RE: APPROVE CONTRACT FOR LONG TERM CARE OMBUDSMAN ADVOCATE/
FRIENDLY VISITOR PROGRAM AND AUTHORIZE CHAIR TO SIGN - VICTIM'S
ASSISTANCE AND LAW ENFORCEMENT BOARD
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Contract for the Long Term Care
Ombudsman Advocate/Friendly Visitor Program between the County of Weld, State of Colorado,
by and through the Board of County Commissioners of Weld County, on behalf of the
Department of Human Services, Area Agency on Aging, and the Victim's Assistance and Law
Enforcement Board, commencing January 1, 2004, and ending June 30, 2005, with further terms
and conditions being as stated in said contract, and
WHEREAS, after review, the Board deems it advisable to approve said contract, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Contract for the Long Term Care Ombudsman
Advocate/Friendly Visitor Program between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Department of
Human Services, Area Agency on Aging, and the Victim's Assistance and Law Enforcement
Board be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said contract.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 24th day of December, A.D., 2003.

CONTRACT
CONTRACT between Victim Assistance and Law Enforcement Board ("Donor") and
Weld County Area Agency on Aging ("Recipient"),P.O. Box 1805, Greeley, CO 80632, for
the funding of Weld County Long Term Care Ombudsman Advocate/Friendly Visitor
("Grant", "Grant Application" or "Project"), as described in your Grant Application for the
period commencing on January 1, 2004.
The parties (Donor and Recipient) in consideration of the mutual promises contained
herein agree as follows:
1. The Recipient hereby contracts to perform the services outlined in the Grant
Application, as submitted to the VALE Board and incorporated herein by reference.
2. The parties acknowledge that the Grant contemplated herein is made pursuant to
Colorado law and the Recipient specifically agrees to use the Grant for the purposes specified
in C.R.S., 1973, S24-4.2-105, as amended. In addition, the Recipient agrees to submit to the
Donor quarterly reports, including a "Grant Report Form" and a "Narrative Report."
Furthermore, EIGHT (8) COPIES of quarterly reports are due no later than the end of the
month immediately following the end of each quarter throughout the grant term (i.e., April 30,
July 31, October 31 and January 31). Failure to submit quarterly reports may result in
withholding of funds and/or grant termination.
3. The Grant commences on January 1, 2004, and terminates on June 30, 2005.
4. The Donor hereby grants to the Recipient the sum of$12,750.00 of which an initial
sum of$2,125.00 will be paid to the Recipient on or about January 15, 2004. The projected
disbursement of the balance will be paid as follows:
$2,125.00 on or about April 15, 2004,
$2,125.00 on or about July 15, 2004,
$2,125.00 on or about October 15, 2004,
$2,125.00 on or about January 15, 2005, and
$2,125.00 on or about April 15, 2005.
5. The Grant may be terminated by the Donor if:
(a) the Recipient does not submit the required quarterly reports, or if the reports or other
information does not show adequate achievement of the Grant purpose(s) as stated on the
Grant Application and pursuant to C.R.S., 1973 S24-4.1-105, as amended; or,

(b) the Donor has insufficient funds to continue the Grant.
Before termination for inadequate achievement, the Recipient shall have an opportunity
to meet with the Donor. The Grant may be terminated by the Recipient at any time.
6. At the end of the Contract term, or upon Contract termination, all unspent funds
shall be returned to the Donor.
7. The Contract termination date maybe extended by written mutual agreement of the
Donor and Recipient. The services to be provided may also be modified by written mutual
agreement of both parties (Donor and Recipient).
The signatories certify that we have read the Contract and are fully cognizant of our duties and
responsibilities for this Project.

SUBJECT: Weld County Area Agency on Aging's Long
Term Care Ombudsman Advocate/Friendly
Visitor Program VALE grant approval
Enclosed for Board signature is the grant contract for VALE (Victims Assistance and Law
Enforcement Board) assistance. The Ombudsman Program has received this grant for the last 6
years. The amount being awarded in 2004 is $12,750.00. The funding period will be from January
1St, 2004 through June 30th, 2005. This grant will allow the Ombudsman program to provide elder
abuse prevention programs to long term care staff and develop and maintain a volunteer
Ombudsman program.

Dear Ms. Jewell:
As you were previously notified, on November 14, 2003, the Victim Assistance and Law
Enforcement (VALE) Board for the Nineteenth Judicial District awarded your project, Weld
County Long Term Care Ombudsman Advocate/Friendly Visitor, a grant totaling
$12,750.00. Once a contract is completed, the funding for your project will begin on January 1,
2004, and this funding will be disbursed in six $2,125.00 quarterly payments.
Now that the grant awards have been finalized, I have enclosed two copies of a contract
concerning the funding being offered to your project. Please indicate acceptance of the VALE
grant offered by returning one copy of the contract, with the appropriate signatures, on or before
December 31, 2003. In addition, you will need to provide the Board with eight (8) copies of
quarterly reports concerning your project's performance, including copies of the enclosed
financial report forms, and these reports will be due on the last day of the month following the
end of each quarter.
