Project Abstract: Please see Appendix F 2. Target Population: The Weld County Department of Public Health and Environment (WCDPHE) Abstinence Education Program (AEP) is designed to address Weld County adolescents, ages 11-18 by providing community-based educational programs that promote abstinence until marriage. The target population in Weld County is Hispanic youth ages 11-18, high risk teens, Weld County students and parents. There have been few abstinence education programs geared toward Hispanic youth. The AEP will focus efforts in 2004 on reaching this demographic because of their high risk nature. The 2001 data for Weld County reveals a teen fertility rate of 14.8 for 10-17 year olds. A major portion of these were among Hispanic girls. In 2001 77% of the births to teens 10-17 in were to Hispanic girls compared to 21.8% for White, non-Hispanic girls. Youth will be recruited through schools, faith based organizations, counseling offices, and medical offices for abstinence education and sexual cessation. Additionally, Hispanic girls will be recruited to participate in the Quinceafiera program,which is a cultural rights of passage mentoring program, and abstinence until marriage will be the focus. Parents and teachers will be recruited through schools, faith based organizations and the community. Weld County is Colorado's third largest and includes 31 primarily agricultural/rural communities totaling a population of 181,000, of which 70% are white, non-Hispanic and 27%are Hispanic. Greeley is the county seat and hosts 94,597 people. There are 31,000 students in grades K-12 (12,000 who are of Latino/Hispanic origin)who attend one of 13 school districts scattered throughout the county. After receiving feedback from the Hispanic community the WCDPHE -AEP identified factors that contribute to teen sex, STDs and teen pregnancy in Weld County, such as: cultural acceptance, sexual activity norms, disassociation of choices with consequences,misinformation, 3 family priorities, poor family support, lack of supervision, lower economics and a higher school drop out rate for Hispanic youth. There is little cultural or consistent support, skill building and encouragement for Hispanic teens to embrace the daunting task of remaining abstinent until marriage. There are two aspects to the WCDPHE-AEP that help fill gaps in services for two specific populations:Hispanic girls ages 11-14 and high risk sexually active teens. These programs provided by the AEP are the Quinceafiera program and the SAGE Advice medical model of sexual cessation. There are currently no known services for abstinence education that are geared directly toward Hispanic girls_ The families of first generation Hispanic girls may not currently benefit from the AEP's messages through the schools because of language and cultural bathers. The Quinceafiera Program will address gaps in information for families in Spanish. The SAGE Advice program will address gaps in counseling services directed toward sexually active teenagers. The WCDPHE -AEP has worked to foster relationships and partnerships with youth serving organizations in Weld County schools as well as organizations that work primarily with Hispanic populations such as the Cesar Chavez Cultural Center. In 2003, the AEP partnered with the Cesar Chavez Cultural Center,and the schools to provide abstinence education, speakers and incentives for the Latino and Latina Youth Leadership Conferences. A national abstinence speaker was brought in to compliment these activities, and the programs reached more than 400 Hispanic youth. Every opportunity will be afforded those individuals with physical limitation for all programming activities. Trainings and programs will be held in wheelchair accessible locations, and accommodations will be made for individuals who need other assistance. 3. Proposed Project Detail: The WCDPHE—AEP will be using the A-H compliant curriculum, WAIT Training, which was recently updated and republished and The Art of Loving Well 4 literature series for middle and high school youth. Both of these curricula were recently approved by the Greeley/Evans school district 6 for use in health education classes. These two programs will reach an expected to serve 3,000 adults, youth and educators. See Attachment A for an outline WAIT Training and Attachment B The Art of Loving Well curricula. Additional programs offered by the WCDPHE -AEP are the Quinceafiera Program (Please see Attachment C), which is expected to reach 300 youth and adults and the SAGE Advice medical model for sexual cessation(Please see Attachment D)which is expected to reach 300 youth and adults. There will also be activities surrounding Abstinence Awareness Week and Talk To Your Kids About Sex Week each expected to reach 400 youth and adults,parent educational opportunities expected to reach 300 adults and staff of the program will attend national and state conferences. The total cost per client is approximately$33.00 per client. The Quinceanera right of passage is for Hispanic girls turning 15 years old. In the Americanized culture, this ceremony is seen as permission for girls to participate in sexual activity. The original purpose of the ceremony is to welcome the girl into womanhood and promote purity until marriage. The Quinceafiera program will pilot in Greeley in 2004 and will teach abstinence until marriage,relationship and communication skills, and the steps to intimacy. The SAGE Advice medical model,based on the 5 A's of cessation, addresses sexual cessation and will reach high risk teens