RE: APPROVE TASK ORDER FOR COLORADO PHYSICAL ACTIVITY AND NUTRITION
PROGRAM AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with a Task Order for Colorado Physical
Activity and Nutrition Program between the County of Weld, State of Colorado, by and through
the Board of County Commissioners of Weld County, on behalf of the Weld County Department
of Public Health and Environment, and the Colorado Department of Public Health and
Environment, commencing October 1, 2003, and ending September 30, 2008, with further terms
and conditions being as stated in said task order, and
WHEREAS, after review, the Board deems it advisable to approve said task order, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Task Order for Colorado Physical Activity and Nutrition
Program between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said task order.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 8th day of October, A.D., 2003, nunc pro tunc October 1, 2003.

Executive Summary.
Steps to a Healthier Colorado (STEPS) is a project meeting the Steps to a HealthierUS
requirements for State-Coordinated Small City and Rural Community Applicants. The
Colorado Department of Public Health & Environment (CDPHE) will coordinate STEPS
projects in four communities: Mesa County, Pueblo County, Teller County, and Weld
County.
Community intervention strategies are evidence based and include community level
interventions, environmental interventions, and educational interventions. The
Colorado STEPS project's planned short-term outcomes include: increased exposure of
community members, educators, health care professionals, and school-aged children to
education about disease prevention and self-management, healthy lifestyles, increased
availability of health care services and screening, and increased access to and
consumption of healthy foods. Major planned intermediate outcomes include: increased
awareness of the benefits of a healthy lifestyle, the risks of diseases and the advantages of
proper treatment for chronic diseases; more community residents engaging a healthier
lifestyle; increased access to healthier environments; and increased correct diagnosis of
and appropriate care for people with chronic diseases.

TASK ORDER
Colorado Physical Activity and Nutrition Program
This Task Order is made this 22nd day of September,2003,by and between: the state of Colorado,acting by and
through the COLORADO DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT,whose address or
principal place of business is 4300 CHERRY CREEK DRIVE SOUTH,DENVER,COLORADO 80246,
hereinafter referred to as"the State";and,the BOARD OF COUNTY COMMISSIONERS OF WELD COUNTY
fa political subdivision of the state of Coloradol,whose address or principal place of business is 915 10th Street,
3r°Floor,Greeley,Colorado 80631, for the use and benefit of the Weld County Department of Public Health
and Environment,whose address or principal place of business is 1555 North 17th Avenue,Greeley,Colorado
80631,hereinafter referred to as"the Contractor".
FACTUAL RECITALS
WHEREAS,the United State Department of Health and Human Services("USDHHS"),through the Centers for
Disease Control and Prevention(CDCP)has awarded the State federal funds under Notice of Cooperative
Agreement Award("NCAA")number U58/CCU823313-01 to perform Steps to a HealthierUS: A Community-
Focused Asthma,Diabetes,and Obesity Program, (see,Catalog of Federal Domestic Assistance("CFDA")number
93.283),Program Announcement Number 03135;
WHEREAS,the State has formulated a comprehensive State plan, with associated budgets,to disburse these funds
throughout the state of Colorado;
WHEREAS,under this comprehensive State plan,the State shall allocate these funds to qualified entities to provide
to the citizens of the state of Colorado on behalf of the State, community consortiums that focus on the prevention
and control of chronic disease and associated risk factors;
WHEREAS,as of the made date of this Contract, the State has a currently valid Group II purchasing delegation
agreement with the division of finance and procurement within the Colorado Department of Personnel and
Administration;
WHEREAS,section 29-1-203, C.R.S., as amended,encourages governments to make the most efficient and
effective use of their powers and responsibilities by cooperating and contracting with each other to the fullest extent
possible to provide any function, service, or facility lawfully authorized to each of the cooperating or contracting
entities;

