RE: APPROVE APPLICATION FOR TOBACCO PREVENTION PROGRAM GRANT AND
AUTHORIZE CHAIR TO SIGN -AMERICAN CANCER SOCIETY, ROCKY MOUNTAIN
DIVISION, INC.
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS,the Board has been presented with an Application for the Tobacco Prevention
Program Grant from the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County,on behalf of the Department of Public Health and Environment,to
the American Cancer Society, Rocky Mountain Division, Inc., commencing August 1, 2003, and
ending August 31,2004,with further terms and conditions being as stated in said application,and
WHEREAS,after review,the Board deems it advisable to approve said application,a copy
of which is attached hereto and incorporated herein by reference.
NOW,THEREFORE, BE IT RESOLVED by the Board of County Commissioners of Weld
County,Colorado,that the Application for the Tobacco Prevention Program Grant from the County
of Weld, State of Colorado, by and through the Board of County Commissioners of Weld County,
on behalf of the Department of Public Health and Environment, to the American Cancer Society,
Rocky Mountain Division, Inc., be, and hereby is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be,and hereby is,authorized to
sign said application.
The above and foregoing Resolution was,on motion duly made and seconded,adopted by
the following vote on the 18th day of June, A.D., 2003.

Step Two: Narrative
Provide a detailed description of the proposed program, including planned activities,the
target audience, the timeline, and the individual or individuals that will be responsible for
carrying out the program(use up to three additional pages if necessary).
The Tobacco-Free Weld County Program (TFWCP) has been actively involved in
the prevention and reduction of tobacco use among adults and youth in Weld County since
the 1980's. The TFWCP, a part of the Weld County Department of Public Health and
Environment (WCDPHE) uses a comprehensive approach to tobacco prevention based on
"Best Practices for Comprehensive Tobacco Prevention Programs" as recommended by the
Centers for Disease Control and Prevention (CDC). On a monthly basis the TFWCP
collaborates with other community tobacco prevention advocates such as the American
Cancer Society (ACS) and a number of other organizations and volunteers who form the
Weld County Tobacco Prevention Coalition. The TFWCP focuses on the four
recommended goal areas: 1) Preventing the initiation of tobacco use among young people,
2) promoting cessation among young people and adults, 3) reducing exposure to
secondhand smoke, and 4) identifying and eliminating the disparities related to tobacco use.
For the purpose of this application the TFWCP is requesting funding from the Rocky
Mountain Division of the American Cancer Society (ACS) to further tobacco prevention in
our community by working on the following ACS 2015 Nationwide Objective: By 2015,
reduce to 12%the proportion of adults (18 and older)who use tobacco products.
The TFWCP will use funding available from Colorado's Master Tobacco Settlement
to meet the requirement of a 20% match for the $12,500 being requested by this proposal.
The TFWCP has a strong belief that there are many direct benefits that can be achieved in
Weld County by using this funding to help reduce the proportion of adults (18 and older)
who use tobacco products. One of the key benefits is that adults who quit using tobacco
products greatly reduce their chances for having a tobacco related cancer such as lung
cancer (the leading cancer cause of death for women). Quality of life is also greatly
improved for adults who quit using tobacco products. For family members such as children
exposed to secondhand smoke quality life can be improved when adult family members
quit. By quitting adults can reduce the likelihood that their children will follow in their
footsteps by using tobacco products, additionally they can reduce the number of childhood
illnesses among children exposed to smoke. For adult tobacco users that are pregnant
quitting can benefit both the mother and reduce the chances for having a low birth-weight
infant. Finally becoming a non-tobacco user can reduce the burden of paying expensive
health insurance premiums that can be a burden to both the individual and their family.
The TFWCP plans to work on reducing the number of adult tobacco users over the
next year by using the following comprehensive approach: 1) through the use of educational
media messages to increase public awareness on the positive health benefits associated with
non-tobacco use behavior. 2) through the use of educational media messages to promote
local tobacco cessation resources such as the ACS "You Can Quit" class and the free
Colorado Ouitline and Quitnet, 3) through distributing educational materials on quitting
tobacco to local healthcare providers, businesses and at community events. 4) through
educational media messages about the benefits of quitting to protect your children, family

