement Statement shall also
indicate the applicable performance dates,the names of payees;a brief description of the services
performed during the relevant performance dates; all expenditures incurred;and,the total
reimbursement requested. Reimbursement during the initial,or any renewal,term of this Task
Order shall be conditioned upon affirmation by the State that all goods were delivered or all
services were performed by the Contractor in accordance with the terms and conditions of this
Task Order. Each Task Order Reimbursement Statement shall be sent to:

3. Payments under this Task Order shall be made either through the State's Electronic Fund Transfer
system or,upon the Contractor's periodic submission of a duplicate"Task Order Reimbursement
Statement".
4. The State may prospectively increase or decrease the amount payable under this Task Order
through a"Task Order Change Order Letter"that is substantially similar to the sample Task Order
Change Order Letter that is incorporated herein by this reference,made a part hereof,and attached
hereto as"Attachment H". To be effective,a Task Order Change Order Letter must be: signed
by the State and the Contractor;and,approved by the State Controller or an authorized designee
thereof. Additionally,a Task Order Change Order Letter shall include the following information:
A. Identification of the related Master Contract and this Task Order by their respective
contract routing numbers and affected paragraph number(s);
B. The type(s)of service(s)or program(s)increased or decreased and the new level of each
service or program;
C. The amount of the increase or decrease in the level of funding for each service or
program and the new total financial obligation;
D. A provision stating that the Task Order Change Order Letter is effective upon approval
by the State Controller,or designee,or its proposed effective date,whichever is later.
Upon proper execution and approval,a Task Order Change Order Letter shall become an
amendment to this Task Order. Except for the General and Special Provisions of the Master
Contract,and the Additional Provisions of the Task Order,if any,the Task Order Change Order
Letter shall supersede this Task Order in the event of a conflict between the two. It is expressly
understood and agreed to by the parties that the task order change order letter process may be used
only for increased or decreased levels of funding,corresponding adjustments to service or program
levels,and any related budget line items. Any other changes to this Task Order,other than those
authorized by the task order option to renew letter process described below,shall be made by a
formal amendment to this Task Order executed in accordance with the Fiscal Rules of the state of
Colorado.
If the Contractor agrees to and accepts a proposed Task Order Change Order Letter,then the
Contractor shall execute and return that Task Order Change Order Letter to the State by the date
indicated in that Task Order Change Order Letter. If the Contractor does not agree to and accept a
proposed Task Order Change Order Letter,or fails to timely return a partially executed Task Order
Change Order Letter by the date indicated in that Task Order Change Order Letter,then the State
may,upon written notice to the Contractor,terminate this Task Order no sooner than thirty(30)
calendar days after the return date indicated in the Task Order Change Order Letter has passed.
This written notice shall specify the effective date of termination of that Task Order. If a Task
Order is terminated under this clause,then the parties shall not be relieved of their respective
duties and obligations under that Task Order until the effective date of termination has passed.

Increases or decreases in the level of contractual funding made through the task order change
order letter process during the initial, or renewal,term of a Task Order may only be made under
the following circumstances:
E. If necessary to fully utilize appropriations of the state of Colorado and/or non-
appropriated federal grant awards;
F. Adjustments to reflect current year expenditures;
G. Supplemental appropriations,or non-appropriated federal funding changes resulting in an
increase or decrease in the amounts originally budgeted and available for the purposes of
a Task Order;
H. Closure of programs and/or termination of related contracts or task orders;
I. Delay or difficulty in implementing new programs or services;and,
J. Other special circumstances as deemed appropriate by the State.
5. The State may renew a Task Order through a"Task Order Option to Renew Letter"substantially
similar to the sample Task Order Option to Renew Letter that is incorporated herein by this
reference,made a part hereof,and attached hereto as"Attachment I". To be effective,a Task
Order Option to Renew Letter must be: signed by the State and the Contractor;and,approved by
the State Controller or an authorized designee thereof. Additionally,a Task Order Option to
Renew Letter shall include the following information:
A. Identification of the related Master Contract and that Task Order by their respective
contract routing numbers and affected paragraph number(s);
B. The type(s)of service(s)or program(s),if any,increased or decreased and the new level
of each service or program for the renewal term;
C. The amount of the increase or decrease,if any,in the level of funding for each service or
program and the new total financial obligation;
D. A provision stating that the Task Order Option to Renew Letter is effective upon
approval by the State Controller,or designee,or its proposed effective date,whichever is
later.
Upon proper execution and approval,a Task Order Option to Renew Letter shall become an
amendment to this Task Order. Except for the General and Special Provisions of the Master
Contract,and the Additional Provisions, if any of that Task Order,a Task Order Option to Renew
Letter shall supersede that Task Order in the event of a conflict between the two. It is expressly

terms and conditions of this Task Order and those of any attachment or exhibit hereto,then the
terms and conditions of this Task Order shall control.

Bioterrorism: Strategic National Stockpile (SNS)
Contract Term: September 1, 2003 thru August 30, 2004
2003-2004 SNS Objective: To ensure state, regional, and local preparedness and
planning (including management, security, and training) should the Strategic National
Stockpile be implemented).
In accordance with Cooperative Agreement U90/CCU 816827-04, the
Contractor shall perform the following duties:
I. Contractor shall continue to develop and augment their local SNS preparedness
program by participating in all SNS preparedness planning, meetings,
orientations, and trainings, as requested and scheduled by CDPHE. Ongoing until
August 30, 2004.
2. Contractor shall collaborate with their Regional Planner, as requested, and
provide information on local resources (to include personnel, storage and
dispensing facilities, security, and transportation resources as well as any
additional information that may be required for the Regional Planner to complete
the regional SNS plan)by December 1, 2003.
3. In conjunction with activity#12 in the Focus Area A workplan; Contractor shall
document their participation in all local, regional, and state SNS preparedness
exercises, as requested or required by CDPHE.
a. Individual counties will dictate timelines for local SNS exercises.
b. All regional SNS exercises will be completed by May 31, 2004. Local
health departments will exercise a portion of their mass vaccination clinic
plans in conjunction with the regional exercise.
c. The statewide functional exercise will occur sometime during June or July
2004. Testing of the state and regional SNS plans will be part in this
exercise and will include the Training, Education, and Demonstration
(TED) package.
4. In conjunction with activity#10 in the Focus Area A workplan; Contractor shall
augment local mass vaccination plans to accommodate larger quantities of any
vaccine which may potentially arrive as part of the overall SNS package by
December 1, 2003.

