RE: APPROVE COLORADO WOMEN'S CANCER CONTROL INITIATIVE GRANT RENEWAL
APPLICATION AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Grant Renewal Application for the
Colorado Women's Cancer Control Initiative between the County of Weld,State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Weld County
Department of Public Health and Environment, and the Colorado Department of Public Health and
Environment, commencing July 1, 2003, and ending June 30, 2004, with further terms and
conditions being as stated in said application, and
WHEREAS, after review, the Board deems it advisable to approve said application, a copy
of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that the Grant Renewal Application for the Colorado Women's Cancer Control
Initiative between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said application.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 19th day of May, A.D., 2003.

Goal Performance Indicators
To increase awareness among priority populations of the need for and availability of breast and A minimum of 4000 women will be reached through all coordinator outreach efforts
cervical cancer screening services,address barriers that prohibit screening, and motivate women
to seek these services by developing and implementing an appropriate mix of broad-based A minimum of 2000 women will be reached through one-on-one efforts
awareness-raising,community-based education and one-on-one outreach strategies,with the
target goal of screening 10%of the projected eligible population A minimum of 200(10%) of program-eligible women reached through one-on-one outreach will
be referred to screening
A minimum of 100(50%)of program-eligible women referred to screening will have a complete
breast and/or cervical cancer screening exam

Provide program eligibility information, brochures,and marketing
materials-pink ribbon cards, hear key rings,mirrors,etc. If woman is
eligible,she will be signed on to the program and given a certificate for
a free mammogram and/or a free Pap test with the provider site contact
information.
1C. Collaborate with 9 Health Fair Screenings in April and other health N/A Ongoing Contact Log Data
fairs as they become available. Screening Referral LIst/Screening Data
1D. Recruit eligible women by doing in-reach at local provider sites N/A Ongoing Contact Log Data
{e.g. Planned Parenthood, Sunrise Community Health Center, Health Screening Referral List/Screening Data
Dept., Plan de Salud del Valle in Ft Lupton and Frederick). Screening
questions: Do you know any one who needs a free mammogram or
Pap test? If women have questions,ask:How long has it been since
your last Pap and/or mammogram?Age, Household size and Income,
insurance?
1E. Provide education and support to 6-10 women in Platicas-small N/A Monthly Contact Log Data
group discussion at a hostess home. In addition,women will receive Screening Referral List/Screening Data
information on program eligibility criteria and community resources. _
2. To recruit and refer a minimum of 40 2A.Achieve the required one-on-one counseling contact to rarely or N/A Ongoing Contact Log Data
rarely or never-screened women for never-screened women through the hotline number. Telephone Hotline Call Summary
CWCCI screening services through one-screening will be done by asking the following questions:name, age, Screening Referral List
on-one activities,including counseling, how long since last Pap test and/or mammogram,household size,
education and support,by June 2004 income, insurance, and how did she find out about the CWCCI
program. If never have had a Pap and/or mammogram ask why and
women who have not been screened reassure client that a Pap and/or mammogram is/are only a bit
in more than 5 years OR have never discomforting but that the screenings can save her life. If client doesn't
been screened know what a mammogram is,explain that a mammogram is an X-ray of
each breast that can detect breast cancer at its earliest stage. If client
doesn't know what a Pap testis,explain that it's a small procedure in
which a few cervical cells are brushed off from the uterus opening. A
lab will check these cells for cancer or other problems. If cancer is
found early,it is easier to cure.
26. Eligible women referred for screenings will receive a letter thanking N/A Ongoing Contact Log Data
them for calling and using the program. They will be reminded of the Screening Referral List/Screening Data
clinic they were referred and a certificate for a free a Pap test and/or
mammogram with the provider's phone number will be included in the
letter,
2C. If a women has reservations about her first mammogram,a hand- N/A Ongoing Contact Log Data
out on facts and myths of mammograms and stats regarding the Screening Referral List/Screening Data
Number of mammograms that actually end up showing breast cancer
will be sent. Hopefully,this will encourage women to keep their
appointment for a mammogram.
2D. If a daughter or sister calls on behalf of the woman, because she N/A Ongoing Contact Log Data
doesn't speak English or isn't comfortable speaking on the phone,she Screening Referral List/Screening Data
will be asked if she plans to go with the woman to her appointment. If
she doesn't,other options will be discussed.
2E. Provide Platicas(small group discussions)on breast&cervical N/A Monthly Contact Log Data
health as requested in hostess homes for 6.10 women and at places Screening Referral List/Screening Data
such as the Villa Correctional Facility, Catholic Charities Plaza del
Milagro, UNC Women's Resource Center, Migrant Sites,churches, and
PTAs throughout Weld County.
2F. Recruit high risk women at Social Services, Health Dept., Plan de Ongoing Contact Log Data
Salud at Ft.Lupton and Frederick,and Planned Parenthood for Platicas Screening Referral List/Screening Data
(small group discussions)hostesses to provide education on breast and
cervical health and information on prograrh`eligibility and resources
available. _
2G.Distribute program brochures,flyers,posters,and other materials N/A Ongoing Contact Log Data
to senior centers,libraries,churches, human service agencies,beauty Screening Referral List/Screening Data
shops and to small/large Latino businesses throughout Weld County
that employ primarily women without the provision of health insurance.
3. To conduct a minimum of 210 paid 3A. Provide printed media to local newspapers,newsletters,church N/A October,January, Contact Log Data/Copies of Media
and/or inkind media events to reach bulletins and broadcasted media one-two weeks prior to scheduled May Hotline Call Summary

