RE: APPROVE TASK ORDER OPTION TO RENEW LETTER #3 FOR THE COLORADO
NURSE HOME VISITOR PROGRAM AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with Task Order Option to Renew Letter#3 for
the Colorado Nurse Home Visitor Program between the County of Weld, State of Colorado, by and
through the Board of County Commissioners of Weld County, on behalf of the Weld County
Department of Public Health and Environment, and the Colorado Department of Public Health and
Environment, commencing July 1, 2003, and ending June 30, 2004, with further terms and
conditions being as stated in said letter, and
WHEREAS, after review, the Board deems it advisable to approve said letter, a copy of
which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County, Colorado, that Task Order Option to Renew Letter#3 for the Colorado Nurse Home Visitor
Program between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized to
sign said letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted by
the following vote on the 21st day of May, A.D., 2003.

Task Order Renewal Letter Number 03, Contract Routing Number 04 FAA 00065
State Fiscal Year 2003-2004, Nurse Home Visitor Program
This Task Order Option to Renew Letter is issued pursuant to paragraph F_5.of the Master Contract identified as
contract routing number 00 FAA 00008 and paragraph D. of the Task Order identified as contract routing number
01 FAA 00942 and contract encumbrance number PO FAA FHS0100942. This Task Order Option to Renew Letter
is between the COLORADO DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT and BOARD
OF COUNTY COMMISSIONERS OF WELD COUNTY. The Task Order has been amended by Task Order
Renewal Letter 01 contract routing number 02 FAA 00183,Task Order Renewal Letter 02,contract routing number
03 FAA 00290 and Task Order Change Order Letter 01 contract routing number 03 FAA 00865. The Task Order,
as amended, if applicable,is referred to as the"Original Task Order". This Task Order Option to Renew Letter is
for the current term of July 1,2003 through June 30 2004. The maximum amount payable by the State for the
work to be performed by the Contractor during this current term is increased by Three Hundred Sixty-Six
Thousand Two Hundred Thirty-One Dollars ($366,231.00)for an amended total financial obligation of the State
of ONE MILLION ONE HUNDRED FIFTY-ONE THOUSAND ONE HUNDRED SIXTY DOLLARS AND
SEVEN CENTS ($1,151,160.07). The revised Budget is incorporated herein by this reference,made a part hereof,
and attached hereto as"Attachment J". The first sentence in Part C.of the Original Task Order is modified
accordingly. All other terms and conditions of the Original Task Order are reaffirmed. This change to the Task
Order shall be effective upon approval by the State controller,or designee,or on July 1,2003,whichever is later.
Please sign,date,and return all four(4)originals of this Task Order Option to Renew Letter by June 12,2003,to
the attention of: Peggy Becker,Colorado Department of Public Health and Environment,4300 Cherry Creek
Drive South,Mail Code PSD-HCP-A4,Denver,Colorado 80246-1530. One original of this Task Order Option
to Renew Letter will be returned to you when fully approved.

SUBJECT: Task Order Renewal Letter#3 for the
Colorado Nurse Home Visitor Program
Enclosed for Board review and approval is a task order renewal letter between the Colorado
Department of Public Health and Environment (CDPHE) and Weld County for the Colorado
Nurse Home Visitor Program.
Under the provisions of this renewal letter, WCDPHE will implement and provide county-wide
nurse home visitor services to low-income, first-time mothers as provided for in the Nurse Home
Visitor Program Act, section 25-31-101, et. Seq., C.R.S., as amended, and administered by the
CDPHE. For these services, Weld County will receive an amount not to exceed $366,231.00 for
the fourth term of the agreement, July I, 2003 through June 30, 2004. This funding will be used
to support health, education and other resources for new young mothers during pregnancy and
the first years of their infants' lives under program guidelines. WCDPHE will collaborate with
multiple community agencies in implementing this program.