WHEREAS,all state of Colorado contracts with its political subdivisions are exempt from the state of Colorado's
personnel rules and procurement code;
WHEREAS, the Contractor is a political subdivision of the state of Colorado. The State and the Contractor
mutually agree that the most efficient and effective way to provide the above-described services is at the local level.
The State and the Contractor previously entered into a Master Contract with contract routing number 02 FAA
00008. This Task Order is issued pursuant to the terms and conditions of that Master Contract.
WHEREAS,as to the State, authority exists in the Law and Funds have been budgeted, appropriated, and otherwise
made available, and a sufficient uncommitted balance thereof remains available for subsequent encumbering and
payment of this Contract in Fund Number 100,Appropriation Account 777,and Organization Number 7350,under
Contract Encumbrance Number PO FLA PPG0400154; and,
WHEREAS,all required approvals,clearances, and coordination have been accomplished from and with all
appropriate agencies.
NOW,THEREFORE, in consideration of their mutual promises to each other,stated below,the parties hereto
agree as follows:
A. PERIOD OF PERFORMANCE AND TERMINATION. The proposed effective date of this Contract is
September 22.2003. However,in accordance with section 24-30-202(1), C.R.S.,as amended,this
Contract is not valid until it has been approved by the State Controller,or an authorized designee thereof
If the underlying NCAA authorizes the State to pay all allowable and allocable expenses of a contractor as
of the effective date of that NCAA,then the State shall reimburse the Contractor for any allowable and
allocable expenses of the Contractor that have been incurred since the proposed effective date of this
Contract. If the underlying NCAA does not authorize the State to pay all allowable and allocable expenses
of a contractor as of the effective date of that NCAA,then the State shall only reimburse the Contractor for
those allowable and allocable expenses of the Contractor that were incurred on or after the effective date of
this Contract. The initial term of this Contract shall commence on its effective date and continue through
and including September 21,2003. If the State receives additional federal funding for the purposes of this
Contract,then this Contract shall automatically renew for the period covered by those additional federal
funds. This Contract is funded entirely with federal funds. Therefore,the State's liability for payment for
all services rendered,or goods supplied,under this Contract is contingent upon the State's receipt of those
federal funds. If the State does not receive all,or any part, of the federal funds necessary to pay its
obligations under this Contract,then the State may immediately amend or terminate this Contract without
further liability to the State. The total term of this Contract, including all automatic renewals,bilateral
options to renew,and extensions,may not exceed five(5)years.
B. DUTIES AND OBLIGATIONS OF THE CONTRACTOR. The Contractor, in accordance with the
terms and conditions of the Master Contract and this Task Order, shall perform and complete, in a timely
and satisfactory manner, all work items described in the Statement of Work and Budget, which are
incorporated herein by this reference,made a part hereof and attached hereto as"Attachment A".

Contractor agrees:
1. That any publications,journal articles,etc.,produced under this project must bear an
acknowledgement and disclaimer, as appropriate,that it was supported by the grant,citing the
name,cooperative agreement number,etc.,from the Centers for Disease Control and Prevention.
Its contents are solely the responsibility of the authors and do not necessarily represent the official
views of the Centers for Disease Control and Prevention.
2. To the greatest extent practicable, all equipment and products purchased with CDC funds should
be American-made.
3. When issuing statements,press releases,requests for proposals,bid solicitations and other
documents describing projects or programs funded in whole or in part with Federal money, all
awardees receiving Federal funds, including and not limited to State and local governments and
recipients of Federal research grants, shall clearly state(a)the percentage of the total costs of the
program or project which will be financed with Federal money;b)the dollar amount of Federal
funds for the project or program,and 3)the percentage and dollar amount of the total costs of the
project or program that will be financed by nongovernmental sources.
4. To comply with the"standard patent rights"clauses in 37 CFR 401.14.
5. To provide monthly reports due the first of each month which:
a. Describe the progress made toward each objective set forth in this Scope of Work;
b. Describe the activities and goals for completion over the following month;
c. Provide a detailed explanation for any activity or objective not completed
6. To complete staff hires by November 1, 2003, and provide proof of such hires CDPHE by
November 2,2003.
7. To provide scopes of work to CDPHE for all sub-contractors by December 1,2003.
8. Complete YRBS by December 30,2003 and provide proof of such completion to CDPHE by
December 31,2003.
9. Conduct their first health coalition meeting by January 30,2004 and provide proof of such
meeting to CDPHE by January 31,2003.
10. Meet the communication plan specified in the grant announcement and application.
11. Attend required trainings,conferences, workshops,etc. as outlined in the grant announcement,
grant application,or communication with CDPHE.