3E. Secure a minimum of 3 quarterly earned media spots in the County N/A Quarterly Contact Log Data/Copies of Media
Line, Grandparents Raising Grandchildren in Weld County newsletter, Hotline Call Summary
United Way newsletter,the Grove apartments newsletter and Aims Screening Referral List
Alive newsletter for a total of 15. Earned media spots will include
information on upcoming promotional campaigns such as National
Breast Cancer Awareness Month,Cervical Cancer Awareness Month,
Mother's Day Campaign,health fair events and program qualification
guidelines.
4.To collaborate*on a minimum of 3 4A. Promotion of Breast Cancer Awareness Month by attending the N/A September/October Contact Log Data
promotional campaigns to promote following: (I) Interfaith Council meetings to reach a minimum of 25 Hotline Call Summary
screening services to 600 program congregations via verbal announcements,church bulletin notices or Screening Referral List
eligible women by June 2004 distribution of program brochures and materials. (II)Latino Women's
Coalition to reach a minimum of 20 human service organizations such
as Weld Food Bank, Guadalupe Shelter, Dist.6 Schools, United Way,
*e.g. Local ACS,Komen Chapter, Bright Beginnings, CASA,Partners,Youth Net,Aims Developmental
Breast Cancer Task Force,Health Education,Headstart Prog., Police Dept., UNC Hispanic Studies,
Department,Community Organization, Sunrise Comm. Health Ctr.,Planned Parenthood,and Boys&Girls'
Business,CWCCI or Other Health Care Club via verbal announcements and distribution of program
Provider etc. brochures/flyers and promotional materials. (III)Other community

SUBJECT: Colorado Women's Cancer Control Initiative Grant Renewal
Enclosed for Board review and approval is the work plan and budget for a proposal for
funding for the Colorado Women's Cancer Control Initiative. If funded, the time period
for this renewal between the Colorado Department of Public Health and Environment
and Weld County Department of Public Health and Environment will be July 1, 2003 —
June 30, 2004. The total amount of this request is $37,284.00 and includes funding for
a .5 FTE bilingual Health Education Specialist.
Weld County Department of Public Health and Environment will use this funding to
provide education and referral for breast and cervical cancer screening to low-income,
underinsured and uninsured women throughout Weld County.