5. Contractor shall provide to the State four(4)narrative progress reports and one
final report, which will summarize all accomplishments, and are due within five
(5) business days after the end of the period.December, March,June and August
billings will not be reimbursed until such time as delinquent progress reports have
been received. Reports shall be submitted to the State to the attention of:

Contractor shall continue to house a Regional Planner to serve the Northeast Region,
including: Kit Carson, Larimer, Lincoln,Logan,Morgan, Phillips,Sedgwick,
Washington,Weld and Yuma counties. Contractor is responsible for ensuring the
Planner successfully performs the following duties:
a. In conjunction with activity "a"of the Focus Area A workplan, Planner shall
collaborate with all other Regional Planners,Epidemiologist(s), Trainer(s) and
Focus Areas funded under this Cooperative Agreement within and between
regions, when appropriate and upon request in all SNS preparedness activities
and deliverables. Ongoing until August 30, 2004.
b. In conjunction with activity"b" of the Focus Area A workplan,Planner shall
document and provide technical assistance to all counties within their region,
as appropriate and as requested to ensure the timely completion of all SNS
preparedness deliverables and activities. However,Planner is not responsible
for actually preparing deliverables or conducting activities that are the
responsibility of the Contractor. Ongoing until August 30, 2004.

c. Planner shall update and append their regional SNS plan by December 1, 2003
to include (but not limited to)the following information:
• All appropriate Memoranda of Understanding;
• An Organizational Chart of key personnel and the functions of
each;
• All local SNS risk communication plans;
• Physical addresses of all dispensing sites or clinics in their
region;
• Transportation routes to all dispensing sites; and
• Mass prophylaxis/immunization clinic plan that allows for
larger quantities of vaccine that may potentially anive as part
of the SNS package.
d. Planner shall assist all counties within their region, as requested or required, in
updating and enhancing their local/county mass vaccination clinic plans to
accommodate larger quantities of the vaccine that may potentially arrive as
part of the overall SNS package by December 1, 2003.
e. In conjunction with activity "o"of the Focus Area A workplan, Planner shall
document their participation in all local (within their region), regional, and
state SNS preparedness exercises, as requested or required by CDPHE.
Additionally, Planner shall document their participation in the development
and execution of all SNS exercises held within their counties.
• Individual counties will dictate timelines for local exercises.
• All regional exercises will be completed by May 31, 2004.
Regional tabletops will be developed by the statewide exercise
contractor and launched by regional planners with participation
from all indicated partner agencies prior to the statewide
functional exercise. Local health departments will exercise a
portion of their mass vaccination clinic plans in conjunction
with the regional exercise.
• The statewide exercise will occur sometime during June or July
2004. Testing of the state and regional SNS plans will be part
of this exercise and will include the TED package.

f. In conjunction with the Regional Training Coordinator and activity "s"of the
Focus Area A workplan, Planner shall schedule and conduct no less than two
(2)NIMS trainings in their region by August 30, 2004. Dates of the regional
trainings are contingent upon completion of activity "s"of the Focus Area A
workplan. In the event that the DHS does not hold this training during the
2003-2004 grant period,Planner shall ensure their agency requests approval
from the Focus Area A Manager or Focus Area A Coordinator for monies to
be reallocated and used for additional leadership/management training or
advanced concepts in incident command training.
g. Planner shall document assistance to all counties within their region, as
requested, to develop or enhance a local SNS risk communication plan by
December 1, 2003.
h. Planner shall continue to develop and augment their regional SNS
preparedness program by participating, as requested, in all SNS preparedness
activities and deliverables. Ongoing until August 30, 2004.

The following budget governs the expenditure of funds for the period of September 1,2003 through
August 31,2004 by the Contractor,as well as the subsequent reimbursement by the State.
These dollars will be paid monthly on a cost reimbursement basis only.Contractor shall submit
copies of receipts for all purchases.Transfer of funds from one line item to another is permissible.
Transfer of funds between focus areas must have prior written approval by the State.
Billings must be submitted by focus area.