12. Complete a five(5)year action plan within eight(8)months of the start date of the contract,and
provide such action plan to CDPHE no later than May 31,2004.
C. DUTIES AND OBLIGATIONS OF THE STATE.
1. The Contractor shall be compensated in accordance with the rates set forth in Attachment A
hereto. In consideration of those services satisfactorily and timely performed by the Contractor
under this Task Order the State shall cause to be paid to the Contractor a sum not to exceed TWO
HUNDRED SIXTY-NINE THOUSAND NINE HUNDRED NINETY-FOUR DOLLARS,
($269,994.00)for the initial term of this Task Order. Of the total financial obligation of the State
referenced above, $269,994.00 are identified as attributable to a funding source of the United
States government and, $0.00 are identified as attributable to a funding source of the state of
Colorado.
2. Payments under this Task Order shall be made either through the State's Electronic Fund Transfer
system or,upon the Contractor's periodic submission of a duplicate"Task Order Reimbursement
Statement".
3. If this Task Order requires the Contractor submit a"Task Order Reimbursement Statement",then
the Contractor shall submit a signed, duplicate monthly Task Order Reimbursement Statement
within sixty(60)calendar days of the end of the billing period for which services were rendered.
A sample Task Order Reimbursement Statement is incorporated herein by reference,made a part
hereof, and attached hereto as"Attachment B". Expenditures shall be in accordance with those
items identified in Attachment A. These items may include,but are not limited to: the
Contractor's salaries, fringe benefits,supplies,travel,operating,and indirect costs which are
allowable and allocable expenses related to its performance under this Task Order.
Each Task Order Reimbursement Statement shall reference the related Matter Contract by its
contract routing number and this Task Order by their respective contract routing numbers. The
contract routing numbers are located on page one of these documents. Each Task Order
Reimbursement Statement shall also indicate the applicable performance dates, the names of
payees;a brief description of the services performed during the relevant performance dates; all
expenditures incurred; and,the total reimbursement requested. Reimbursement during the initial,
or any renewal,term of this Task Order shall be conditioned upon affirmation by the State that all
services were rendered by the Contractor in accordance with the terms of this Task Order. Each

4. The State may prospectively increase or decrease the amount payable under this Task Order
through a"Task Order Change Order Letter"that is substantially similar to the sample Task Order
Change Order Letter that is incorporated herein by this reference,made a part hereof,and attached
hereto as"Attachment C". To be effective,a Task Order Change Order Letter must be: signed by
the State and the Contractor;and,approved by the State Controller or an authorized designee
thereof. Additionally,a Task Order Change Order Letter shall include the following information:
A. Identification of the related Master Contract and this Task Order by their respective
contract routing numbers and affected paragraph number(s);
B. The type(s)of service(s)or program(s)increased or decreased and the new level of each
service or program;
C. The amount of the increase or decrease in the level of funding for each service or
program and the new total financial obligation;
D. A provision stating that the Task Order Change Order Letter is effective upon approval
by the State Controller, or designee, or its proposed effective date, whichever is later.
Upon proper execution and approval,a Task Order Change Order Letter shall become an
amendment to this Task Order. Except for the General and Special Provisions of the Master
Contract, and the Additional Provisions of the Task Order,if any,the Task Order Change Order
Letter shall supersede this Task Order in the event of a conflict between the two. It is expressly
understood and agreed to by the parties that the task order change order letter process may be used
only for increased or decreased levels of funding, corresponding adjustments to service or program
levels,and any related budget line items. Any other changes to this Task Order,other than those
authorized by the task order option to renew letter process described below,shall be made by a
formal amendment to this Task Order executed in accordance with the Fiscal Rules of the state of
Colorado.
If the Contractor agrees to and accepts a proposed Task Order Change Order Letter,then the
Contractor shall execute and return that Task Order Change Order Letter to the State by the date
indicated in that Task Order Change Order Letter. If the Contractor does not agree to and accept a
proposed Task Order Change Order Letter,or fails to timely return a partially executed Task Order
Change Order Letter by the date indicated in that Task Order Change Order Letter,then the State
may,upon written notice to the Contractor, terminate this Task Order no sooner than thirty(30)
calendar days after the return date indicated in the Task Order Change Order Letter has passed.
This written notice shall specify the effective date of termination of that Task Order. If a Task
Order is terminated under this clause,then the parties shall not be relieved of their respective
duties and obligations under that Task Order until the effective date of termination has passed.
Increases or decreases in the level of contractual funding made through the task order change
order letter process during the initial,or renewal,term of a Task Order may only be made under
the following circumstances:

