Project: The section of gravel roadway is approximately 2.3 miles in length and begins at State Hwy 52 and extends
north to the 1-76 frontage road. General engineering services to be provided consist of Project Survey, Project
Development, Preliminary Design, Final Design, Preparation of Construction Plans,Project Special Provisions and
Standard Specifications, and Engineers Estimate.

The following change is hereby made to the Contract Documents:
Include additional survey of the area of WCR 51 and WCR 16. The additional survey shall include several irrigation
ponds on the north end of the project and survey information along Jim Creek.
CHANGE TO CONTRACT PRICE:
Original Contract Price: $ 63,760.50
Current Contract Price adjusted by previous Change Order: $ 63,760.50
The Contract Price due to this Change Order will be increased by: $ 2,500.00
The new Contract Price, including this Change Order, will be$ 66,260.50
CHANGE TO CONTRACT TIME:
The Contract Time will be increased by 0 calendar days. (Contract time will be suspended during this change)
The date for completion of all Work will be unchanged .