Contract Term: September 1, 2003 thru August 30, 2004
2003-2004 Focus Area A Contract Objective: To ensure state, regional, and local
collaboration as well as preparedness for and response to bioterrorism, other outbreaks of
infectious disease, and other public health threats and emergencies.
In accordance with Cooperative Agreement U90/CCU 816827-04, the
Contractor shall perform the following duties:
1. Contractor shall document four(4)quarterly meetings/discussions with their
Regional Planner and provide meeting documentation, including all participant
contact information, meeting minutes, and any follow-up activities by August 30,
2004.
2. Contractor shall document one (1) meeting with each of the following partners:
the county and/or city emergency manager, the sheriff's office, the Regional
Emergency and Trauma Advisory Council (RETAC), and all hospital emergency
planners and provide meeting documentation, including all participant contact
information, meeting minutes, preparedness outcomes, and any "follow-on"
activities by August 30, 2004. Discussions with non-public health and partner
agencies (i.e. fire departments/districts, coroner/medical examiners, and
veterinarians) are strongly encouraged. Ongoing until August 30, 2004.
3. The Office of Local Liaison (OLL) - Colorado Public Health Capacity Building
Steering Committee will begin Phase II of the assessment of the ten (10)essential
services of public health and supplemental assessment of public health emergency
preparedness and response capabilities related to biotenorism and disease
outbreaks. OLL will develop public health performance standards (with
deadlines)by which counties will conduct a self-assessment and a public health
infrastructure improvement plan that includes recommended corrective actions,
goals, and timelines for local public health to begin developing capacity in
identified areas of deficiency.
a. Contractor shall provide any additional information, as requested, to OLL
to facilitate completion of the public health performance standards by
December 1, 2003.
b. Contractor shall complete a self-assessment using the public health
performance standards developed by OLL. The self-assessment must be
completed and submitted to OLL by January 16, 2004.
c. Based on findings from the Public Health Infrastructure Improvement Plan
developed by OLL, Contractor shall update or enhance all local county

plans, as appropriate, and submit to their Regional Planner by April 30,
2004.
4. Contractor shall participate in the 2003 Centers for Disease Control and
Prevention (CDC)co-sponsored Department of Homeland Security (DHS)
Assessment. All assessments will be completed before December 31, 2003. All
local/county plans shall be updated, as appropriate, based on findings and
suggested corrective measures and submitted to their Regional Planner by April
30, 2004.
5. The Colorado Department of Human Services and local mental health agencies
will conduct an assessment regarding state and local capacity for mental health
preparedness and response as it relates to bioterrorism, other infectious disease
outbreaks, and other public health threats and emergencies.
a. Contractor shall document a meeting/discussion with each community
mental health agency in the Contractor's county and provide all
information, as requested, to the Colorado Department of Human Services
regarding integration of local capacity for mental health preparedness and
response into public health plans by November 14, 2003.
b. Based on findings from the Department of Human Services' Mental
Health Preparedness and Response Assessment, Contractor shall update or
enhance all local/county plans, as appropriate, and submit to their
Regional Planner by April 30, 2004.
c. Contractor shall include, and document inclusion of, community mental
health agencies in all public health emergency preparedness training or
exercises.
6. Contractor shall provide all information, as requested, to the State (contact person
and agency to be identified) regarding state and local public health jurisdictions
that provide for credentialing, licensure, and delegation of authority for executing
emergency public health measures, as well as special provisions (in coordination
with adjacent states) for the potential liability of healthcare personnel aiding in an
emergency situation,by January 30, 2004. Workers' compensation issues and the
health issues of workers and their families who may be involved in emergency
response will be addressed.
7. Contractor shall provide all information, as requested, to the Attorney General's
Office and the Colorado Department of Labor and Employment (CDLE)by
December 5, 2003, regarding statutes, regulations, and ordinances within local
public health jurisdictions that include special provisions for liability protection
and compensation for adverse events related to vaccination of healthcare
personnel who participate in the National Smallpox Vaccination Program.
8. Contractor shall participate in a minimum of one public health leadership and
management development training of their choice as well as training in advanced

concepts of incident command and bioterrorism communication. Documentation
of all courses must be provided to the Bioterrorism Contracts Monitor,Deanna
Butler,including name of course, list of participants, and course content, by
August 30, 2004. Participation in any courses not listed on the Focus Area A
Public Health Leadership Training website must be approved in writing by the
Focus Area A Manager or Focus Area A Coordinator prior to enrollment.
Approval must be documented and submitted with the quarterly progress reports.
9. Contractor shall participate in the statewide bioterrorism conference (tentatively
scheduled for June or July 2004). If the Contractor is not able to attend, the
Contractor shall recruit at least two community officials(i.e. emergency manager,
fire department command staff, mental health representative, or EMS
management hospital safety officers)to attend in their place.
10. Contractor shall update and enhance their local/county mass vaccination clinic
plans to include:
• Performing healthcare facility-based triage;
• Providing short-term acute psychosocial interventions as well as longer-
term services to large populations;
• Statewide emergency mutual aid and assistance agreements (MAAA);
• Hospitals and pre-hospital facilities; and
• Distribution to ambulance services and hospitals as well as their family
members to minimize the impact of the lost work time.
All mass vaccination clinic plans must be submitted to OLL by December 1,
2003.
11. In conjunction with Focus Area E, Contractor shall provide updated contact
information for the long-term circular web-based communication system to
ensure 24/7 messaging capability by March 31, 2004. Contractor shall verify and
update own information, monthly, via the Internet or telephone. Ongoing,
monthly, until August 30, 2004.
12. In conjunction with activity#3 of the Strategic National Stockpile (SNS)
workplan, Contractor shall document their participation in all local,regional, and
state bioterrorism preparedness exercises, as requested or required. Additionally,
Contractor shall document their participation in the development and execution of
all bioterrorism/emergency preparedness exercises held in their county or
counties.
• Individual counties will dictate timelines for local exercises.
• All regional exercises must be completed by May 31, 2004. Local health
departments will exercise a portion of their mass vaccination clinic plans
in conjunction with their regional exercise.