that are currently sexually active. There will be training available to mental health and medical professionals who treat youth, and technical assistance provided by the AEP staff for implementation. Additionally, there will be a support group formed that will meet at the WCDPHE for additional support for teens currently working toward changing their high risk behavior and will promote abstinence until marriage. Youth will be involved in planning an end of program event for the Quinceafiera Program, and forming the SAGE Advice support group. They will be involved in planning events for 5 Abstinence Awareness Week and Talk to Your Kids about Sex Week and will act as advisory members to plan school based activities as well as opportunistic events. The WCDPHE AEP collaborates with many agencies, organizations, schools and faith based organizations in Weld County. There is also collaboration with the Colorado Abstinence Relationship and Education, and many abstinence education programs across the state of Colorado and Nationally. There will be contact between the AEP and local organizations to provide services,maintain relationships and collaborate on projects such as the Latina Youth Leadership Conference. The program has a great relationship with the nursing staff of the WCDPHE and will continue to utilize their expertise to reach teens that come to the health department for services. There will be a referral process put into place to aid in recruitment and maintenance of the SAGE Advice support group. The WCDPHE - AEP falls under the umbrella of the HIPPA(Health Insurance Privacy and Portability Act)regulations as a health department entity and will be required to follow strict federal regulations regarding confidentiality. The WCDPHE Abstinence Education staff is primarily responsible for the planning implementing of the program. There will be opportunity for the Board of County Commissioners to have input on the program activities as well as the administration of the WCDPHE. Input will also be received from collaborating partners such as the Latina Women, the Cesar Chavez Cultural Center and other organisations. The faith based community will be encouraged to partner with the Abstinence Program to provide leadership within their organizations as well. Please see Attachment E. The AEP uses a pre/post evaluation design for WAIT Training classroom lessons and The Art of Loving Well lessons. There will be pre/post test designs for the Quineafiera Program and SAGE Advice as well as a tracking form for SAGE Advice to monitor the amount of time spent with clients. The program will use the unduplicated client forms as provided by the CDPHE. These 6 forms will track ages, ethnicity, and total numbers reached. The program will develop tracking forms to record the numbers present and time served at SAGE Advice events and counseling sessions as well as unduplicated client counts. 4. Methodology: The WCDPHE -AEP facilitates behavior change and knowledge change among youth and adults in many ways. WAIT Training and The Art of Loving Well are both educational curricula, and are compliant with the federal definition of abstinence education programs. Both promote character and resiliency in youth and help build skills youth will use to manage their sexual behavior until marriage. WAIT Training is currently being evaluated in Weld County by Prime Solutions, Inc. of Fort Collins. The Art of Loving Well was used and evaluated by Boston University in Boston, Massachusetts. The Quinceafiera Program involves parents, youth, faith based organizations and the community to not only teach abstinence until marriage,but to engage the community in collaboration and increase awareness and support for abstinence education in Weld County. The program is currently being evaluated by FRIENDS FIRST of Longmont, Colorado. SAGE Advice improves self-efficacy and conflict resolution, anger management and communication and refusal skills of youth. It was developed by Dr. Alma Golden,a pediatrician, currently with the Dept. of Health &Human Services in Washington,DC. The model is based on a the Stages of Change Health Model. The curriculum used for WAIT Training, The Art of Loving Well and SAGE Advice are written in English. There are Spanish speaking individuals who provide the education in Spanish for those with language barriers. The Quinceafiera program is primarily is Spanish because the population reached is largely Spanish speaking only. The programs are all used with auditory, visual and kinetic activities to provide for all students and families. 5. Goals, Objectives and Work plan 7 a. Project goals: 1).To pilot and evaluate a new training model of the FRIENDS FIRST Quinceanera Mentoring Program for Hispanic adolescent females ages 11-14 years old. 2). To train and equip health care providers in Weld County with necessary knowledge, skill and supportive material to counsel unmarried teens to maintain healthy sexual behaviors and change risky behaviors. 3).To reduce teen sexual activity and the subsequent consequences of adolescent pregnancy, out of wedlock births, sexually transmitted diseases and reduce the emotional pain associated with early, uncommitted sexual involvement in Weld County. b. Process objectives: 1).By December 31, 2004, host at least one (1) end of program event/celebration for abstinence commitment for Quinceaneras, their families and the community.2). By December 31, 2004,provide at least one (1)parent recruitment/education session regarding abstinence and sexual behavior of Hispanic teens in Spanish to parents in Weld County. 