RE: APPROVE TASK ORDER CHANGE ORDER LETTER#2 FOR TUBERCULOSIS
PROGRAM AND AUTHORIZE CHAIR TO SIGN
WHEREAS, the Board of County Commissioners of Weld County, Colorado, pursuant to
Colorado statute and the Weld County Home Rule Charter, is vested with the authority of
administering the affairs of Weld County, Colorado, and
WHEREAS, the Board has been presented with Task Order Change Order Letter#2 for
the Tuberculosis Program between the County of Weld, State of Colorado, by and through the
Board of County Commissioners of Weld County, on behalf of the Weld County Department of
Public Health and Environment, and the Colorado Department of Public Health and
Environment, commencing July 1, 2002, and ending June 30, 2003, with further terms and
conditions being as stated in said task order change order letter, and
WHEREAS, after review, the Board deems it advisable to approve said task order
change order letter, a copy of which is attached hereto and incorporated herein by reference.
NOW, THEREFORE, BE IT RESOLVED by the Board of County Commissioners of
Weld County, Colorado, that the Task Order Change Order Letter#2 for the Tuberculosis
Program between the County of Weld, State of Colorado, by and through the Board of County
Commissioners of Weld County, on behalf of the Weld County Department of Public Health and
Environment, and the Colorado Department of Public Health and Environment be, and hereby
is, approved.
BE IT FURTHER RESOLVED by the Board that the Chair be, and hereby is, authorized
to sign said task order change order letter.
The above and foregoing Resolution was, on motion duly made and seconded, adopted
by the following vote on the 13th day of January, A.D., 2003, nunc pro tunc July 1, 2002.

Task Order Change Order Letter Number 02, Contract Routing Number 03 FHA 00173
State Fiscal Year 2002-2003, Tuberculosis Procram
This Task Order Change Order Letter is issued pursuant to paragraph F. of the Master Contract identified
as contract routing number 00 FAA 00008 and paragraph.E_1. of the Task Order identified as contract
routing number 01 FHA 00158 and contract encumbrance number PO FHA EPI 0100158. This Task
Order Change Order Letter is between the COLORADO DEPARTMENT OF PUBLIC HEALTH AND
ENVIRONMENT and,the Board Of County Commissioners Of Weld County for the use and benefit
of, the Weld County Department of Public Health and Environment. The Task Order has been
amended by Task Order Option to Renew Letter 01,contract routing number 02 FHA 00036 and Task
Order Change Order Letter 01 contract routing number 02 FHA 00156 and Task Order Option to Renew
Letter 02 contract routing number 03 FHA 00036. The Task Order,as amended,is referred to as the
"Original Task Order". This Task Order Change Order Letter is for the current term of January 10,
2003 through June 30 2003. The maximum amount payable by the State for the work to be performed by
the Contractor during this current term is increased/decreased by FOUR THOUSAND THREE
HUNDRED FIFTY-SEVEN Dollars,f$4357.00)for an amended total financial obligation of the State of
ONE HUNDRED NINETY FIVE THOUSAND, FIVE HUNDRED EIGHTY-SEVEN DOLLARS
($195,587.00). The revised Budget are incorporated herein by this reference,made a part hereof,and
attached hereto as"Attachment A" . The first sentence in paragraph C. of the Original Task Order is
modified accordingly. All other terms and conditions of the Original Task Order are reaffirmed. This
change to the Task Order shall be effective upon approval by the State controller,or designee,or on
January 10,2003 whichever is later.
Please sign,date,and return all 3 originals of this Task Order Change Order Letter by January 2,2003,to
the attention of:Marti Wood,Colorado Department of Public Health and Environment,4300 Cherry
Creek Drive South,Mail Code DCEED TB A3,Denver,Colorado 80246. One original of this Task
Order Change Order Letter will be returned to you when fully approved.

SUBJECT: Change Order Letter Number 2 to the
Tuberculosis Control and Outreach Task
Order
Enclosed for Board review and approval is Change Order Letter Number 2 to the Tuberculosis
Control and Outreach Task order between the Colorado Department of Public Health and
Environment and Weld County.
Under the provisions of the task order, WCDPHE will provide TB treatment, preventive
treatment, provide for and interpret chest x-rays and other medical evaluation services, conduct
contact investigations and TB skin test screening programs. WCDPHE will also follow-up with
active patients to ensure they receive appropriate information and education and they adhere to
the completion of their TB therapy.
Through this change order letter, CDPHE is providing additional funding in the amount of
$4,357 for outreach activities. This funding will bring the total reimbursement for the time
period July 1, 2002 through June 30, 2003 to $95,792.
I recommend your approval of this change order letter.