• Statewide exercise will occur sometime during June or July 2004.Testing
of the state and regional SNS plans will be part in this exercise and will
include the Training,Education, and Demonstration (TED).
13. CDPHE will hire a contractor to conduct a vulnerability assessment in terms of
human health outcomes related to a variety of biological, chemical, and mass
casualty terrorist scenarios as well as predictions of human health effects resulting
from releases of chemical or etiologic material.
a. Contractor shall provide information, as requested, to the vulnerability
assessment contractor by November 28, 2003.
b. Contractor shall update and enhance all local/county plans based on
findings and suggested corrective measures of the vulnerability assessment
by April 30, 2004.
14. In conjunction with the Health Resources and Services Administration (HRSA)
Cooperative Agreement for Hospital Preparedness, Contractor shall provide all
information, as requested, to the Planner and participate, as requested, by the
Planner to ensure the successful collaboration with hospitals, the medical
community, acute care, pre-hospital entities, and others to plan coordinated
delivery of critical health and mental health services and effective medical
management of emergencies by August 30, 2004.
15. Contractor shall ensure at least one public health leader within their agency shall
participate in National Incident Management System (NIMS)training by August
30, 2004. Date of training is contingent upon announcement by the DHS. In the
event that the DHS does not hold this training during the 2003-2004 grant period,
Contractor shall request approval from the Focus Area A Manager or Focus Area
A Coordinator for monies to be reallocated and used for additional
leadership/management training or advanced concepts of incident command
training.
16. Contractor shall assist their Regional Planner, as requested, to identify
environmental health specialists to conduct environmental sampling following a
terrorist attack, as well as identify mental health teams to conduct follow up
health assessments of victims of terrorist attacks by November 1, 2003.
17. In collaboration with the HRSA Cooperative Agreement for Hospital
Preparedness, Contractor shall provide information, as requested, to the
assessment contractor regarding current capacity to respond to injuries (including
psychological) resulting from terrorist events, including the capacity to develop
and standardize instruments used in conducting needs assessments of the
healthcare system, capacity to provide optimal trauma care, and the capacity to
conduct injury surveillance in an acute event, and to survey ongoing victim needs
by May 1, 2004.

18. Contractor shall provide information, as requested, to the Colorado Attorney
General's Office and the Colorado Department of Regulatory Affairs regarding
worker health and safety data related to bioterrorism by December 1, 2003. The
primary focus will remain on protection of emergency response workers,
remediation workers, workers responsible for restoring essential public services,
and other exposed occupational groups such as postal workers and healthcare
workers.
19. Contractor shall collaborate and provide information, as requested, to the
Mobilization for Action Through Planning and Partnerships (MAPP)contractor or
OLL(contact person to be determined) to conduct comprehensive system and
community health improvement planning. Contractor shall update or enhance all
local or county plans, as appropriate. This project will be implemented following
completion of activities and as determined by CDPHE. Timelines will be
established once it is determined that this activity will be completed.
20. Contractor shall provide to the State four(4) narrative progress reports and one
final report, which will summarize all accomplishments, and are due within five
(5)business days after the end of the period. December,March,June and August
billings will not be reimbursed until such time as delinquent progress reports have

Contractor shall continue to house a Regional Planner to serve the Northeast Region,
including: Kit Carson,Larimer,Lincoln,Logan,Morgan,Phillips,Sedgwick,
Washington,Weld and Yuma counties. Contractor is responsible for ensuring the
Planner successfully performs the following duties to the benefit of all counties of the
region:

a. In conjunction with activity "a" of the SNS workplan,Planner shall
collaborate with all other Regional Planners, Epidemiologist(s),Trainer(s) and
Focus Areas funded under this Cooperative Agreement within and between
regions, when appropriate and upon request in all bioterrorism preparedness
activities and deliverables. Ongoing until August 30, 2004.
b. In conjunction with activity "b" of the SNS workplan, Planner shall document
and provide technical assistance to all counties within their region, as
appropriate and as requested to ensure the timely completion of all
bioterrorism preparedness deliverables and activities. However,Planner is not
responsible for actually preparing deliverables or conducting activities that are
the responsibility of the Contractor. Ongoing until August 30, 2004.
c. Planner shall document participation in quarterly bioterrorism preparedness
meetings with other focus areas, as requested. In the event that the Planner
cannot attend the meetings, the Planner shall send regional written input on all
agenda items no less than one week prior to the meeting as well as send an
appropriate replacement to the meeting. The State shall be responsible for
scheduling all meetings, developing the agenda, and funding the venue and
materials, unless otherwise indicated. The Planner may be responsible for
assisting with the scheduling of and contracting for meetings in his/her region,
as requested. Ongoing, quarterly until August 30, 2004.
d. Planner shall participate in monthly conference calls with the Focus Area A
Manager and Focus Area A Coordinator, and provide an update on regional
progress and requests for clarification/assistance, unless other wise indicated.
Ongoing until August 30, 2004.
e. Planner shall document four(4) quarterly meetings/discussions with each
county health agency/nursing service in their region and provide meeting
documentation, including all participant contact information, meeting minutes,
and any follow-up activities to contracts management by August 30, 2004.
f. Planner shall document no less than tow (2)meeting/discussions with all
public health agencies, environmental health partners, mental health agencies,
emergency management agencies,RETAC, hospitals, EMS, and fire
departments, in each county in the region to ensure a collaborative effort in all
bioterrorism preparedness planning, training, and exercises as well as ensure
that high-level policy makers and elected officials at the state and local level
are updated at least quarterly regarding preparedness activities.
Documentation must include a list of participants, meeting minutes, and any
"follow-on" action items. Ongoing until August 30, 2004.
g. Planner shall assist the OLL Colorado Public Health Capacity Building
Steering Committee, as requested, with Phase II of the 2003 Public Health
Capacity Assessment. This includes the development of public health