and friends from secondhand smoke hazards/exposure. The TFWCP believes this
comprehensive approach will increase help move more adult tobacco users towards
considering quitting and actually taking action to quit.
Media and educational materials used as part of the program will be focused at
reaching the target population that consist of adults ages 18 and older. English and Spanish
ads and educational materials will be used as appropriate to reduce any disparities within
the population. If awarded the program will begin activities as soon as the funds are
received and work to provide the comprehensive program activities on a monthly basis over
the yearlong grant period. Educational media activities will consist of running print ads in
local newspaper such as the Greeley Tribune, business newsletters, the Greeley City Leisure
Connection activity guide, Fin de Semana and Detalles Spanish_publications, University of
Northern Colorado (UNC) Minor and others. Radio messages on quitting will be used on
local radio stations such as KFKA, KGRE and K99. Educational materials such as posters
to advertise the Colorado Quitline, Quitnet and the local ACS "You Can Ouit" Class will be
distributed along with educational brochures and materials to local healthcare clinics such
as the North Colorado Medical Center, Sunrise Clinic, Monfort Children's Clinic, First
Steps Prenatal Program, Weld County Department of Public Health and Environment
Clinic, and a number of other medical and dental clinics. Healthcare providers will be
encouraged to council each client and provide the educational resources to adults who use
tobacco. Similar educational posters and brochures will be distributed to local area
businesses and organizations with lots of employees such as ConAgra, Metal Container
Corporation, Kodak, State Farm, UNC, Greeley City, Weld County, and others. Educational
brochures on quitting will also be distributed at the local events such as health fairs,
parenting conferences and others. Parents at events will be encouraged to quit smoking and
also to step outside to smoke until they quit so as not to expose others to their secondhand
smoke.
Each of the program activities will be carried out by existing tobacco prevention
staff funded by Colorado Master Tobacco Settlement funding so as to maximize the reach
of the funding awarded by the ACS and to compliment other tobacco prevention efforts
being funded in the community to reach the largest number of tobacco users.

Printing will include flyers to promote quit resources and classes and for a replacement
color printer ink cartridges as needed. Educational items will include mainly professionally
developed brochures, posters, Quit class materials, self help materials and healthcare
provider educational materials. The travel budget allows for traveling 30 miles per week A
$0.34 per mile. Advertising will include mainly print ads both in English and Spanish
within local county papers, newsletter, magazines, and some local radio spots. Postage and
shipping will also be used to distribute educational materials within the 4000 square miles
of Weld County.
Evaluation
At the conclusion of the project,please provide a detailed account of the outcome(success)of the
project, a final budget detailing how the grant money was spent,and whether you would
recommend this project for duplication in other communities. Submit as a separate document to:

The STEPP program is happy to submit this letter of support for your grant to the
American Cancer Society, Rocky Mountain Division. I understand that the application
requires matching funds for consideration. The funds awarded to the Weld County
Health Department through the Community Programs for Comprehensive Tobacco
Control Programs for 2003-2004 maybe used as a match. One of the general goal areas
within the ACS application applies to STEPP's overall tobacco prevention goals; reduce
the proportion of adults (18 and older) who use tobacco products.
The contract between Weld County Health Department and the Colorado Department of
Public Health and Environment for 2003-2004 in the amount of$99,969 is anticipated to
start on July 1, 2003.

SUBJECT: Proposal for Additional Tobacco Prevention Funding
for Weld County(Tobacco-Free Weld County
Program)
Enclosed for board review and approval is a grant application to the American Cancer Society,
Rocky Mountain Division. This grant if awarded would provide an additional $12,500.00 to the
Tobacco-Free Weld County Program. The program would use $2,500.00 from the Master Tobacco
Settlement grant already awarded to meet the 20% matching requirement of the application. The
State Tobacco Education and Prevention Partnership who awarded the funding has provided a
letter of support to use the matching funds. No additional staff will be requested, but funding will
be used to strengthen the existing program allowing for increased tobacco prevention education in
the county.
This grant will provide funding for a comprehensive approach to reducing the number of adult
tobacco users in the county. Specifically it will help provide funding for educational materials and
resources on quitting tobacco, which will be distributed to businesses, healthcare clinics, and a
variety of other places and events in the county. Local media will be used to promote low cost or
free tobacco cessation programs available in the county. The program will target both English and
Spanish speaking residents in the county.
The term of the grant if awarded, will be for a one-year period. It is anticipated that the program
would be awarded the funding for the period of August 1, 2003 —August 31, 2004.