3).By December 31, 2004, 50% of health care professionals trained in the medical model of sexual cessation(SAGE Advice) will apply the strategies in their medical practices with adolescents in Weld County. 4).By December 31, 2004,the WCDPHE Abstinence Education Program will implement a support group for at risk adolescents using the medical model of sexual cessation (SAGE Advice)to promote health sexual behavior. 5).By December 31, 2004, 90% of middle/high school teachers who participate in the(A-H compliant curricula) the Art of Loving Well teacher training will agree to present at least five (5) lessons as part of health education classes in Weld County schools at least one (1)time per semester to promote a reduction in sexual activity among students. 6).By December 31, 2004,host at least one (1) Why Wait Now Training for counselors and faith based organizations who work with youth in Weld County. 7).By December 31, 2004,host at least one (1) activity for adults and one (1)for youth for Abstinence Awareness Week. 8). By December 31, 2004,host at least one (1)community event for Teach Youth Kids about Sex Week. 8 c. Impact Objectives: 1).By December 31, 2004, WCDPHE will increase by 25% the number of protective factors (Assets)in the lives of Hispanic adolescent females reached by the Quinceafiera program. 2). By December 31, 2004, 50%of the Hispanic adolescents attending the Quinceafiera program will commit or recommit to sexual abstinence until marriage after participating in 16 hours of abstinence until marriage education. 3). By December 31, 2004, at least 90%of the adolescents attending the SAGE support group through the WCDPHE will indicate their knowledge of healthy sexual behaviors has increased. 4). By December 31, 2004, at least 60% of Weld County teens who complete (A-H compliant curriculum) WAIT Training course curriculum will consider remaining or committing to abstinence from sexual behavior as a result of implementation of WAIT Training. d. Personal Services and Fringe Benefits-$85,820. 41: There are three positions, a .75 FTE, a .50 FTE and a .25 FTE employed with the AEP. There is an additional .05 supervisor and .05 administrative assistant that spend time on abstinence education projects. The funding for personnel including fringe benefits and $2,500 for contractual services for the evaluator is provided as matching from the WCDPHE. Quinceafiera Rite of Passage Mentoring Program- $8,100: Two sixteen hour rites of passage programs targeted for at least 40 Hispanic females ages 11-14 and their families. Development, advertising,recruitment and promotion for program and final event costs $1200. Forty Quinceanera handbooks/CD that provide abstinence education information and relationship building information @$37.50 cost $1500. Eight meeting room rentals and class materials/food 9 cost $1000. Forty purity rings @ $32.50 cost$1300. Education incentives cost$900. Stage rental, DJ/sound system, decorations, and video recording cost for sessions and end of program event $2200. SAGE Advice Medical Model of Sexual Cessation Healthcare Training-$4,800: One SAGE Advice training for at least 30 Weld County Healthcare professionals. Trainer fee/expenses cost $2,550. Capability of the Applicant: a. WCDPHE has been an organized health department since 1938. The AEP began at the health department in 1998. 11 b. The program staff has a combined 24 years of experience in teaching, providing abstinence education, program planning and implementation, program management, counseling and other skills. The remaining staff members are supervisory and administrative assistants who are also experienced in managing programs and providing support as needed. c. WCDPHE has computer hardware and software, Internet access and technical support available to each staff member. Meeting facilities are available onsite in Greeley and additionally at the south county health department location. The WCDPHE also provides a broad array of other health professionals including the director who is a medical doctor,nurses,health educators, dieticians, environmental health experts and assorted administrative support personnel who are willing to provide program and technical support on an ongoing basis. d. The AEP is housert at the WCDPHE in the health promotion and nursing division where there is already adequate internal structure in place to monitor program activities and keep track of program expenses.All equipment and resource needs have been addressed previously. There are 14 high schools and 16 middle schools hosting 70.5%white and 27% Hispanic youth of which many are high risk. 6. Briefly describe the program's services. The Weld County Abstinence Education program offers sexual cessation counseling to sexually active teens, is piloting a Quinceanera community based educational program to promote abstinence to Hispanic adolescent females and their families,provides abstinence education training for teachers and students, and finally parent education to provide up to date sexuality information in parent workshops. 7. Briefly describe how the program will monitor and evaluate its impact and process objectives. The WCDPHE will consult with Cindy Kronauge of Prime Solutions, Ft. Collins, Co, as the Abstinence Education Program's evaluator. There is currently a pre/post evaluation tool used in Weld County to assess the success of the curriculum in schools. Additionally, there will be a scan able format in both English and Spanish to evaluate the Quinceafiera program.