E. If necessary to fully utilize appropriations of the state of Colorado and/or non-
appropriated federal grant awards;
F. Adjustments to reflect current year expenditures;
G. Supplemental appropriations, or non-appropriated federal funding changes resulting in an
increase or decrease in the amounts originally budgeted and available for the purposes of
a Task Order;
H. Closure of programs and/or termination of related contracts or task orders;
I. Delay or difficulty in implementing new programs or services; and,
J. Other special circumstances as deemed appropriate by the State.
5. The State may renew a Task Order through a"Task Order Option to Renew Letter"substantially
similar to the sample Task Order Option to Renew Letter that is incorporated herein by this
reference,made a part hereof, and attached hereto as"Attachment D". To be effective, a Task
Order Option to Renew Letter must be: signed by the State and the Contractor; and,approved by
the State Controller or an authorized designee thereof. Additionally, a Task Order Option to
Renew Letter shall include the following information:
A. Identification of the related Master Contract and that Task Order by their respective
contract routing numbers and affected paragraph number(s);
B. The type(s)of service(s)or program(s), if any, increased or decreased and the new level
of each service or program for the renewal term;
C. The amount of the increase or decrease, if any,in the level of funding for each service or
program and the new total financial obligation;
D. A provision stating that the Task Order Option to Renew Letter is effective upon
approval by the State Controller, or designee,or its proposed effective date, whichever is
later.

Upon proper execution and approval,a Task Order Option to Renew Letter shall become an
amendment to this Task Order. Except for the General and Special Provisions of the Master
Contract, and the Additional Provisions,if any of that Task Order, a Task Order Option to Renew

date indicated in that Task Order Option to Renew Letter. If the Contractor does not agree to and
accept the proposed renewal term,or fails to timely return a partially executed Task Order Option
to Renew Letter by the date indicated in that Task Order Option to Renew Letter,then the State
may,upon written notice to the Contractor,terminate this Task Order no sooner than thirty(30)
calendar days after the return date indicated in the Task Order Option to Renew Letter has passed.
This written notice shall specify the effective date of termination of that Task Order. If a Task
Order is terminated under this clause,then the parties shall not be relieved of their respective
duties and obligations under that Task Order until the effective date of termination has passed.
6. All attachments or exhibits to this Task Order are incorporated herein by this reference and made a
part hereof as if fully set forth herein. If a conflict or inconsistency is found to exist between the
terms and conditions of this Task Order and those of any attachment or exhibit hereto,then the
terms and conditions of this Task Order shall control.