performance standards (with deadlines) by which counties will conduct a self-
assessment and a public health infrastructure improvement plan that includes
recommended corrective actions, goals, and timelines for local public health
to begin developing capacity in identified areas of deficiency by December 1,
2003.
• Planner shall also assist counties within their region, as
requested or required, in completing the self-assessment that is
due to OLL by January 16, 2004.
• Based on findings from Phase I (Capacity Assessment) and
Phase II (Performance Standards and Public Health
Infrastructure Improvement Plan) developed by OLL, Planner
shall assist counties within their region, as requested or
required,in updating or enhancing all local or county plans, as
appropriate, as well as all regional plans (public health
emergency response and SNS)by April 30, 2004.
h. Planner shall assist each county in their region in the completion of the 2003
CDC/DHS co-sponsored Homeland Security Assessment. All assessments
will be completed by December 31, 2003. OLL will tabulate the results of this
assessment and analyze them against the 2000 DOJ Survey and 2002 Capacity
Assessment results. All local and regional plans will be updated, as
appropriate, based on findings and suggested corrective measures by April 30,
2004.
i. Planner shall assist each county in their region in providing information, as
requested, to the State (agency or contractor to be identified)regarding local
public health jurisdictions that provide for credentialing, licensure, and
delegation of authority for executing emergency public health measures, as
well as special provisions for the liability of healthcare personnel in
coordination with adjacent states by January 30, 2004. Workers'
compensation issues and the health issues of workers and their families who

j. Planner shall assist each county in their region in providing information, as

their participation in all local (within their region), regional, and state
bioterrorism preparedness exercises, as requested or required. Additionally,
Planner shall document their participation in the development and execution
of all bioterrorism/emergency preparedness exercises held within their
counties.
• Individual counties will dictate timelines for local exercises.
• All regional exercises will be completed by May 31, 2004. Regional
tabletops will be developed by the statewide exercise contractor and

launched by regional planners including participation from(but not
limited to) local emergency managers, local/county law enforcement,
local fire departments, and hospital planners prior to the statewide
functional exercise. In addition, local health departments must exercise
a portion of their mass vaccination clinic plans in conjunction with
their regional exercise.
• The statewide exercise will occur sometime during June or July 2004.
Testing of the state and regional SNS plans will be part of this exercise
and will include the TED package.
p. Planner shall provide information, as requested,by November 28, 2003, to the
vulnerability assessment contractor hired by CDPHE, in terms of human
health outcomes related to a variety of biological, chemical, and mass casualty
terrorist scenarios as well as predictions of human health effects resulting
from releases of chemical or etiologic material.
q. Planner shall update refine, and enhance their regional public health response
plans based on the '02— '03 vulnerability assessment. All regional public
health response plans must be submitted by April 30, 2003.
r. In conjunction with the HRSA Cooperative Agreement for Hospital
Preparedness, Planner shall collaborate with hospitals, the medical
community, acute care, pre-hospital entities, and others to plan coordinated
delivery of critical health and mental health services and effective medical
management of emergencies. Planner shall follow the model for surge
capacity planning (to be provided by CDPHE) and update all regional plans,
as applicable, with surge capacity planning, mass casualty planning, and pre-
hospital care planning by August 30, 2004.
s. In conjunction with activity "f' of the SNS workplan, Planner shall participate
in NIIMS training by August 30, 2004. Date of training is contingent upon
announcement by the DHS. In the event that the DHS does not hold this
training during the 2003-2004 grant period,Planner shall ensure their agency
requests approval from the Focus Area A Manager or Focus Area A
Coordinator for monies to be reallocated and used for additional
leadership/management training or advanced concepts in incident command
training.
t. Planner shall assist the State, as requested, in identifying environmental health
specialists to conduct environmental sampling following terrorist attacks by
November 1, 2003.
u. In conjunction with Focus Area B, and HRSA, Planner, as requested, will

once it is determined that this activity will be completed.
y. Planner shall continue to develop and augment their regional preparedness
program by participating, as requested, in all other bioterrorism preparedness

1.To rapidly detect a terrorist event through a highly functioning, mandatory reportable
disease surveillance system, as evidenced by ongoing timely and complete reporting by
providers and laboratories in a jurisdiction, especially of illnesses and conditions possibly
resulting from bioterrorism, other infectious disease outbreaks, and other public health
threats and emergencies.
2. To rapidly and effectively investigate and respond to a potential terrorist event as
evidenced by a comprehensive and exercised epidemiologic response plan that addresses
surge capacity, delivery of mass prophylaxis and immunizations, and pre-event
development of specific epidemiologic investigation and response needs.
3. To rapidly and effectively investigate and respond to a potential terrorist event, as
evidenced by ongoing effective state and local response to naturally occurring individual
cases of urgent public health importance, outbreaks or disease, and emergency public
health interventions such as emergency chemoprophylaxis or immunization activities.
In accordance with Cooperative Agreement U90/CCU 816827-04,the
Contractor shall perform the following duties:
Contractor is responsible for housing, enabling, and supporting the Regional Bioterrorism
Epidemiologist in serving the Northeast Region, including; Kit Carson,Lorimer,
Lincoln, Logan, Morgan, Phillips, Sedgwick,Washington,Weld, and Yuma counties.
Contractor is responsible for ensuring the Epidemiologist successfully performs the
following duties;
1. Epidemiologist shall serve as work leader for the Communications 1 work group.
Duties include but are not limited to the following:
a. Assign a"recorder" for the work group, who will record and keep
meeting minutes from each work group meeting.
b. Submit meeting minutes electronically to the Focus Area B
Manager within one week of each meeting and distribute minutes
to the work group.
c. Ensure role call is taken for each work group meeting and keep
documentation available for request.
d. Develop a twelve-month work plan for the required activities of
the assigned work group. Submit this work plan to the Focus Area
B Manager by September 29, 2003.

