HomeMy WebLinkAbout20031243.tiff RESOLUTION RE: APPROVE COLORADO WOMEN'S CANCER CONTROL INITIATIVE GRANT RENEWAL APPLICATION AND AUTHORIZE CHAIR TO SIGN WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to Colorado statute and the Weld County Home Rule Charter, is vested with the authority of administering the affairs of Weld County, Colorado, and WHEREAS, the Board has been presented with a Grant Renewal Application for the Colorado Women's Cancer Control Initiative between the County of Weld,State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Department of Public Health and Environment, and the Colorado Department of Public Health and Environment, commencing July 1, 2003, and ending June 30, 2004, with further terms and conditions being as stated in said application, and WHEREAS, after review, the Board deems it advisable to approve said application, a copy of which is attached hereto and incorporated herein by reference. NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld County, Colorado, that the Grant Renewal Application for the Colorado Women's Cancer Control Initiative between the County of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County, on behalf of the Weld County Department of Public Health and Environment, and the Colorado Department of Public Health and Environment be, and hereby is, approved. If never have had a Pap and/or mammogram ask why and women who have not been screened reassure client that a Pap and/or mammogram is/are only a bit in more than 5 years OR have never discomforting but that the screenings can save her life. If client doesn't been screened know what a mammogram is,explain that a mammogram is an X-ray of each breast that can detect breast cancer at its earliest stage. If client doesn't know what a Pap testis,explain that it's a small procedure in which a few cervical cells are brushed off from the uterus opening. A lab will check these cells for cancer or other problems. If cancer is found early,it is easier to cure. 26. Eligible women referred for screenings will receive a letter thanking N/A Ongoing Contact Log Data them for calling and using the program. They will be reminded of the Screening Referral List/Screening Data clinic they were referred and a certificate for a free a Pap test and/or mammogram with the provider's phone number will be included in the letter, 2C. If a women has reservations about her first mammogram,a hand- N/A Ongoing Contact Log Data out on facts and myths of mammograms and stats regarding the Screening Referral List/Screening Data Number of mammograms that actually end up showing breast cancer will be sent. Hopefully,this will encourage women to keep their appointment for a mammogram. 2D. If a daughter or sister calls on behalf of the woman, because she N/A Ongoing Contact Log Data doesn't speak English or isn't comfortable speaking on the phone,she Screening Referral List/Screening Data will be asked if she plans to go with the woman to her appointment. If she doesn't,other options will be discussed. 2E. If funded, the time period for this renewal between the Colorado Department of Public Health and Environment and Weld County Department of Public Health and Environment will be July 1, 2003 — June 30, 2004. The total amount of this request is $37,284.00 and includes funding for a .5 FTE bilingual Health Education Specialist. Weld County Department of Public Health and Environment will use this funding to provide education and referral for breast and cervical cancer screening to low-income, underinsured and uninsured women throughout Weld County.