ATTACHMENT A
WELD COUNTY DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT
SCOPE OF WORK AND BUDGET
Objective 1: Increase awareness, among 3,000 adolescents and adults, of health
complications that result from being overweight/obese and work towards decreasing obesity
prevalence in Weld County by September 2004. (Baseline: 2000 residents currently served)
Actions: 1. Introduce the Colorado On The MoveTM program using pedometers to two large
worksites, per year, in Greeley, with at least 20% of the employees participating at each
company by September 2004 through 2008. 2. Develop and implement a walking program at
both the Greeley and Evans Senior Centers incorporating the pedometers and Colorado On The
MoveTM with at least 20% of the members participating by September 2004 through 2008. 3.
Work with major worksites to encourage the use of local fitness centers and implement overall
wellness programs by September 2004. 4. Work with major worksites to implement point of
decision prompts by September 2004. 5. Provide three six-week nutrition education and cooking
classes on reducing fat and increasing fruits and vegetables into a healthy diet to parents of
participants of the after school nutrition and fitness program, spring and summer semesters, by
September 2005. 6. Develop a multi-faceted campaign to promote the 5 A Day program working
with restaurants, the farmer's market, and major worksites by September 2006. 7. Develop and
implement guidelines to encourage offering healthy snacks and foods in vending machines in
schools, recreation centers and adolescent congregations by September 2007. 8. Work with local
restaurants to provide nutrition and portion size information to patrons by September 2008.
Objective 2: Increase both the proportion of adults diagnosed with diabetes in Weld County
and provide increased care to at least 1000 by September 2004. (Baseline: 10,218 current
adults with diabetes)
Actions: 1. Conduct a media campaign to inform residents of the screenings available and to
educate adults with diabetes on the necessity of annual foot and dilated eye examinations by
September 2004 through 2008. 2. Train at least 70 health care professionals on office-based
procedures for foot screenings, teaching blood glucose monitoring, and insulin administration
techniques by September 2004 through 2006. 3. Collect data on the number of adults with
diabetes who currently receive an annual foot and dilated eye exam by September 2004.
4. Increase the proportion of adults with diabetes in Weld County who have a foot examination
by 300 by September 2005. 5. Increase to at least 20%the proportion of adults with diabetes who
engage in moderate physical activity for at least 30 minutes, five or more days per week by
September 2005. 6. Increase the proportion of adults with diabetes in Weld County who receive
at least two A1C measures per year by 250 by September 2006. 7. Provide dilated eye exams for
120 low-income individuals with diabetes in Weld County, who do not have vision insurance by
September 2007. 8. Offer American Diabetes Association self assessment risk tool Take the Test,
Know Your Score to senior centers, worksites and community groups by September 2008. 9.
Offer four cooking classes for people with diabetes and their families specializing in healthy
menu choices by September 2008.
Objective 3: Increase by 20% the number of Weld County residents who receive education
about treatments, resources, and causes of asthma by September 2004. (Baseline: To be
determined)[Banner Health, WDPHE Tobacco Program Coordinator]
Actions: 1. Collect data on numbers of children and adults diagnosed with asthma from schools,
community health centers and local PCPs by September 2004 through 2008. 2. Increase by 20%
over the data collected in Action#1 the number of persons with asthma who receive formal
patient education, including information about community resources, as an essential part of the
management of their condition by September 2004 through 2008. 3. Provide schools, community
health centers and local PCPs with educational in-service opportunities using best practices to
identify appropriate diagnosis, degree of the disease and the effectiveness of medication control
by September 2005. 4. Increase by 150 the number of adult Weld County Smokers making quit
attempts by September 2004 through 2008. 5. Work with at least three communities in Weld
County on a comprehensive clean indoor air ordinance, which protects the public from
secondhand smoke in public places and the workplace by September 2006. 6. Visit at least 50
Weld County tobacco retailers to disseminate educational information about youth access laws
by September 2007. 7. Promote local and state cessation resources to 30 worksite/businesses in
Weld County, with at least five of these sites being Latino businesses by September 2008.
Effectiveness. The WCDPHE intends to allow the differences between agencies to continue to
benefit the community while leveraging scarce resources and information to increase capacity for
preventive chronic disease services. It is the goal of the WCDPHE to build on and complement,
not duplicate, existing programs.
Sustainability Plan. The budget estimates the key partners in the STEPS project will donate in-
kind funding in the amount of$120,000. Many of the proposed projects will become a part of
the participating organizations' operating budgets and, therefore, funded from patient revenues
and traditional grant sources. The grant manager will continue to leverage local, state and
federal funds to support those projects not absorbed by operating budgets.
WELD SCHOOL INTERVENTION PLAN
Objective 1: Increase awareness, among 2,000 adolescents of health complications that result
from being overweight/obese and work towards decreasing obesity prevalence in Weld County
by September 2004. (Baseline: approximately 18,000 students in Weld County School District
6)[School District 6]
Actions: 1. Introduce the TAKE 10! Program into at least four elementary schools in the Weld
