RE: APPROVE CHANGE ORDER LETTER#2 FOR WEST NILE VIRUS TASK ORDER
AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Change Order Letter#2 for the West
Nile Virus Task Order between the County of Weld, State of Colorado, by and through the Board
of County Commissioners of Weld County, on behalf of the Weld County Department of Public
Health and Environment, and the Colorado Department of Public Health and Environment,
commencing July 1, 2003, and ending June 30, 2004, with further terms and conditions being as
stated in said change order, and
WHEREAS, after review, the Board deems it advisable to approve said change order, a
copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Change Order Letter#2 for the West Nile Virus Task Order
between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby is,
approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said change order.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 8th day of September, A.D., 2003, nunc pro tunc, July 1, 2003.

Task Order Change Order Letter Number 2 Contract Routing Number 04 FHA 00124
State Fiscal Year 2004-2004, Disease Surveillance Program
This Task Order Change Order Letter is issued pursuant to paragraph F.3.of the Master Contract identified as contract
routing number 00 FAA 00008 and paragraph E_1.of the Task Order identified as contract routing number 01 FHA 00076
and contract encumbrance number PO FHA EPI0100076. This Task Order Change Order Letter is between the
COLORADO DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT and WELD COUNTY
DEPARTMENT OF PUBLIC HEALTH AND ENVIRONMENT. The Task Order has been amended by Task Order
Option to Renew Letter 1 contract routing number 02 FHA 00059,and Task Order Option to Renew Letter 2 contract
routing number 03 FHA 00062 Task Order Change Order Letter 1,contract routing number 03 FHA 00124 and Task Order
Option to Renew Letter 3 contract routing number 04 FHA 00072. The Task Order,as amended,if applicable,is referred to
as the"Original Task Order". This Task Order Change Order Letter is for the current term of July 1,2003,through June
30,2004. The maximum amount payable by the State for the work to be performed by the Contractor during this current
term is increased by EIGHTY FIVE THOUSAND DOLLARS ($85,000.00)for an amended total financial obligation of
the State of TWO HUNDRED FIFTY FIVE THOUSAND THREE HUNDRED SEVENTY TWO DOLLARS
($255,372.00). The revised specifications to the original Scope of Work and the revised Budget are incorporated herein by
this reference,made a part hereof,and attached hereto as"Attachment A". The first sentence in paragraph C of the Original
Task Order is modified accordingly. All other terms and conditions of the Original Task Order are reaffirmed. This change
to the Task Order shall be effective upon approval by the State controller,or designee,or on August 26,2003 whichever is
later.

West Nile Virus Prevention & Control Activities
Scope of work:
1. The Contractor shall use all funds it receives from the State from the supplemental
Notice of Cooperative Agreement Award (NCAA) only for those authorized purposes
identified in the NCAA. These authorized purposes are for West Nile Virus
emergency control measures that include, but are not limited to:
• adulticide spraying of mosquitoes in the most heavily impacted regions of the
state
• purchase of equipment and supplies as needed to meet the requirements of this
emergency control measure
• retroactive and proactive spraying costs
• surveillance and human case investigation activities
2. The Contractor shall maintain and make available upon request, to the State,
applicable receipts of purchases related to those activities.

SUBJECT: Change Order Letter#2 to Task Order for
West Nile Virus Surveillance Program
Enclosed for Board review and approval is Change Order Letter#2 to the task order between
the Colorado Department of Public Health and Environment (CDPHE) and Weld County for
the West Nile Virus Surveillance Program.
This change order letter provides additional one-time funding in the amount of$85,000
from the Notice of Cooperative Agreement Award (NCAA) for West Nile Virus emergency
control measures. These measures include, but are not limited to adulticide spraying of
mosquitoes, purchase of equipment and supplies, retroactive and proactive spraying costs,
and surveillance and human case investigation activities. If approved, this letter will bring the
total amount of State funding for this program to $145,560 for the time period July 1, 2003
through June 30, 2004.
I recommend your approval of this change order letter.