e. Develop and maintain/monitor timelines for all the required
activities of the assigned work group. Ongoing through August 31,
2004.
f. Assign tasks to work group members. Ongoing through August 31,
2004.
g. Complete a narrative evaluation quarterly, detailing the progress of
the work group overall and in general. This narrative is to be
submitted with the quarterly progress reports.
2. As work leader of the Communications 1 work group, Epidemiologist is
responsible for collaborating with Focus Area B Manager and other CDPHE
designated appropriate staff and contractors to ensure the following deliverables
are completed:
a. Collaborate with Focus Area F to develop content and strategies for
developing materials relating to bioterrorism, other infectious disease
outbreaks, other public health threats and emergencies and other relevant
technical information for public use in a terrorism event. Ongoing through
August 31, 2004.
3. Epidemiologist shall be a fully active participant in the following workgroups
through August 31, 2004:
• Epidemiology Education and Health Care Worker Training
Development
• GIS
• Novel and Syndromic Surveillance
4. Epidemiologist shall participate in all the monthly teleconference calls of their
assigned workgroups, as well as the CDPHE monthly teleconference calls.
5. Epidemiologist shall participate in related workgroup list serve discussions by
daily review of messages received from the BT-listserv.
6. Epidemiologist shall participate in the CDC Epidemic Information Exchange
(EPI-X). Epidemiologist shall receive regular e-mail from EPI-X that will keep
them informed on disease outbreaks in Colorado and throughout the country.
Epidemiologist shall check these messages daily during scheduled work periods.
7. Epidemiologist shall attend each Focus Area B quarterly meeting and any agreed
upon major statewide meetings.
8. Epidemiologist shall complete each of the tasks assigned to them in the
workgroup by the workgroup leader on time and effectively.

9. Epidemiologist shall maintain adequate Personal Protective Equipment (PPE) and
prophylactic medication supplies (consistent with current Board of Medicine
requirements) for public health staff in their counties/regions and ensure training
in the use of PPE for new employees, who might be required to respond to a
bioterror-related or other epidemiolgic emergency.
10. Epidemiologist shall continue contact with physician and other health care worker
groups and assist in local implementation of training developed by the Health
Care Worker Education, Pandemic Flu and Smallpox Planning work groups. In
addition,the epidemiologist shall be prepared to support the efforts of the
Communication workgroups in distributing materials or information as needed.
11. Epidemiologist shall be prepared to implement reporting improvement strategies
designed by (and in collaboration with)the CEDRS Surveillance Evaluation and
Reporting Improvement workgroup within their own counties/regions.
12. Epidemiologist shall participate with local partners in developing local and
regional exercises.
13. Epidemiologist shall participate and assist in any large scale or regional outbreak
and disease investigation as needed.
14. After a regional or cross-regional outbreak or emergency the Epidemiologists
involved shall work together on an"after-action"report identifying what worked
and what did not work, including timely reporting, timely response and timely
intervention. Written reports shall be sent to Focus Area B Manager within six (6)
weeks of completing the investigation/response.
15. Epidemiologists shall attend all required Epidemiology trainings and are
encouraged to attend others as scheduling allows, and as determined by CDPHE.
16. Epidemiologist shall work with the understanding that all activities listed above
could be diverted temporarily to accommodate emergent requirements from the
Governor's office or from the Centers for Disease Control (CDC), as defined by
the Focus Area B Manager.
17. The Epidemiologist shall provide a method of rapid and accurate disease
reporting utilizing the Colorado Electronic Disease Reporting system (CEDRS) to
facilitate information flow between all necessary public health agencies and
hospitals. This will require the Epidemiologist to ensure all necessary public
health agencies and hospitals have access to CEDRS. Ongoing through August
31, 2004.
18. The Epidemiologist shall provide training to health care providers, and the
laboratory personnel community on general principles of surveillance and
CEDRS use. Ongoing through August 31, 2004.

19. The Epidemiologist shall maintain the structure which they previously developed
in the last grant year, at the local level to provide an around-the-clock capacity for
immediate response to reports of urgent cases, outbreaks, or other public health
emergencies, including any events that suggest intentional release of a biologic
agent, on a 24-hour by seven-day/week basis.
20. The Epidemiologist shall provide regular monitoring of surveillance data and
disseminate reports to key partners, included but not limited to; those providing
surveillance data such as health care providers, laboratories,hospital infection
control practitioners, other hospital personnel, coroners etc.
21. The Epidemiologist shall maintain the written protocol for notification of key
partners, in the event that aberrations in the surveillance data are noted.

22. The Epidemiologist shall participate with the CEDRS Surveillance Evaluation
and Reporting Improvement workgroup in reviews of the surveillance system
(CEDRS) for timeliness and completeness at the county level. (Due date to be
determined by the CEDRS Surveillance Evaluation and Reporting Improvement
workgroup.)
23. The Epidemiologist shall collaborate with the Epidemiology Education and
Health Care Worker Training Development workgroup to design and implement a
reporting program aimed to increase disease recognition and reporting with an
emphasis on bioterrorism agents for the health care providers, and laboratory
personnel community. Ongoing through August 31, 2004.
24. The Epidemiologist shall continue to provide training on bioterrorism recognition
and response, including the potential agents and their epidemiology, and including
smallpox,to the medical, clinical and laboratory personnel community. Ongoing
through August 31, 2004.
25. The Epidemiologist shall facilitate, in coordination with the Pandemic Flu
workgroup and the Smallpox workgroup,the development of agency specific
epidemic response plans, which include an appendix to address planning for
smallpox and pandemic influenza, and which also meet at the minimum,
"Regulations Pertaining to Preparations for a Bioterrorist Event, Pandemic
Influenza, or an Outbreak by a Novel and Highly Fatal Infectious Agent or
Biological Toxin", as found in the Board of Health Regulations. These plans will
be integrated with the work undertaken by the Focus Area"A" Planners. Ongoing
through August 31, 2004.
26.The Epidemiologist shall ensure the integration of the aforementioned epidemic
response plans with the agency's overall plan, local and regional plans, and the
local emergency response structure including hospitals. Ongoing through August
31, 2004.