County School District 6 with at least 50% of the classrooms participating in each school by
September 2004. 2. Implement the Nu-Fit Program in four Weld County School District 6 after
school programs providing education and skill building activities on physical activity and
nutrition twice a week at each site by September 2004. 3. Implement the Nu-Fit Program in one
Weld County School District 6 summer program providing education and skill building activities
on physical fitness and nutrition twice a week by September 2004. 4. Establish a Nutrition
Advisory Committee for Weld County School District 6 by September 2005. 5. Provide
nutrition services and access to variety of nutritious meals which reflect the Dietary Guidelines
for Americans in all elementary and middle schools in Weld County School District 6 by
September 2005.
6. Establish a school nutrition program that limits the availability of foods high in fat, sodium,
and added sugars in all schools in Weld County School District 6 and alter the food environment
in Weld County School District 6 by making healthy food an easy, less expensive, and a
desirable choice by September 2005. 7. Serve wheat breads instead of white breads in all Weld
County School District 6 schools by September 2006. 8. Limit the number of a la carte items
offered in all Weld County School District 6 schools to only include foods, which reflect the
Dietary Guidelines for Americans by September 2006. 9. Increase the number and kinds of fresh
fruits and vegetables offered daily at the elementary, middle, and high schools sites to at least
three times a week by September 2007. 10. Implement a marketing campaign on 5 A Day in all
District 6 schools the third week of September 2004 for the entire week by September 2007.
11. Increase knowledge of fruits, vegetables and grains, preparation methods and benefits of
eating five or more servings daily by youth ages 6— 13 participating in the after school program,
the Nu-Fit Program, by providing nutrition education and food preparation classes by September
2008. 12. Increase knowledge and skills for consuming a diet with less than 30% calories from
fat by providing nutrition education and food preparation classes to the participants, ages 6— 13,
in the after school program, the Nu-Fit Program by September 2008.
Objective 2: Provide increased care to at least 250 diagnosed students with diabetes by
September 2004. (Baseline: To be determined[School District 61)
Actions: 1. Conduct a media campaign to inform residents of the screenings available and to
educate people with diabetes on the necessity of annual foot and dilated eye examinations by
September 2004 through 2008. 2. Train at least 10 school nurses about blood glucose monitoring
and insulin administration techniques by September 2004 through 2008.
Objective 3: Increase by 20% the number of Weld County adolescents who receive education
about treatments, resources, and causes of asthma by September 2004. (Baseline: To be
determined)[Banner Health, School District 6, WDPHE Tobacco Coordinator]
Actions: 1. Identify adolescents who have been diagnosed with asthma from schools, community
health centers and local PCPs by September 2004 through 2008. 2. Increase the proportion of
adolescents with asthma who receive formal patient education, including information about
community resources, as an essential part of the management of their condition by September
2004 through 2008. 3. Provide schools, community health centers and local PCPs with
educational in-service opportunities using best practices to identify appropriate diagnosis, degree
of the disease and the effectiveness of medication control by September 2004 through 2008. 4.
Increase smoke-free and tobacco-free environments in schools, including all school facilities,
property, vehicles, and school events by September 2004 by marketing evidence-based tobacco
reduction and cessation programs to schools. 5. Conduct at least five CDC Got A Minute
presentations at various schools, parent organizations or community organizations in Weld
County by September 2005. 6. Present Tobacco Free Weld County presentation to at least four
Educational staff development meetings regarding Alternative to Suspension, Not On Tobacco,
video resource library, and other programs at the WCDPHE by September 2007. 7. Increase to
23% the proportion of Weld County School Districts with one or more schools offering an
Alternative to Suspension program for students facing suspension for violating their schools
tobacco policy by September 2008.
Effectiveness. Because of the size of Weld County School District 6, approximately 18,000
students, the effects of the STEPS project in conjunction with the Improving the Health
Education and Well-Being of Colorado's Young People grant, recently received by the Colorado
Department of Education, will improve student health dramatically in large numbers of students.
Another factor that will significantly enhance the impact of these two major initiatives is the fact
that the Executive Director of the WCDPHE is currently the Greeley/Evans School Board
President and wholeheartedly supports efforts focused towards preventing chronic diseases.
Sustainability Plan. Many of the proposed school-based projects will require policy changes,
which will then be implemented with no need for future funding. The grant manager will
continue to leverage local, state and federal funds to support those projects, which are not policy
changes.

Department of Public Health and Environment's application for the Health &Human Services
Steps Grant. The total amount of award is $1.35 million for the funding period of October 1,
2003 through September 30, 2008. This grant has been awarded and has a five-year project
period, with the initial year's funding amount of$269,994.
Weld County Department of Public Health and Environment will use this funding for a 1.0 FTE
Project Coordinator, a 1.0 FTE School Health Coordinator, a 0.5 FTE Tobacco Coordinator, and
a 0.5 FTE administrative assistant.
