To Whom It May Concern:
The above patient suffers from coronary artery disease
for which she has had coronary artery bypass surgery.
In addition she suffers from hypertensive heart disease
and severe allergies.
The patient must be seen weekly for allergy vaccine
and is seen monthly for monitoring of her cardiovascular
disease.
Because of gradual deterioration in her health and
because of the elderly problem with her husband being
78, transport and looking after them are somewhat of
a hardship to the family.
Any help that you could give to bringing their son
home to reside on their property to help them with
their medical care and other problems that they are
having in their old age, would be greatly appreciated.
Sincerely yours,
Miles D. Lee, D.O. i l , 1 1 r