27.The Epidemiologist shall ensure that each agency will participate in preparedness
drills and exercises that simulate activation of the epidemic response plans at least
annually, and in coordination with the Statewide exercise.
28.The Epidemiologist shall create a written evaluation of the epidemic
response/investigation procedures after completion of drills and exercises to be
held in conjunction with Focus Area A by July 1, 2004. This written evaluation
will include feedback and suggestions regarding the successes and failures of the
said drills and exercises, and shall be kept available for request by CDPHE.
29.The Epidemiologist shall attend local and state sponsored training activities.
30. Epidemiologist shall provide training to public health agencies and other
response partners, such as law enforcement, on epidemiology methods and
outbreak investigation techniques, as required.
31. The Epidemiologist shall assist environmental health partners as needed on food
and water system security.
32. The Epidemiologist shall include a broad range of community partners in
planning and response activities. These partners may include but are not limited
to; local emergency response teams, hospitals, and tribal leaders in Indian health
services if applicable to the area.
33. The Epidemiologist shall collaborate, when appropriate, and upon request, with
the Planner(s) in their region in preparedness activities.

In accordance with Cooperative Agreement U90/CCU 816827-04, the
Contractor shall perform the following duties:
1. Contractor shall upgrade laboratory to Laboratory Response Network (LRN)
Level B, and develop the capacity to test for B. anthracis,F. tularensis, Brucella
and Yersinia pestis in human and environmental samples. Ongoing through
August 31, 2004.
2. Contractor shall make laboratory staff and other personnel available for training
and planning.
3. Contractor shall perform testing according to the LRN protocols.
4. Contractor shall make available one staff member to serve as a representative to
the Colorado Laboratory Forum, which will meet at least once annually, and
attend audio conferences as needed.
5. Contractor shall participate in exercises and assessment studies and conduct
proficiency testing.
6. Contractor shall, when capable, provide testing as needed, for clinical laboratories
and other agencies within the Office of Local Liaison area in which the laboratory
resides and certain contiguous areas: the Northeast Region encompassing Weld,
Larimer,Morgan, Logan, Sedgewick, Phillips,Washington, Yuma, Lincoln
and Kit Carson counties.
7. Contractor shall refer isolates and specimens to a higher LRN laboratory as
needed.
8. Contractor shall report rapidly (within 24 hours) on testing activity to the Focus
Area C leader and to the Centers for Disease Control (CDC) on request from the
LRN during an event.

9. Contractor shall use grant funding to purchase equipment and supplies in
accordance with the attached budget and modify laboratory facilities and
procedures to ensure the security of cultures and laboratory.
10. Contractor shall upgrade computer facilities and communications to allow

pager, phone and fax, and, when capable of providing laboratory testing for

required and requested by the state HAN Coordinator. If the currently designated
HAN Coordinator is not the correct point of contact, please submit any changes to
the state HAN Coordinator by October 15, 2003, and as necessary throughout the
year.
2. Contractor shall ensure access for the contractor's own agency to a high-speed(at
least 384Kbps)link from the contractor's computer network to an Internet Service
Provider. The contractor will serve as a contact point for secure Internet
transactions with the state.
3. Contractor shall assess the capacity in their county(ies)for redundant
communication systems/devices, the capacity for existing systems to broadcast
and/or autodial to automatically distribute alerts and messages to those
systems/devices, and the capacity to link to the emergency communication
systems of local emergency response partners. If necessary, make improvements
before August 30, 2004. Examples of system/devices may include cell phones,
satellite phones, two-way radios, hand-radios, wireless devices with messaging
capabilities).
4. Contractor shall implement a method of receiving critical alerts that are in full
redundancy with e-mail. Methods may include faxes,cell phones, pagers, PDA,
etc. Contractor is expected to test the system quarterly.
5. Contractor shall work with the state HAN coordinator and local emergency
service providers to acquire high frequency radios and satellite voice/data systems
based on interoperability standards provided by the Centers for Disease Control
(CDC).

6. Contractor shall routinely (at least quarterly)test and assess the timeliness and
completeness of the redundant method of alerting.The test should ensure the
public health response partners can receive and send alerts.
7. Contractor shall maintain the up-to-date directory of participants in public health
emergency response.
8. Contractor shall manage the communication with hospitals,clinics, and other
participants of National Smallpox Vaccination Program (NSVP) to ensure the
smallpox vaccination team member directory,by various expertise, is maintained
and regular updates on implementation of program activities are provided. The
categories of expertise may include,but are not limited to: vaccinators, clinic
managers, screeners, vaccine storage and handling staff, and other technical,
clinical, and epidemiological experts. The contractor will provide the smallpox
member directory to the HAN coordinator upon request.

