The review was conducted in accordance with Article 2.1.5 of the Colorado State of Colorado Design Criteria(Cr Potable Water �Drinking Water Regulations and the Systems. The components of the review included: • Existing 51-foot deep well(Well Permit Number 041698-F) • Hypoehlorinati.on disinfection system • 600 gallons of potable water storage,consisting of two,300-gallon,tanks in.seties • Water filtration system,consisting of three Aqua-Pure API2T housings,with Parker 39R10A wound filter cartridges,in parallel. A condition of this approval involves the completion of a microscopic particulate analysis(MPA) if well depth is less than 100 feet. As your well is less than 100 feet deep,you arc-required to perform a MPA immediately and transmit the results to this office. Following completion of the required MPA,if your water is classified as ground water under the influence of surface water, you will need to maintain the water filtration system in conformance with the State of Colorado's requirements,and daily turbidity and chlorine residual measurements will need to be reported. Regardless of the outcome of the MIA,you will be required to continue to disinfect the water. The existing septic system is adequate for the proposed flows provided that no increase in sewage flows occur. An increase in sewage flows could occur from, but not be limited to the following: • An increase in food service area seating capacity above 60 seats • More than 158 meals per day served • Extended restaurant service hours • Elimination of single service/single use articles (paper) for all meals • Dance hall occupancy loads increasing over the 350 people per night • Discharge water from a Reverse Osmosis (R.O.)water treatment system In reviewing the engineer evaluation it was noted that the engineer states the existing 1000- gallon septic tank will be used as a grease trap and that an additional 1000-gallon septic tank will be installed for the kitchen flows. During the final inspection of the system it was noted that the existing 1000-gallon septic tank was removed and replaced by a new 1000-gallon tank. The additional 1000-gallon septic tank for the kitchen flows was never installed. This does not pose a problem because the required tank capacity for the proposed flows would be 3238 gallons and the restaurant currently has a tank capacity of 5250 gallons. HAND DELIVERED Mr. Ruben Rodriguez April 11, 2003 Page 2 The presence of nitrates in excess of the federal limits for drinking water in the well may require the installation of some type of remediation,most likely in the form of a R.O. system. These systems produce a large quantity of wastewater, which would need to be disposed of. The use of a subsurface disposal system is adequate for disposal of such waste provided it meets all the requirements of the Colorado Department of Public Health and Environment and the Weld County Individual Sewage Disposal System Regulations. In this particular case, it would be advisable to provide a separate system for the R.O. water treatment waste. COLORADO April 4, 2003 Ruben Rodriguez El Revention Restaurant 13015 WCR 16 Fort Lupton, CO 80621 RE: Revocation of Certificate of Occupancy Dear Mr. Rodriguez: A Certificate of Occupancy was issued by the Building Inspection Department for the remodel of the building located at 13015 WCR 16 into a restaurant and dance hall. This Certificate of Occupancy was issued in error. Prior to issuing the Certificate of Occupancy, all conditions of approval for the Site Plan Review (SPR) 341 were to be met. Two of these conditions required approval of the septic system and water systems which serve this building. To date these conditions have not been met. Attached to this letter is a request from the Department of Environmental Health Services for Weld County to revoke the Certificate of Occupancy for the restaurant and dance hall located at 13015 WCR 16, Fort Lupton, Colorado. The reasons stated for the revocation are that the septic system serving that building is not of adequate size and capacity to handle the load, and the water source and system have not been approved by the Colorado Department of Public Health & Environment. In accordance with Section 109.6 of the 1997 Uniform Building Code and Section 29-3-280 of the Weld County Code, I am declaring that the Certificate of Occupancy issued for the remodel of the Restaurant and Dance Hall located at 13015 W CR 16 for building permit number BC- 0100508 is revoked. This building shall not be open to the public until the septic and water systems have received final approval from the Weld County Department of Environmental Health Services. A new Certificate of Occupancy will be required prior to opening this building to the public. All conditions of approval will need to be met before this Certificate of Occupancy will be issued.