public and key partners during a terrorism event by establishing critical baseline
information about the current communication needs and barriers within individual
communities, and identifying effective channels of communication for reaching the
general public and special populations during public health threats and emergencies.
In accordance with Cooperative Agreement U90/CCU 816827-04, the Contractor shall
perform the following duties:
1. Contractor shall designate one staff member and one alternate for public information,
communications, or media relations, and submit the names to the Focus Area F
Manager by September 30, 2003. Contractor shall require at least one of the
designated staff to attend and fully participate in a two-day training to be scheduled
by Focus Area F(tentatively scheduled for December 15-16, 2003).
2. Contractor shall help coordinate and participate in one or more regional meetings
with their Regional Planners, Regional Trainers,Regional Epidemiologists, and other
same-region counties' public information, communications, and media relations staff,
to review and coordinate crisis and emergency risk communication plans, particularly
among adjacent counties/agencies, by February 29, 2004. Contractor shall document
activities and submit a brief summary of activities and dates to CDPHE by August 31,
2004.
3. Contractor shall help coordinate and participate in regional exercises to practice
implementation of crisis and emergency risk communication plans and identify
strengths and weaknesses that become evident during the exercise(s). Contractor shall
document activities related to regional exercises, and submit a brief analysis of the
strengths and weaknesses and a plan for remedying the weaknesses to the Focus Area
F Manager by May 31, 2004.
4. Contractor shall apply information from coordinating meeting(s) and exercise(s) to
finalize the Contractor's Crisis and Emergency Risk Communication plan. Crisis and
Emergency Risk Communication plan shall be submitted to the Focus Area F
Manager by electronic mail or U.S. mail by July 31, 2004.

5. Contractor shall submit the Crisis and Emergency Risk Communication Plan for
review and approval to its agency/county chief executive and/or Board of Health,
after review and approval by Focus Area F Manager. After approvals are obtained,
Contractor shall integrate the final Crisis and Emergency Risk Communication plan
into the agency's/county's all-hazards emergency response plan by August 31, 2004,
and submit copy of approval and Crisis and Emergency Risk Communication plan to
Focus Area F Manager by August 31, 2004.
6. Contractor shall participate in the crisis and emergency risk communication
component of the statewide exercise(date(s)to be determined) to practice
implementation and coordination. Contractor shall document activities related to
statewide exercise and keep documentation available upon request by CDPHE.
7. Contractor shall contact each hospital within its county(ies),to coordinate special
populations outreach education with hospital communications staff, based on the
2003 research on special populations and the community's needs, by November 30,
2003. Focus Area F Contractors, as well as identified individual community needs
will determine specific details regarding the outreach education. Contractor shall
document activities related to coordination with hospitals and special populations'
outreach and submit a brief summary of activities and dates to CDPHE by December
31, 2003.
8. Contractor shall develop or adapt electronic and print materials for special
populations as needed for Contractor's county(ies), using the results and
recommendations of the 2003 needs assessment, called"The Capacity of Public
Health Systems in Colorado" (Omni Research and Training, May 2003), and 2003
research on special populations, called"Colorado Demographics and Effective Risk
Communication" (Judith Cohen, PhD, Market Views, LLC, July 11, 2003), and using
prepared information from Focus Area F, as it becomes available (to be distributed by
the Focus Area F Manager), by March 31, 2004. Contractor shall submit any
electronic and print materials developed or adapted for local or regional needs, if
applicable, to Focus Area F Manager by April 30, 2004.
9. Contractor shall incorporate strategies for special populations' needs and
psychosocial consequences of Strategic National Stockpile (SNS) activation into local
and regional Crisis and Emergency Risk Communication plan(s), and into local and
regional SNS plan(s), if applicable, and by December 1, 2003. Contractor shall
document activities and submit a brief summary of strategies, activities, and dates to
CDPHE by December 31, 2003.
10. Contractor shall contact local emergency response communicators by September 30,
2003, document contacts for records, and begin the ongoing process of sharing
strategies and tools for risk communication planning throughout the grant year, until
August 31, 2004. Contractor shall provide strategies and tools for risk communication
planning to Focus Area F Manager by July 31, 2004.

11. Contractor shall participate in Health Alert Network (HAN) system training provided
by Focus Area E and its contractor, to learn how to update the Contractor's public
information/communications/media relations contact information, and shall make
updates accurately and promptly as needed throughout the grant year, until August

12. Using demographic data and recommendations of area human services agencies (i.e.,
public and private mental health and social welfare assistance agencies, and agencies
serving persons of non-majority ethnic/racial groups or national origins), Contractor
shall identify populations of non-English speaking citizens in the Contractor's
county(ies). Contractor shall determine by February 29, 2004 whether translation and
interpretation services will be needed for these populations in a public health
emergency. If translation and interpretation services are needed, Contractor shall
contact potential translator and interpreter services to locate credible services with
adequate surge capacity and develop contingency contracts and/or memoranda of
understanding with those credible services by July 31,2004. Contractor shall submit

SUBJECT: Task Order for Public Health Preparedness
and Response for Bioterrorism Program
Enclosed for Board review and approval is the Bioterrorism Task Order for focus areas A, B, C,
E, F, and SNS. If approved, this task order will become part of the Master Contract between the
Colorado Department of Public Health and Environment (CDPHE) and Weld County
Department of Public Health and Environment (WCDPHE).
CDPHE is awarding Weld County renewal funding in the amount of$328,035 as pass through
from the United States Department of Health and Human Services, Centers for Disease Control
and Prevention (CDC)to provide public health preparedness and response for bioterrorism
activities. Funding from this task order is for the time period September 1, 2003 through August
31, 2004. The amounts for each focus area are listed below:
SNS: Strategic National Stockpile - $17,657
A: Preparedness Planning and Readiness Assessment - $73,264
B: Surveillance and Epidemiology Capacity- $104,232
C: Laboratory Capacity—Biologic Agents - $90,000
E: Health Alert Network—Communications and Information Technology- $32,083
F: Risk Communication and Public Health Information Dissemination - $10,799
While this will be renewal funding for focus areas A, B, C and E, the amounts for focus area F
and SNS are new for Weld County this year. The objective of focus area F is to provide needed
health/risk information to the public and key partners during a terrorism event by establishing
critical baseline information about the current communication needs and barriers within
individual communities. The SNS objective is to ensure state, regional, and local preparedness
and planning (including management, security, and training) should the Strategic National
Stockpile be implemented.
